We report a 22-year-old female patient who presented at a teaching hospital, Baghdad, Iraq, in 2023. She presented with right leg bone pain that was exacerbated during menstruation, a presentation that had never been reported in the literature before. The pain intensity increased and interfered with her daily activity. A computed tomography scan showed OO signs and surgical excision and histopathology confirmed OO. The patient's pain reduced to 0 on the visual analog scale. She has no recurrence signs 2 years after the surgery. This novel OO presentation may increase physician awareness of atypical presentation. PubMed, Google Scholar, Web of Science and Scopus were searched for published studies until October 2022. Studies evaluating the sliding sign as a predictor of intra-abdominal adhesions after repeat CD were included. STATA and Comprehensive Meta-Analysis for meta-analysis was used. A total of 7 studies (1,318 patients) were included. For identifying severe intra-abdominal adhesions, sliding sign on TAS had a combined sensitivity (64%, 95% confidence interval [CI]: 55-71%), specificity (93%, 95% CI: 89-96%), positive likelihood ratio (9.5, 95% CI: 5.7-16), negative likelihood ratio (0.39, 95% CI: 0.31-0.49) and diagnostic odds ratio (24, 95% CI: 13-46). Prediction intervals for sensitivity and specificity were 0.444-0.786 and 0.711-0.985, respectively. This study aimed to examine the Wells score's (WS) predictive value for PE, the effect of incorporating sickle cell disease (SCD) on its predictive accuracy and to estimate prevalence of PE in pregnanct and postpartum women.</p><p><strong>Methods: </strong>This retrospective study included pregnant and postpartum women who underwent computed tomography-pulmonary angiography (CTPA) for suspected PE at two tertiary hospitals (Sultan Qaboos University Hospital and Royal Hospital in Muscat, Oman) from 2013 to 2019. The relationship between WS and CTPA was evaluated before and after adding 3 points for SCD.</p><p><strong>Results: </strong>A total of 193 pregnant and postpartum women with a mean age of 31.45 ± 5.60 years underwent CTPA for suspected PE. PE incidence was 0.31 in 1,000 deliveries and 28 women (14.5%) had PE confirmed in CTPA. WS stratified probability of PE as low in 44 (22.8%) patients, moderate in 142 (73.6%) patients and high in 7 (3.6%) patients. In the women with a low score, 2 (4.5%) had PE, giving 95.2% negative predictive value (NPV) of low WS. High WS had a sensitivity of 33.3% and a specificity of 87.0%. Women with SCD had a higher incidence of PE compared to those without (36.7% versus 10.4%, odds ratio = 4.9, 95% confidence interval: 2.0-12.1; <i>P</i> < 0.001). When modifying WS with additional SCD points, NPV for the low score patients was 94.4%, while for the high score group sensitivity and specificity were 72.3% and 85.7%, respectively.</p><p><strong>Conclusion: </strong>The study confirms that low WS effectively excludes PE, reducing the need of imaging in pregnancy and postpartum. This case report describes a 69-year-old man who presented to a tertiary care hospital in Shahrekord, Iran, in 2022 with a history of cough and haemoptysis. He had previously undergone total gastrectomy and adjuvant chemotherapy for gastric adenocarcinoma three years earlier. Imaging and pathological evaluation confirmed endobronchial metastasis with extension to the left atrium. The study population included 98 patients older than 18 years who underwent surgery/adjuvant radiotherapy for breast cancer and developed complications. Patients with tendon/muscle injuries in neck/skin conditions, which made MRT difficult, were excluded. A trained physiotherapist delivered the interventions. TAVR has become the standard of care for patients with severe aortic stenosis at high risk for surgery. However, it is associated with a high PPM implantation rate.</p><p><strong>Methods: </strong>This was a retrospective study involving all patients undergoing TAVR at two leading cardiac centres in Oman over seven years, from May 2013 to September 2020. We included patients who survived the procedure to discharge and those who were followed-up for at least 1 year post-procedure.</p><p><strong>Results: </strong>A total of 153 patients (mean age = 74.2 ± 8.2 years; 82 males [53.6%]) were enrolled in the study. Of these, 15 patients (age = 74.3 ± 9.8 years, 46.6% male) required a permanent pacemaker within 1 year of follow-up, giving a pacemaker implantation rate of 9.8% following TAVR in the cohort. The factors that predicted the requirement of a pacemaker were pre-existing right bundle branch block (odds ratio [OR] = 10.9, 95% confidence interval [CI]: 3.31-36.33; <i>P</i> < 0.001); abnormal QRS axis (OR = 9.11, 95% CI: 2.77-29.91; <i>P</i> < 0.001); prolonged QRS duration (OR = 3.26, 95% CI: 1.06-9.92; <i>P</i> = 0.03); and any pre-existing conduction abnormality (OR = 1.18, 95% CI: 1.08-1.29; <i>P</i> = 0.01).</p><p><strong>Conclusions: </strong>The PPM implantation rates post-TAVR at two Omani institutions are comparable to those reported in the literature. {"title":"Prevention of Post-Mastectomy Seroma in Breast Cancer Patients: <i>A comparative analysis of the effectiveness of fibrin glue versus quilting</i>.","authors":"Ibrahim A Umar, Bello M Bashir","doi":"10.18295/2075-0528.2850","DOIUrl":"10.18295/2075-0528.2850","url":null,"abstract":"<p><strong>Objectives: </strong>This study aimed to evaluate the effectiveness of fibrin glue versus quilting in preventing post-mastectomy seroma formation in breast cancer patients.</p><p><strong>Methods: </strong>This study was a prospective, randomised interventional trial conducted between July 2021 and June 2022 at Usmanu Danfodiyo University Teaching Hospital, Sokoto, Nigeria. It involving patients with breast cancer undergoing modified radical mastectomy. Patients were randomly assigned to Group A (fibrin glue) or Group B (quilting). The level of statistical significance was set at <i>P</i> < 0.05, corresponding to a 95% confidence interval.</p><p><strong>Results: </strong>A total of 42 patients were recruited for the study. The total volume of drainage for Group A was lower, and there was a statistically significant difference (<i>P</i> = 0.00) between the two groups. Similarly, the evaluation of the length of hospital stay and duration of drainage revealed a shorter period for Group A, with a statistically significant difference (<i>P</i> = 0.00) between the two groups. However, a comparison of seroma formation and post-mastectomy wound infection between the two groups revealed no statistically significant differences (<i>P</i> = 1.00).</p><p><strong>Conclusions: </strong>This study has demonstrated that there was no statistically significant difference in the effectiveness of fibrin glue compared to quilting in preventing post-mastectomy seroma in breast cancer patients.
