Although there are reports of simulated liver phantoms with hepatocellular carcinoma (HCC); no studies have documented the simulating of the typical dynamic pattern. Aim: The present study aimed to investigate properties of developed phantoms with respect to the stability, including chemical, mechanical, electrical, as well as MRI properties (T1- and T2- signal intensities), in addition to simulating HCC in a dynamic way. Methods: In this study, liver parenchyma of size 23 ×18 ×13 cm was simulated using three different agarose-wax samples (agarose at concentrations of 2.5 wt%, 4.5 wt%, and 6.0 wt%) mixed with a fixed wax concentration of 2.6 wt%.. HCC samples were fabricated using polyurethane and glycerol of various diameters (0.5, 1.0, and 2.0 cm). Results: The results showed that the agarose-wax sample with a concentration of 2.5 wt% was the most stable sample among the other samples (P-value, 0.468) in mechanical and T1 and T2 intensities. It was also noted that the sample at the concentration of 4.5 wt% had the closest density value to human liver with a difference of 7.88%. Moreover, the agarose-wax sample at a concentration of 2.5 wt% had the closest compressibility and conductivity values, and T1-relaxation time compared to the human liver. However, the largest changes in relaxation times were observed in the fifth week of all samples (P-value = 0.047). The typical enhancement pattern for a simulated HCC of a minimum size, which can be measured with MRI, was 1.0 cm, using a body coil, and 0.5 cm using a head coil. Conclusions: The chemical, electrical density, compressive strength, density, and MR imaging properties of the phantom were measured and compared to those of the human body.","PeriodicalId":232854,"journal":{"name":"Journal of Medical - Clinical Research & Reviews","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125865746","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Phenobarbitone remains the drug of choice in treatment of neonatal seizures. This study was conducted to determine the predictors of duration of Phenobarbitone therapy in seizures of neonates with Hypoxic-Ischemic Encephalopathy (HIE). Methodology: The study was conducted in Neonatal Intensive Care Unit (NICU) of East West Medical College Hospital from 1st february 2016 to 31st january 2017 on fifty neonates with HIE and seizures. The dose and duration of phenobarbitone therapy for initial control of seizure was noted. Total duration of phenobarbitone therapy after control of seizure to prevent further recurrences was also noted. Follow up was given up to 3 months of age to see any recurrence of seizures. Results: Out of the 50 neonates,25 cases (50%) responded well within 24 hours. Mean duration of phenobarbitone therapy after initial control of seizure was 4.70 ± 2.93days and Mean total duration of therapy was 6.89 ± 3.58 days in cases who responded well within 24 hours.. 3 (6%) cases had recurrence of seizures. The recurrence cases required more than 72 hours for initial control of seizure (p 0.006) and had poor primitive reflexes and activity after initial control of seizure (p 0.0002). Mean duration of phenobarbitone therapy after initial control of seizures was 6.33 ± 6.11 days and Mean total duration of phenobarbitone therapy was also longer 9.67 ± 7.37 days. Conclusion: Neonates who have good initial response and have better reflexes required short duration of phenobarbitone therapy. It should be discontinued soon after control of seizure. Neonates with HIE III, those requiring more than 72 hours for initial response and poor reflexes after initial control of seizure may require longer duration of phenobarbitone therapy (more than 7 days) to prevent recurrence of seizures. Thus, judicious use of phenobarbitone will minimize the unnecessary non-specific treatment.","PeriodicalId":232854,"journal":{"name":"Journal of Medical - Clinical Research & Reviews","volume":"48 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115937576","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Without data on knowledge of dementia and the occurrence of ageism towards old people among adolescents, it is difficult to introduce measures to reduce and prevent them. Aim: To asses dementia-related knowledge with a focus on comparison between female and male adolescents and whether any stereotype about the elderly is expressed among them. Methods: 1128 students (aged 14 -19) from non-health related secondary schools in Slovenia completed the survey. In the questionnaire with twenty claims of dementia, one contained stereotype: »Impaired memory is normal part of getting old«. Results: Responses to claims of dementia ranged from 4 to 20 points, with participants responding to an average of 71.5% of claims correctly (M = 14.30; SD = 2.56). Girls showed better knowledge of dementia than boys (M = 14.63; SD = 2.36 vs. M = 13.63; SD = 2.82; p<0.05). Suprisingly, with the stereotypical and discriminatory claim: »Impaired memory is normal part of getting old« as many as 86.0 % of girls and 81.9 % of boys (p>0.05) agreed. Conclusions: Dementia-related knowledge is satisfactory, but lower in male than female adolescents. The research results point to the need of introducing gender-specific formal and non-formal education on dementia. Furthermore, the results of our study show that most