Distinguishing a distant lesion in breast cancer patients can be challenging. Therefore, obtaining a histopathological confirmation of a metastasis is advisable, as a suspicious metastatic lesion may turn out to be benign or exhibit different immunohistochemistry compared to the primary site. In this case, we describe a woman undergoing staging scans for newly diagnosed breast cancer, where radiological findings suggested appendix metastasis. However, subsequent laparoscopic appendicectomy revealed an appendicular schwannoma, confirmed through immunohistochemistry. The patient then received curative-intent breast cancer treatment. Previously reported in South India, but the resurgence of cases is currently reported in Eastern India. This study aimed to estimate the prevalence and describe the clinico-laboratory profile of scrub typhus in paediatric patients (1-12 years old) in Eastern India. Methods: A prospective observational study was conducted from January to December 2019 in a paediatric tertiary care centre in Kolkata. All acute undifferentiated febrile illness cases (1-12 years) were tested for scrub typhus serology by ELISA. Demographic details, clinical features, laboratory findings, complications and treatment outcomes of scrub typhus patients were extracted in Microsoft Excel spreadsheet and further analysed. Results: The prevalence of scrub typhus among acute febrile illness patients was 4.5 %. The mean age of patients was 5.22 years and the majority (64.2 %) had a fever for the last 7-14 days. Gastrointestinal symptoms like vomiting (43.3 %) and pain abdominal (32.8 %) were frequently seen. Major clinical signs were hepatomegaly (41.8 %) and splenomegaly (31.3 %). Complication was observed in 74.6 %, with thrombocytopenia (40.3 %) and meningoencephalitis (29.9 %) being more frequent. The case fatality rate was 1.5 %. Conclusions: Classical eschar was absent in three-fourth of our patients, hence we advocate laboratory scrub typhus testing for all suspected cases in endemic region. Thrombocytopenia and meningoencephalitis were prominent complications in our study. They are seen as round nodules mostly in the paediatric age group in infants less than 2 months of age and disappear spontaneously within 6 months of life. We report a unique case of an 11-month-old girl who presented at a tertiary healthcare hospital in 2022 with a blood-filled cyst on the posterior leaflet of the pulmonary valve that was successfully treated. This case report highlights the characteristics and course of a paediatric patient with blood-filled cysts. We report a case of PIWD in a 22-year-old Omani male patient with post-concussion syndrome and epilepsy presented to a tertiary care hospital in Muscat, Oman, in 2021 for evaluation of paroxysmal events. His behaviour of PIWD was misinterpreted by his family until characterised in the epilepsy-monitoring unit as a manifestation of epilepsy that was treated medically. We report this case series of 10 patients with successful SAB with newly launched 0.75% hyperbaric ropivacaine, in patients with recent scorpion sting. It is caused by reactivation of varicella-zoster virus (VZV) that lies dormant in the geniculate ganglia. It can be complicated by VZV encephalitis rarely. We report the case of an 8-year-old previously healthy boy who presented to a tertiary care hospital in Muscat, Oman in 2021 with fever, progressive left ear pain, vesicular rash around his ear pinna and left-sided facial nerve palsy. His course was complicated by VZV encephalitis where he was managed with intravenous (IV) acyclovir and IV corticosteroids. We report a rare case of chemical peritonitis and prolonged fever following laparoscopic salpingo-oophorectomy for torsion of a large ovarian dermoid and discuss the management of this patient with prolonged hospital stay, antibiotics and anti-inflammatory use, repeated drainage of the collection as well as re-laparotomy. The occurrence of this rare condition can be extremely distressing for the patient and treating surgeon alike, as the recommendations for management are limited. Only a few cases of nephropathy have been reported in association with tubo-ovarian/peritoneal malignancies. We report a 55-year-old female patient who presented to a tertiary care centre, Muscat, Oman, in 2022. She developed combined immune complex-mediated glomerulonephritis and pauci-immune necrotising crescentic vasculitis simultaneously with the diagnosis of tubo-ovarian/peritoneal cancer. The baseline estimated glomerular filtration rate (eGFR) was 13 mL/min. The patient received two doses of rituximab and three doses of pulse corticosteroids, leading to significant improvement in renal function and the disappearance of her proteinuria. The eGFR improved to >60mL/min; her proteinuria gradually resolved after 10 weeks of treatment.
