Simiyu","doi":"10.47672/ejhs.1227","DOIUrl":"https://doi.org/10.47672/ejhs.1227","url":null,"abstract":"Purpose: The use of biomass fuels poses great threats to environmental degradation and public health risk accounting for 32% of the total attributable burden of diseases due to indoor air pollution (IAP) in especially Africa. Heavy reliance on biomass fuels for household energy in Kenya makes the country more vulnerable with 90% of the rural population relying on biomass fuels for domestic purposes. The objective of this study was to assess cooking fuel types and efficiency of improved biomass stoves in fuel consumption in Western, Kenya. \u0000Methodology: The data were collected through continuous real-time monitoring of kitchen Particulate Matter and Carbon II Oxide concentration for a period of 24 hours using UCB-PATS and CO monitors, questionnaires and time activity budgets. The total target population was 383 households and 204 households were selected as the sample size for HH survey. The sample size was determined using sample size algorithm by Boyd et al. (2014) where a sample size is determined by the sample population size. Selection of households for indoor air monitoring was done through quasi system where there was a predefined criterion from survey data. Tables and means were used to present results. \u0000Findings: The study found that Hazard quotients (HQ) for both long-term and short-term PM exposure using all stoves were all above 1 implying that health risk is real. During 24-hour cooking duration, three-stone stove using crop residues produced 145.8 times higher PM2.5 compared to RfD (Reference Dose) value while Cheprocket produced 26.4 times higher than PM2.5 RfD. People using solid biomass fuels are likely to experience headaches and running nose by the end of 24-hour period as a result of CO exposure when mud rocket stove, three stone stove and Cheprocket stoves were used. However cooks who use Chepkube stoves are not likely to experience any adverse health effects from CO exposures since the HQs were less than 1 using both wood and crop residues as fuel. The study concluded that, improved biomass stoves provided an overall reduction in pollutant concentration compared to three-stone fire but the local innovation Chepkube stove that has been classified as ungraded stove had the highest pollutant reduction. It is a true urological emergency. It is a relatively rare condition. The aim was to highlight the presentation and management of Fournier’s gangrene following circumcision. \u0000Methodology: This was a retrospective study. Eight patients who presented with features of Fournier’s gangrene post circumcision between January 2012 and December 2021 to University of Port Harcourt Teaching Hospital UPTH were included in the study. A questionnaire was used to obtain data from all patients listed in the medical records department as having been treated for Fournier’s gangrene during the study period. Data collected include age at presentation, level of education of mother, duration of symptoms before presentation, who carried out the circumcision, where the circumcision was carried out, number of debridement carried out and complications. The data was entered using Microsoft Excel 2016 version and transferred into the statistical package for social sciences (SPSS) for windows (version 20) (IBM SPSS Inc. Chicago, IL) for analysis. Categorical data was presented in the form of frequencies and percentages using tables. Continuous variables were presented in means and standard deviation. Results were presented in tables and charts. \u0000Findings: A total of eight patients had FG following circumcision. The median age was 14days and range was from 10 days to 10years. The parents of most patients (87.5%) presented with no formal education or primary level of education. Most patients (62.5%) presented 4 to 6 days after onset of symptoms. Five out of 8 had their circumcision by a traditional attendant. Three patients had complications. One patient died. Circumcision can be a predisposing factor to FG especially if it is carried out at home, and by untrained personnel. Early presentation and management gives the best outcome. \u0000Recommendation: Only trained personnel should carry out circumcision. The objective of this study was to investigate the effect of adequate funding and health service provision on micro health insurance growth, taking BEPHA as a case study. \u0000Methodology: The descriptive and explanatory research designs were used in this study. Data was collected by administering questionnaires to 400 respondents. Interviews were also carried out on 10 staff of BEPHA. Data were analyzed using descriptive statistics, correlation, and multiple regression analysis. \u0000Findings: In results regarding BEPHA financing, 70% of those interviewed believed it is high while 30% indicated that it is low. Out of 400 participants 191 (47.8%) of them perceived services offered by BEPHA partner hospitals as fair while 182 (45.5%) perceived their services as good. The correlation results showed a strong positive and significant relationship between sufficiency of BEPHA financing (r= 0.802, p < 0.05) and quality of BEPHA health services (r = 0.667, p < 0.05) with the growth of BEPHA as a health insurance scheme. The multiple regression analysis results showed that sufficiency of BEPHA financing (β=0.166; p < 0.1), has a positive and significant effect on growth of BEPHA. The quality of BEPHA health services (β=0.200; p < 0.1) also has a significant positive effect on growth of BEPHA. Concisely, the study revealed that the funding and quality of BEPHA health services can determine the variations in the growth of BEPHA