Pulmonary hypertension (PH) is considered a common comorbidity in CKD patients. This study aimed at assessing the correlation between CKD and pulmonary hypertension. \u0000Methodology: This is a retrospective study that recruited 100 Egyptian patients with stage 3 and above Chronic Kidney Disease (CKD) (KDOQI    guidelines). The study sample was composed of 100 patients with 60% overall prevalence of PH. Transthoracic echo Doppler study was performed to measure pulmonary artery systolic pressure based on tricuspid jet in the sample. \u0000Findings: There was significant association between PH and severity of renal disease. The hemodialysed group showed a higher prevalence of PH with a more severe PH compared to groups not on regular dialysis. Radial artery occlusion (RAO) which can occur during transradial intervention, impairs radial artery (RA) to be the future access site, and prohibits the artery from being used as an arterial conduit. Aim of this research was comparing incidence and predictors of RAO among individuals receiving elective cardiac catheterization by conventional radial access vs distal radial access. \u0000Methodology: This prospective study enrolled 120 patients from June 2022 to January 2023 (84 males, 36 females; mean age 68.5 (10.4) years with 62 patients had elective cardiac catheterization via conventional radial approach (CRA) and others via distal radial approach (DRA). Clinical follow up at 24 hours and 30 days was recorded with analysis of the incidence and predictors of RAO among all included participants. \u0000Findings: This study reported no substantial difference among groups in terms of socio-demographic and clinical characteristics. Time to sheath insertion and Procedure time were long among patients who had Distal radial approach with statistically significant difference (P <0.01). Moreover, RAO at 24 hours and 30 days follow up was higher among patients had CRA than those had DRA with no significant difference (P >0.05). This research demonstrated that RAO incidence was significantly high among younger patients, smoker, DM and those with previous CAD. Also, time to sheath insertion and hemostasis were long in patients with RAO with statistically significant difference (P <0.05). Smoking, DM, long Procedure time and increased time to hemostasis with diminished blood supply in wrist throughout hemostasis were strong predictors for radial artery occlusion. \u0000Recommendations: Maintaining radial patency must be done with all procedures using the radial approach. DRA may be useful to lower RAO incidence through shortening hemostasis time and sustaining radial artery flow during hemostasis. However, it is unclear if longitudinal strain following mitral valve surgery has any predictive utility in clinical settings. The best time of operation is usually detected through the left ventricular internal dimensions as well as the function.. So this study aimed to evaluate the utility of myocardial strain by echocardiogram with other parameters in forecasting postoperative outcomes after primary MR surgery. \u0000Methodology: This prospective study enrolled 514 patients (279 males and 235 females; mean age (SD) of male participants was 57.3 (12.9). Included patients had severe primary mitral valve regurgitation and were candidates for valve surgery from April 2018 to April 2022. Patients with combined valvular disease as severe mitral stenosis, redo mitral valve replacement, and previous CABG and ischemic heart disease were excluded. Global longitudinal strain (GLS) was performed. Hospital re-admission for heart failure symptoms, reoperation and mortality were followed up as Cardiac incidents. \u0000Findings: In median follow-up of about four years, Age, previous ischemia, concomitant CABG, left atrial size, LVEF%, atrial fibrillation and Global longitudinal strain were all predictive of heart incidents. Age, left atrial dimension, and GLS were independent predictors to heart incidents on multivariate analysis (HR: 1.386, 95% CI: 1.083-1.776; P <0.004, HR: 1.003, 95% CI: 0.976-1.031; p value 0.018, HR: 1.192, 95% CI: 1.101-1.291; P <0.001 respectively). All-cause mortality was linked to impaired GLS (HR: 0.003, 95% CI: 0.973-1.102; P =0.039). \u0000Recommendation: GLS has a substantial link to clinical outcomes and is a stronger forecaster to cardiac issues than standard measurements. The purpose of this study was to describe the clinical and medico-legal management of child victims of sexual violence, 30 years after the first works on the issue. \u0000Methodology: A retrospective multicentre cross-sectional study was conducted from November 2019 to May 2020. A consecutive and non-exhaustive recruitment was done. Children aged less than 18 years, admitted for sexual assault in the 04 reference health facilities of the city of Yaoundé were included. The analysis of the collected data was done using the software: Epi-info ™ version 7.2. \u0000Findings: Of the 19187 children received in 5 years in the study’s host hospitals, 88 child survivors of sexual assault were identified. The prevalence of sexual assault was 0.45%.  