The study for surgical outcomes still vary and study for cost-effectiveness are still lacking. We aimed to compare the surgical outcomes and cost effectiveness of prone versus supine PCNL in our institution. Material & Methods: This study was done retrospectively by analyzing medical records on 51 patients underwent PCNL in Saiful Anwar General Hospital Malang from January 2016 until October 2017. The outcomes of stone free rate, body mass index (BMI), stone size, operative time, anaesthetic time, length of stay (LOS) in hospital and complications were compared. For Cost Effectiveness Study, total cost, anesthesia and disposable equipment was recorded. Results and Discussion: There was no significant difference in both groups characteristics. The supine group had lower blood loss (0.54 mg/dl vs 1.37 mg/dl, p=0.001), shorter mean operative time (57 minutes vs 78 minutes, p=0.001), shorter mean anesthetic time (71 minutes vs 107 minutes, p<0.001). Supine group was associated with significantly cheaper disposable equipment used for surgery (4,985,636 IDR in prone vs 4,229,770 IDR in supine (p<0.001)) and anesthesia (353,454 IDR in prone vs 105,120 IDR in supine (p<0.001)), and cheaper total cost (17,623,363 IDR in prone vs 15,175,305 in supine (p = 0.038)). Conclusion: In our study, supine position has lower blood loss, shorter operative and anaestethic time. The supine PCNL is also more cost-effective in our institution compared to prone PCNL. We suggest that the PCNL in the supine position is a promising alternative.","PeriodicalId":232854,"journal":{"name":"Journal of Medical - Clinical Research & Reviews","volume":"33 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134456856","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Both allergic and nonallergic mechanisms may be involved. It has a very typical presentation and worsens with continued exposure. There is a very wide list of substances that cause this disease, mainly products associated with ammonia. The diagnosis is made based on the clinical manifestations and the first line treatment is the cessation of exposure. The authors present a case report of a 52-year-old woman, who worked as laboratory assistant in techniques of biology, physics and chemistry, in a secondary school, for 20 years. Preparation of ammoniacal solutions and solutes in association with the manipulation of the products without personal protective equipment, led to the development of a clinical picture compatible with the diagnosis of occupational rhinitis. In the clinical history, the symptomatology described by the patient worsened in periods of labor exposure and was absent in vacation periods, which is typical of occupational pathology. Several treatments were attempted, medical and surgical, before occupational rhinitis was suspected. However, improvement of the clinical picture with work reallocation, led to the assumption that occupational rhinitis was the most likely diagnosis. This case demonstrates that the diagnosis of occupational rhinitis can be easily missed, if we ignore the occupational exposure of patients, which can lead to unnecessary and costly medical interventions, since the only effective treatment is the cessation of exposure to the causal agent.","PeriodicalId":232854,"journal":{"name":"Journal of Medical - Clinical Research & Reviews","volume":"51 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-10-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124425229","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is important to consider health and safety first when accepting new job schedules or offers. This study investigates how night shift schedules have contributed to workers’ poor well-being, quality of life and marriages. One-hundred ninety-one night shift workers voluntarily participated in the study. Respondents were grouped into three age categories: 18-34, 35-54, and 55-74. Responses were quantified and statistical analyses were run on the data. Results revealed that 24% of the respondents responded to be depressed and 57% indicated to have low energy after working a night shift schedule. The risk of divorce among the married respondents was found to be 1-person in every 5-workers. Poor appetite revealed 1 out of every 3-workers and 1 out of 2-workers have the risk of losing interest in their usual activities. Approximately seventeen percent reported having very bad quality of life, while 39% declared to have average quality of life. Thirty percent of the respondents revealed having concentration problems after working night shift. The paired t-test result revealed that a night shift schedule has a statistically significant effect on the workers’ well-being who work on night shift (t = 16.13, df = 190, p < 0.0001). The level of divorce, poor quality of life, less interest in usual activities and health issues as a result of workers’ schedule suggest the need for better work schedule structuring. The findings from this study highlight potential damages night shift schedule has done to human well-being and life style in general. Findings from this study could help families to better avert or handle health issues at home and with their spouses.","PeriodicalId":232854,"journal":{"name":"Journal of Medical - Clinical Research & Reviews","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-10-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127409149","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