health insurance scheme. \u0000Recommendations: This study thus recommended that, the scheme should reduce the waiting period for minor health risk to at most 2 weeks while the 6 months waiting period for major health risk including delivery should be reduced to one (01) month to increase finances from large enrolment. This study, in response to this gap, examined the effect of being a TVET-trained housekeeper on health and safety practices in hotels in Cape Coast and Elmina. \u0000Methodology: The explanatory research design was employed. Data were collected, using self-administered questionnaire, from 100 conveniently selected housekeepers of 51 hotels in Cape Coast and Elmina in the Central Region. Data collected were analysed using means, standard deviations and regression analysis. \u0000Findings: The results showed that seven dimensions account for about 60% of the variance in health and safety practices. Also, the general level of health and safety practices in hotels in Cape Coast and Elmina was high – with higher average scores recorded among TVET-trained housekeepers compared to non-TVET-trained housekeepers. Several factors play role in the development of hemorrhoids and subsequently their prevalence. However, these factors are under-researched in Pakistan. This study aims to recognize the risk factors of hemorrhoids among patients in a surgical unit of Holy Family Hospital Rawalpindi. This may lead to a decrease in hemorrhoids incidence and its associated social and economic burden through the prevention of risk factors that would have the main role in the development of hemorrhoids. \u0000Methodology: This descriptive cross-sectional study was conducted among one hundred and three diagnosed patients with hemorrhoids for seven months from April 2021 to October 2021 in the surgical outdoor patient department of Holy Family Hospital Rawalpindi. Patient enrollment was made in the study via a set of inclusion and exclusion criteria and a non-probability convenient sampling technique. A self-designed questionnaire was applied for the collection of data. Inform consent was taken before the data collection from all participants. Data analysis was performed via SPSS (Statistical package for social sciences) version 25. Descriptive statistics were applied (IBM Corp., Armonk, NY). \u0000Findings: Hemorrhoids were more prevalent among the patients who had, an age group above 40 years (73.78%), male gender (66.99%), a low education level (56.31%), lower socioeconomic status (62.14%), family history of hemorrhoids (55.34%), constipation (78.64%), longer duration of stay in the toilet for evacuation (58.25%), chronic cough (51.46%), no daily intake of fresh fruits and vegetables (66.99%), daily intake of spicy foods (50.48%), inadequate intake of water (60.19%), sedentary lifestyles (73.78%), and overweight (41.75%). Loss of bone mass with aging leads to osteoporosis. Different factors affect the bone mass loss and consequencltly osteoporosis prevalence. However, these factors are under-studied in the Pakistan.This study aims to identify the risk factors of osteoporosis among patients in Holy Family Hospital  Rawalipindi. \u0000Methodology: This descriptive cross-sectional study was carried out in among 120 diagnosed patients of osteoporosis for six months from April 2021 to September 2021 in a tertiary care hospital of Rawalpindi. Patients were recruited in the study via a set  of inclusion and exclusion criteria and non-probability convenient sampling technique. A self-strcutured questionnaire was used to collect data. Before data collection information consent was acquired from all participants. Data analysis was performed on SPSS version 25. Descriptive statistics were applied. \u0000Findings: Osteoporosis was more prevalent among the patients who had, age group with more than 45years (79.16%), female gender (72.50%), low education level (64.16%), lower socioeconomic status (68.33%), family history of osteoporosis (61.67%), sedentary lifestyle (57.50%), history of corticosteroids therapy (53.33%), no milk use (69.20%), no fresh vegetables and fruits use (80.00%), no inatke of calcium supplements (78.33%), no sunshine exposure (77.50%), and overweight (48.33%). In short, age group with age above 45years, female gender, low education level, lower socioeconomic status, family history of osteoporosis, sedentary lifestyle, overweight, history of use of corticosteroids, no milk use, no intake of fresh vegetables and fruits, no intake of calcium supplements, and no sunshine exposure, all raise the incidence of osteoporosis. \u0000Recommendations: People should take milk, fresh fruits and vegetables, calcium supplements, and sun bath. The health facilities were selected by multi stage sampling technique. Data were collected using a self-structured questionnaire that incorporated the Breastfeeding Self-Efficacy Scale-Short Form questionnaire by Faux and Dennis and analyzed with Statistical Package for Social Sciences (SPSS) version 21. \u0000Results: Findings showed that 78.1% had adequate initiation of breastfeeding (0-1 hour after childbirth), 28.9% of the participants initiated breastfeeding at 0-30 minutes of childbirth while 54.7% of the participants did not practice exclusive breastfeeding. Most (83.6%) of the participants had adequate BFSE scores. About 31.3% of the participants practiced good commencement of complimentary feeding. Majority of the participants used infant milk formula (33.6%) and Pap with sugar (12.5%) for complimentary feeding. The hypothesis tested revealed that there is a significant strong positive relationship between age and BFSE (p = 0.000*, γ effect size = 92.1%) and there is also a significant strong