adolescents of both genders already think stereotypically about the elderlies, which can lead to ageism. The area should be researched further.","PeriodicalId":232854,"journal":{"name":"Journal of Medical - Clinical Research & Reviews","volume":"103 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121123189","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The possibility of the development of thrombotic events (VTE) with this type of cancer is frequent. Objective: To determine the risk of presenting VTE and haemorrhagic events in patients affected by GBM. Methods: Observational retrospective study of patients with GBM diagnosis at the General University Hospital of Ciudad Real between 2012 and 2015. The demographic characteristics of patients were studied, predictive models were compared, and a survival analysis was performed. Results: 77 patients were studied, 42 (55.3%) / 34 (44.7%), men and women respectively, with an average age of 66.42 years. 13 (16.9%) presented VTE; of which 10 (61.54%) in the form of deep venous thrombosis (DVT), 3 (23.08%) pulmonary embolism (PE) and 2 (15.38%) mixed events. The quality of life according to the performance status ECOG scale at the moment of diagnosis was 1 in 42 (15.38%) patients, and at the time of VTE, 5 (41.7%) had a value of 2, and 4 (33.3.3 %) registered 3. In the group that developed VTE according to the predictive model of risk for thrombosis in Khorana 5 (38.5%) had low risk and 8 (61.5%) intermediate; on the ASCO 2013 modified scale 5 (38.5%) had an Intermediate risk and 8 (61.5%) high. With a median, 1 year follow-up, 64 (84.2%) patients died, with an average time after the diagnosis of 279.09 days (216.6-341.6) (SE 31,8). 2 (2.6%) of the patients presented a greater haemorrhagic event and 7 (7.9%) cerebral haemorrhage, of which 4 (44.4%) had prophylactic Low molecular weight heparins (LMWHs). In the survival analysis of Kaplan Meyer, patients who received prophylactic treatment with LMWHs had a higher survival rate with an average of 298.5 days compared to 239.3 of those who did not (p> 0.05). There were no significant variables in the multivariate analysis for thrombotic or haemorrhagic events. Conclusion and Discussion: The demographic and clinical characteristics of our patients were similar to those reported in other international publications. The predictive scale of Khorana was not validated in our study, in contrast, the modified ASCO 2013 scale was closer to our results. The creation of a precise predictive model would help to delineate the benefit of prophylactic anticoagulation in high-risk patients. Long-term prophylaxis with LMWHs has demonstrated a reduction of thrombotic events without significantly increasing the fatal haemorrhagic episodes, also demonstrating greater long-term survival, independent of thrombotic events. Randomized prospective studies are needed to demonstrate its benefits.","PeriodicalId":232854,"journal":{"name":"Journal of Medical - Clinical Research & Reviews","volume":"62 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114813508","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In the Covid era, positive patients were postponed for a few weeks before any surgery to ensure their safety. However, with complete ureteric obstruction with large stones, delayed ureteroscopy and stone fragmentation could potentially compromise renal function. We aim to evaluate the Thulium Fiber Laser outcome as an alternative to holmium YAG laser as intracorporeal lithotripter in Covid-19 positive patients. Methods: From March-July 2020, during the peak of the corona outbreak, 11 COVID-19 positive patients were admitted through the emergency department with obstructing ureteric stones underwent urgent JJ stent insertion 2 weeks later from the initial surgery a definitive treatment offered. Results: The average age was 36+-11.7 years, all-male, all fit and healthy despite their Covid-19 status. The average stone size was 11+-3.1 mm with a volume of 395.8+-280 mm3 . The Hounsfield unit measurement was 1033.3+-57.7. Seven underwent short general anesthesia and 4 had spinal anesthesia. The average laser duration was of 593.9+-357.5 seconds, and the average stone ablation speed was 0.66+-0.19 mm3 /Sec. The overall stone- free rate was 90.9%. All 11 patients were discharged safely on the same day with only 1 patient readmitted with steinstrasse and underwent urgent ureteroscopy. None of our patients developed late complications. Among these, Methicillin- resistant Staphylococcus aureus (MRSA) is one of the major hospitals acquired multidrug resistant strains. MRSA shows it is resistant against methicillin and other penicillin like antibiotics by producing PBP2a (penicillin binding protein 2a) which is encoded by its mecA gene. In this process mecR1 and mecI gene act as regulator of mecA gene. The aim of this study is to identify sequence variance located in mecA, mecR1 and mecI promoter region and the effect of those changes in relation to pathogenicity of these MRSA strains. In this current research, work wound infections samples were collected from 90 patients who were infected during post-surgical mamagement. Samples were collected from Rajshahi Medical college hospital, Bangladesh. In antibiotic sensitivity test, MRSA was found 100% sensitive against only Choramphenicol and Bacitracin.