About 87 (98.86%) of the victims were female. All 88 (100%) children received medical-surgical treatment of the physical injuries. hjujuyk8care was received by 60 (68.18%) of them. In 46 (52.27%) of the cases, the perpetrators of sexual assault were known to the victims. For 39 (44.31%) of the victims, a report was made. The alleged perpetrator was arrested in 6 cases (6.81%). One of the alleged perpetrators was convicted. Due to the probability of left main (LM) affection, an LM bifurcation stenting technique may be required in certain individuals. So we evaluated result of Intravascular Ultrasound (IVUS) guided percutaneous coronary intervention (PCI) in isolated LAD coronary artery lesions, either by crossing over the ostium or not, with a focus on major adverse cardiovascular events (MACE). \u0000Methodology: Our prospective study from January2021 to November 2022 included 79 isolated ostial LAD patients with Ostial LAD stenting (OS) or LM to LAD cross-over (CO) stenting at the National Heart Institute. As per the recommended guidelines, the participants were divided into two groups: the first one had IVUS guided PCI and Group (2) had Angiography guided PCI. The data was collected and statistically analyzed with SPSS 23.0 program. \u0000Findings: No statistically significant difference was present between the groups regarding socio-demographic or clinical data (P-value > 0.05). It was discovered that an increase in Rotablator use, a decrease in Fluoroscopy time (min), a decrease in Contrast volume (ml), and a decrease in PCI duration (min) in patients who had IVUS guided PCI, with statistically significant difference when compared to other group who had Angiography guided PCI (P-value < 0.05). In addition, it was discovered that an increase in the necessity for a cross-over stenting method in group (1), with no statistically significant difference (P-value > 0.05). High prevalence of mortality and morbidity among IVUS guided PCI patients was present with statistical significance regarding TVR (P-value < 0.05). \u0000Recommendations: IVUS can offer valuable information on vascular lumen, plaque features, stent deployment, & device failure causes. As a result, IVUS-guided PCI may enhance clinical impact among participants, particularly those with complicated coronary lesions & those at high risk. To achieve this, the study was guided by two specific objectives: To identify the role of stakeholders in disaster management in the informal settlements of Nyeri Town and to determine the challenges encountered during disaster risk reduction initiatives in the informal settlements of Nyeri Town. \u0000Methodology: The study adopted a descriptive research design using primary data collected through a structured questionnaire. The population for this study was 384 respondents of the four settlements of Nyeri town, that is, Majengo Witemere Ngangarithi, Mathari and Ruring’u Muslim village. The study used a sample of 384 respondents that was divided proportionally between the four settlements. \u0000Findings: The study found out that, majority of the respondents [93.0%] was aware of the disasters that can affect them in their area of residents. The study further established that, majority of the residents 196 (51.0%) were aware of the existence of disaster risk reduction policies as compared to 49.0% who were not aware of any disaster risk reduction policy. The results also showed that there was a significant association between the level of education of the respondents and the level of awareness of the disaster that could affect them [X2 (3) = 14.848, p-value =0.002<0.05] and the religions of the respondents and the level of awareness of the disaster that could affect them [X2 (2) = 7.090, p-value =0.029<0.05]. The results however indicated that, there was no significant association between the level of awareness of the disaster that could affect the respondents with the area of residents, age of the respondents and their occupation as given by the p-values of 0.393, 0.485 and 0.390 respectively. The study further established that, there was a significant association between the education level of the respondents and the level of awareness of any policy rule concerning with disaster risk reduction [X2 (3) = 8.056, p-value =0.045<0.05], and the religions of the respondents and the level of awareness of any policy rule concerning with disaster risk reduction [X2 (2) = 10.031, p-value =0.007<0.05]. It was concluded that Risk assessment as a step for