positive relationship between parity of mother and BFSE (p = 0.000*, γ effect size = 61.2%). There was good initiation of breast feeding but poor practice of exclusive breastfeeding amidst good BFSE. The target group composed of mothers who delivered in the hospital during the study period (July-August, 2019). A sample was obtained from all willing mothers. Sample size was determined using the Cochran formula assuming a proportion (p) of 16.4% of low birth weight. Desired sample size was 210 mothers which was adjusted to 215 participants. Independent variables included: medical equipment and supplies, medical personnel and accessibility of health facility. Dependent variable was LBW which was classified further as very LBW, extremely LBW. The primary focus was first to identify LBW which was then categorized according to the standard classification. Quantitative and qualitative data were collected and analyzed using logistic regression and NVivo respectively. \u0000Findings: Accessibility to a health facility OR= 0.45, p=0.04, CL [0.21-0.97] was associated with very low birth weight. The mothers (FGDs) indicated that there were challenges at the laboratory; some health workers were not good; and ANC counselling sessions were inadequate. Health workers who were Key Informants (KI) reported inadequacy of resources and suggested campaigns to reach mothers in order to attend ANC early and a provision of a preconception centre at the Hospital.   \u0000Unique contribution to theory, practice and policy: Inaccessibility of a health facility by pregnant mothers is a risk factor for very low birth weight neonates. ANC guidelines to be revised to cater for the concerns of mothers. (EPT) will have a significant outcome for the threshold readings. These relationships in previous studies have been documented. As dental flourosis is an endemic in regions of Pakistan the aim of this study was to compare and determine the appropriate EPT probe placement sites on fluorotic and non flourotic teeth. \u0000Methodology: In this cross-sectional clinical study equal number of flourotic and non flourotic maxillary central incisors(40 volunteers)  were selected from a known region of Pakistan for endemic of flourosis to measure threshold levels on EPT on four different parts of each tooth. (Incisal edge, incisal third, middle third and cervical third). Before the readings were taken flourotic teeth were also categorized on the bases of severity (Deans Index).The purpose was to assess if severity of dental flourosis had an impact on the EPT readings.  Once the mean values were taken the Dunn–Bonferroni post hoc tests was used to assess the statistical significance (p<0.05) between the four site in both groups. \u0000Results:  Mean values from the EPT presented that the threshold values increased from incisal to cervical in gender and both age groups i.e. 20-29 and 30-40. It was also found that the threshold values increased from incisal edge to the cervical third in flourotic and non flourotic teeth. Statical significance was found between the reading of flourotic and non flourotic groups across the four probe site readings taken (p value= 0.00).  Like previous literature the insical edge was considered the most ideal site for EPT assessment on anterior incisors. As this is where the lowest threshold response was recorded in our study .Dental flourosis does have a significant effect on the EPT threshold values. This is most likely due to structural changes because of dental flourosis. \u0000Recommendations: From the study conducted it can be recommended that clinicians need to be aware of the fact that fluorotic teeth response will vary from natural teeth on the EPT. Zarif","doi":"10.47672/ejhs.1144","DOIUrl":"https://doi.org/10.47672/ejhs.1144","url":null,"abstract":"Purpose: The study aims to overview the smoking habits, their prevalence, and their correlation with socio-cultural determinants by screening smoking in the national heart institute population. \u0000Methodology: A cross-sectional study was conducted in the form of a face-to-face survey. The interviews were carried out by a well-trained team of interviewers, using a survey according to a stratified random sampling approach. The study design was mainly about meeting up to about 300 health care workers and 300 patients visiting the clinics in the national heart institute. Then, start to gather information from them if they smoke and learn more about the type of smoking, they do and their smoking habits. The survey included questions for smokers about quitting trials and their reasons for sticking to smoking. After that, the interviewers tried to raise awareness about the negative impact of tobacco. The next step was to try to advise these smokers to quit. \u0000Findings: The study cohort was 160 visitors (36%), 82 employee (19%), 76 nurses (17%), 64 patients (14%), 41 physician (9%) and 20 technician (5%). Regarding the education level, 63% had primary school education, 17% had a university level, and 15% were illiterate. Smoking prevalence was 60% of the patients, 45% of the visitors, and 30% of the health care workers. In the study sample, smokers were more likely to have diabetes (P0.071), to be hypertensive (P0.034), and to have a history of heart disease (P 0.004). They were more likely to be males (P<0.001). There was no significant difference between smokers and non-smokers regarding geographical residence or educational level. Smokers are more likely to be aware of smoking hazards, including heart problems, COPD, cancer, and E.D (P<0.001). \u0000Recommendations: This cross-sectional survey study showed only the association, therefore, the cause, pathogenesis, and effect were not evaluated.
