In the Netherlands endoscopists are obligated to register their Endoscopic Retrograde Cholangiopancreatography (ERCP) performances in a National ERCP quality registration (NEQR). NEQR is a self-assessment that evaluates ERCP quality and volume per centre and endoscopist, but complications and outcome of the procedure are both not registered in NEQR. The aim of this study is to examine whether NEQR correlates with objective post-ERCP clinical outcome in a tertiary referral centre. Material and methods: A retrospective cohort study of all ERCPs performed at a tertiary referral centre in 2016. Correlations between NEQR self-assessments and post-ERCP complications and mortality were measured. Results: 333 procedures were performed, of which 254 were registered in the NEQR (76.3%). No significant correlations were found between the subjective self- assessment and objective post-ERCP outcomes. Of all 333 procedures the total post-ERCP complication rate was 13.5% and the ERCP-related 30-day mortality rate was 0.6%. Conclusions: The currently used NEQR does not correlate with post-procedural clinical outcomes. {"title":"Suspected veterinary poisoning cases: A retrospective toxicoepidemiology study (2009-2019) in south-eastern Italy","authors":"D. Suspected poisoned animals (SPAs) and supposed poisoning baits (SPBs) were collected between 2009 and 2019 by public health authorities, veterinary practitioners and owners and were analyzed for the presence of rodenticide (non-anticoagulant rodenticides and coumarin derivatives), molluschicide, herbicides, pesticide and other molecules (ethylene glycol and metals). Among SPAs analyzed, small animals were prominent (76.2%). In totally, 23.7% animals (408/1719) and 47.3% baits (224/474) were found positive. Small animals were positive in 26.0% of cases (341/1310) and the molecule more involved was metaldehyde, a molluschicide. Only 11.4% (16/123) farm animals were found positive prevalently for rodenticide and pesticide. Though wild ones 18.5% (53/286) were positive mainly for pesticide. This examination due to their procedures can generate some anxiety in patients that can disturb the good performance of the exam. The main objective of this study is to perform an analysis of patient`s undergoing myocardial perfusion scintigraphy experience, assess patient`s anxiety and check Nuclear Medicine Department communication and information effectiveness. Material and methods: For this study, State-Trait Anxiety Inventory (STAI) and two Scan Experience Questionnaires were used. Sixty patients answered the questionnaires before and after myocardial perfusion scintigraphy. Results: Patients reported medium levels of scan-related anxiety before the myocardial perfusion scintigraphy. After the scan, there is a slight decrease in patients` anxiety. Results of the scan, radiation used, duration of the scan and the injection of the radiopharmaceutical are the most relevant patients` concerns before the scan. 15% of the sample did not receive the necessary information or the most effective way. Conclusions: The results suggest that there are several factors that tend to trigger patient`s experience and anxiety. Therefore, in this study, we aimed to determine the effects of sofosbuvir-based therapy on the metabolic status of patients with chronic hepatitis C virus (HCV) infection. Materials and Methods: In this observational before-after study, a total of 128 consecutive patients with chronic HCV infection, presenting to the Middle East Liver Disease Center (MELD) in Tehran, Iran, were enrolled. Metabolic indices, including triglyceride, total cholesterol, low-density lipoprotein (LDL), high-density lipoprotein (HDL), and fasting blood sugar (FBS), were determined before and after sofosbuvir-based therapy. Results: The mean FBS level was 108.3±33.8 and 106.2±36.4 mg/dL before and after treatment, respectively (P>0.05). Also, the mean triglyceride level was 101.8±46.4 and 110.5±54.8 mg/dL before and after treatment, respectively (P=0.03). The findings showed that the mean total cholesterol level was 150.9±34.9 and 168.0±40 mg/dL before and after treatment, respectively (P=0.001). Also, the mean LDL-cholesterol level was 82.4±27.5 and 95.8±31.6 mg/dL before and after treatment (P=0.001), and the mean HDL-cholesterol was 44.1±13.1 and 46.9±13.4 mg/dL before and after treatment, respectively (P=0.01). Conclusion: According to the results, it can be concluded that sofosbuvir-based therapy has effects on some metabolic parameters (lipid profile and blood glucose) of chronic HCV patients. Escape of sperms into the injured female tissues might encourage her body to develop antibodies against the seminal content of male partner and causing infertility. Method: We recruited and ultimately selected 86 volunteers with essential hypertension and obesity in this study as a quasi experiment. The volunteers went through three phases, namely, the baseline anti-hypertensive therapy stage (Phase-1), the FF intervention stage (Phase-2), the normal diet recovery stage (Phase-3), each for seven days. During the Phase-2, all subjects fasted and began to substitute the FF food for conventional food. Patients' systolic/diastolic blood pressure (abbreviated as SBP/DBP), body mass index (BMI) and body weight were monitored in details each day. The trial data of all subjects were statistically analyzed using self-controlled designs. Result: After the FF intervention, the subjects' blood pressure, body mass and BMI all showed naturally decreases by comparison to that before the intervention. The remarkable decreases in SBP/DBP, Body Mass and BMI were observed in Phase-3 after FF intervention when compared to Phase-1 [SBP: (115.2±8.492) vs (121.2±13.72) mmHg, q=34.14, P<0.001]; [DBP: (76.03±5.37) vs (78.36±7.094) mmHg, q=10.96, P<0.05)]; [Body Mass: (68.86±13.04) vs (73.11±13.49)kg, q=61.56, P<0.0001]; [BMI: (25.42±3.359) vs (27.03±3.45) kg/m2, q=100, P<0.0001]. All patients did not experience any discomfort or symptoms during the trial. It is caused by multiple factors such as genetic predisposition and hormonal alteration including growth factor IGF-I, among others. Acne being related to IGF-I expression, is involved in possible apparition of malignant diseases. The pathology of acne is limited to the pilosebaceous unit. The evolution is governed by the increase in colonization of the bacteria Propionibacterium acnes (P. acnes). Acne is clinically manifested by the appearance of comedones with a tendency to inflammation. Finally causes scarring damaging self-esteem and quality of life of those who suffer it. As proposed therapies, external treatments have been developed such as inhibitors of IGF-I among others. As an internal treatment, the innate response of the patient's immune system to the presence of an invading microorganism has been studied, highlighting anti-microbial peptides as the host's own defense molecules. This work shows a compilation of the most relevant and current antimicrobial peptides that could be used as potential therapeutic agents against microorganisms located in the skin and related to acne disease. *Correspondence to: D.M.