successful disaster reduction measures will ensure that the community members are aware of the possible hazards. National and County government should incorporate the national and international policies and guidelines in their policy. \u0000Recommendations: The Government should be keen on learning on previous disasters that have affected other informal settlements and other parts of the country by having disaster management well kwon by the communities living in informal sectors. In the public healthcare providers, the health facilities are built by the government and the healthcare workers, draw their salaries from government treasuries. In the private providers, the health facilities are owned by individual or a group of people and the salaries of the workers are paid from the resources generated in the facilities. They are either for profit or non-profit making. Enrollees are given freehand to choose either of the providers, but the information available to them is not enough to make decision on which of the providers to choose. This study aimed at determining the enrollee’s choice of private and public healthcare providers of community-based health insurance scheme in Edu LGA Kwara state. \u0000Methodology: The design was descriptive cross-sectional study. Sample size of 400 was used in each of the provider. The respondents were recruited by systematic sampling method among private healthcare provider while multistage sampling method was adopted in public healthcare providers. Data was collected using semi structured questionnaire. Focus Group Discussion was also carried out. Data collected were analyzed using SPSS version 23.0. Results were tabulated and logistic regression was adopted to determine level of significance. Level of Significance was set at P<0.05 \u0000Findings: Nine- point-five (9.5%) of the respondents of the private healthcare provider and 5.7% of the respondents of the public healthcare providers had good knowledge score of community based health insurance scheme. The difference in knowledge score was statistically significant as the p=0.035. Mode of premium collection had Odd ratio 2.99 (CI =1.934- 4.622), P<0.001; Trust of the system Odd ratio 2.987 (CI = 1.884-4.733); Quality of health care Odd ratio 2.673 (CI = 1.757-4.065) P<0.001; Proximity to health facility Odd ratio 2.225 (CI = 1.412-3.506) P=0.001. Mode of joining the scheme Odd ratio 0.400 (CI = 0.290-0.552) P<0.001 and cost of accessing care Odd ratio 0.577 (CI = 0.42-0.779) P<0.001. \u0000Recommendation: It was recommended that private health facilities should be maintained which will improve access to health care for the enrollees. Obstetrical ultrasound screening is the prime tool to rule out any possible adverse outcome that might lead towards worst scenarios outcome such as growth retardation, C-section and preterm delivery. The objective of this study was to determine sonographic association of polyhydramnios with adverse perinatal outcomes in diabetic mothers from 34 to 40 weeks of pregnancy. \u0000Methodology: A cross sectional analytical study was carried out at the Department of Radiology in Services Hospital Lahore, Pakistan. Duration of study was from March 2017 to November 2017. Patients in 3rd trimester of pregnancy with Polyhydramnios and diabetes were chosen subsequent to fulfilling consideration (inclusion) and rejection (exclusion) criteria. A complete history and investigation were finished. All necessary examination done. Polyhydramnios confirmed by estimating amniotic fluid index. Statistical Software for Social Sciences (SPSS version 24) was used for analysis of all data.  Standard and Mean Deviation (SD) remained calculated for continuous variables. \u0000Findings: The sample size was 101 in this study. While throughout the study period data of 89 patients was collected.  Patients were between the age group of 18 to 35 years. Severity of polyhydramnios was classified as mild moderate and severe. Out of 89 babies delivered during study 66 had no complications while 4 were macrosomic, 1 infant suffered from shoulder dystocia, 1 was lager for gestational age, 3 had respiratory distress syndrome and 3 were smaller for gestational age. Hb1AC levels were higher in diabetic patients with the mean value of 6.18%.  In this study, we found that polyhydramnios is associated with poor diabetic control. This research shows that polyhydramnios is associated with an increased risk of adverse perinatal outcomes, and there is a significant positive relation with maternal age, diabetes, fetal anomalies, and fetal macrosomia. Pregnant women with GDM showed higher risk of macrosomic newborns. The burden is greatest in the developing countries of the tropics especially Africa south of the Sahara. In Kenya, particularly the Lower Lake Victoria Basin (LLVB), it is blamed on the historical high rainfall, temperature and relative humidity. This study sought to determine spatiotemporal variation of malaria transmission in different altitudes of the LLVB, Kenya. \u0000Methodology: The study relied on data from routine malaria case transmission records archived by the Health Information System for ten years. Data for Kakamega, Kisumu and Migori Counties were obtained from the Kenya Health Information System (KHIS) through Sub-County Health Facilities. Pearson’s Product Moment Correlation Coefficient was used to correlate the suspected and confirmed transmission cases. ANOVA was used in testing the variability of transmission, Time Series to determine transmission characteristics while malaria case transmission tables and Tukeys Honest Significance Difference (HSD) were for testing the significance of the distribution of malaria. \u0000Findings: The suspected cases were found to have been overstated in Migori and Kakamega Counties. Malaria transmission varied by altitude, space and time during the study period. Trends increased in Kisumu and Kakamega Counties while it decreased in Migori County. Transmission depicted both endemic and epidictic characteristics in the study area. \u0000Unique Contribution to Theory, Policy and Practices: Health facilities in the LLVB, Kenya should be equipped with more modern laboratory equipment to improve confirmation of transmission so as to reduce suspicions. Since most of the observations confirmed the varying nature of malaria transmission in relation to altitude, the aspect of blanket assumption concerning malaria transmission in the LLVB, Kenya should be stopped. LLVB, Kenya should be zoned by altitudes for effective mitigation and eradication strategies. The reduction of malaria case transmission in Migori County was an indication of possible future eradication. Its cause should be investigated and inferred to other counties. More research is necessary to establish situations elsewhere for effective management and control of transmissions. A comparative assessment of the impact of traditional cookstoves (TCS) and improved cookstoves (ICS) coupled with the characteristics of kitchen was conducted to estimate the PM2.5 and CO concentrations in the micro-environments of kitchen and living area of the households. \u0000Methodology: The study incorporated both extensive and intensive real-time indoor air quality (IAQ) monitoring during the two cooking sessions of the day. A multi-stage sampling technique was used in this study and the total target population was 383; where 204 households were selected as the sample size for HH survey and 56 households were selected for indoor air pollution monitoring. Influence of the different types of kitchen characteristics; enclosed, semi-enclosed and open was also comprehensively analyzed to measure its impact on the IAQ. Both UCB-PATS instrument and CO loggers were launched in kitchens for 24 hours. The Kitchen Performance Test (KPT) was applied to demonstrate the effectiveness of stove interventions on household fuel consumption. The pollutant concentrations were reported in terms of 24-h, 1-h peak cooking and long-term time scales. Data was analyzed using Statistical Packages for the Social Sciences (SPSS). Multiple regression analysis was undertaken to evaluate the association between pollutant concentration and kitchen characteristics and to determine a set of variables that best predict the pollutants. Units of analysis included means, standard deviations, minimum values, median values and maximum values and IQR. Spearman’s rank correlation coefficients (r) were used to assess the relationship between mean daily kitchen CO concentrations and kitchen PM2.5 concentrations in order to determine whether all kitchen PM2.5 concentrations were as a result of biomass combustion or there were other microenvironment PM2.5 sources. One-way ANOVA was used to compare the quantified fuel use from different stoves and further multiple tests of mean separation were done according to Tukey’s test of significance at p < 0.05. \u0000Findings: The results of the study highlighted that households with improved cookstoves that included the Cheprocket and mud rocket stoves consumed 1.5 kg/day (95% CI: 1.3, 5.8) and 1.3 kg/day (95% CI: 1.2, 5.9) less fuel than households with three-stone stoves respectively. The multiple regression models indicated that well ventilated kitchens (β = 2.556, SE = 1.646, p = .036) using ICS,  with cemented floors (β = -.091, SE = .026, p = .001) using Chepkube stove and higher number of windows (β = -4.475, SE = 2.841, p = .031) using Cheprocket stove;  (β = -.446, SE = .042, p = .030) using rocket stove; (β = -.045, SE = .010, p = .000) using Chepkube stove were associated with lower kitchen PM2.5 concentrations.
