Benyass","doi":"10.47191/rajar/v10i03.01","DOIUrl":"https://doi.org/10.47191/rajar/v10i03.01","url":null,"abstract":"Introduction: Chronic kidney disease (CKD) represents a distinct risk factor by itself for the development of coronary artery disease (CAD). Notably, CAD stands as the primary driver of both sickness and death among individuals diagnosed with CKD. Furthermore, individuals with CKD tend to experience worse outcomes when it comes to CAD. In addition to conventional risk factors, numerous factors associated with uremia, including inflammation, oxidative stress, endothelial dysfunction, coronary artery calcification, elevated homocysteine levels, and the use of immunosuppressants, have been linked to an increased risk of accelerated atherosclerosis. Objective and method: In this study, we aim to assess the differential effect of renal impairment across the spectrum of patients with acute coronary syndrom. We conducted this study in the intensive care unit of cardiology in the Military Teaching Hospital of Rabat. Results: A total of 318 patients were included in the study. The average age was 63.8 +/- 9.41 years and 77% (244/318) were male. Normal kidney function was reported in 72.8% (220/318) of patients. Of the 318 included patients, 121(38.3%) were presented with STEMI, 154 (48.7%) with NSTEMI, and 41 (13%) with unstable angina. The difference of age was significant with a p-value of <.001 with a mean of age of 61.8 in patients with normal renal function vs 68.1 in impaired renal function patients. Patients with impaired GFR had more history of previous PCI (21) compared to those with normal GFR (23) with a p value of 0.001. Hb was lower (12.5vs 13.9 p<0.01) and Grace score was higher (45 vs44 p<.001) in patients with impaired GFR vs patients with normal GFR. Percentage of cardiogenic shock and death was respectively higher in renal impairment (12(60%) vs 8 (40%) with p of 0.004); (10(62.5%) vs 6(37.5%) with p 0.005. Objective: To find out how common Helicobacter pylori infection is among patients at Al Sadder Teaching Hospital in Al-Najaf, Iraq, as well as related factors including age, gender, residency, and blood group. Methods: The entirety of 197 patients—79 men and 118 women—of both genders and various age groups—from under 10 to around 60 years old—were reported to have symptoms of gastrointestinal inflammation. Result: The total numbers of participants in the current study were 197 patients, found 156 (79.2%) positive and 41(20.8%) negative results. Females had a greatest percentage of H. pylori (47.2%), while males had the lowest incidence of prevalence (32%). The age group (31-40) had greatest percentage with (14.7%), followed by the age group (41-50) with (13.2%), and then the age group (21-30) with (11.7%) while lowest percentages in age groups < 10 and ˃ 60 years old. The blood group type O, which had the greatest positive rate (27.9%), was followed by the blood groups types A, B, and AB, with 16.8%, 13.7%, and 4.1%, respectively. People who live in rural areas (30.4%) have a elevated frequency of H. pylori infection when compared with persons who reside in urban areas (28.4%). Conclusion: The findings showed that H. pylori was more frequent in women than men, in rural areas, and in patients between the ages of 31 and 40. Aim of the Study: The aim of this thesis is to investigate the occurrence of genes encoding clinically important 16S rRNA methylases and AMEs enzymes in aminoglycosides resistance P.aeruginosa isolates recovered from Najaf hospitals and the characteristics of these isolates. Methods: a total of 68 specimens were isolated from patients at burn center in Najaf City from the whole study period. The 24 isolates were screened for the sensitivity versus 3 various antibiotics disks of the one class by using Kirby-Bauer disk diffusion method. Results: The resistance rate of 24 isolates of XDR Pseudomonas aeruginosa against aminoglycoside groups: gentamicin, amikacin, and tobramycin were 91.6 %, 83.3 %, and 66.6 %, respectively. However, the resistance profiles of the isolates show that 22 (91.6 %) were resistant to 1 or more antibiotics of aminoglycoside. Present study show that only 3 (12.5%) of XDR isolates harbored ant(4')-IIb gene isolates and none of the XDR isolates investigated were positive for rmtA and aac(6')-IIb gene. One hundred and thirty one (131) cattle fatteners were interviewed based on a questionnaire focusing mainly on the conduct of the fattening activity; the constraints encountered and the impact of the fattening activity on the living conditions of the respondents, including observations in the field. A descriptive analysis is made with the analysis software SPSS-18 has shown that 95 (72.5%) men against 26 (27.5%) women practice cattle fattening activity. Respondents encounter some difficulties; only 16% benefited from loans from financial institutions. Cattle rationing is mixed, the most used feeds being brown cake (90.8%) and rice bran (85.5%). The acquisition of veterinary products is done on the local market (83.2%), itinerant merchants (6.9%) and ordered from outside (9.9%). The aim of the cattle fattening activity is to increase the pecuniary income for a better living condition. Pharmaceuticals reach the nature through the discharge of effluents from urban waste water treatment plants, the discharge of effluents from hospitals and drug factories, the using of sewage sludge in agriculture, the breeding of herbivorous animals, the treatment of pets, the spreading of manure and aquaculture, the improper disposal in landfills of unused pharmaceutical substances and contaminated waste. The interdimensional diagnostic Scale has been applied and coded for 10 dimensions: 1) conceptual units, 2) signifiers, 3) hierarchy, 4) nodal relationships, 5) categorical relationships, 6) Recovery, 7) social interaction, 8) social communication, 9) stereotypical behaviours, and 10) restrictive behaviours. A total of 75 participants with ASD have participated in this study of three ASD´ intensity levels and different age intervals, from 3 years old. Data analyses focused along study the factorial analysis determinant (KMO and Bartlett's test) statistic, as well as, bivariate correlations analysis for three dimensions calculated statistically: \"processing\", \"social\" and \"behaviours\", show significant critical inter-relational levels (sig: .00), which allows conclude to existence of highly relationships between both variables group and their practical applications for consequent, reliable and valid diagnosis process. Zambia Correctional Service in particular has been undergoing a paradigm shift in order to implement new security sector reforms. Like many other public entities world over has continuously recorded failure in achieving strategic objectives, as noted by cabinet Office’s 2020 performance assessment. Therefore, this study sought to identify challenges that affect effective application of strategic management techniques in the process of implementing organisational changes. This research is a literature review that gathered facts from literature findings of a scientific publications and existing data in the study area of the topic under review. Research findings indicate that the Service has been experiencing a number of challenges and these include; having an ambiguous vision statement, operating on an outdated organisational structure, inadequate end-user engagement and ineffective examination and modification of work processes. The findings show that the Service needs to develop a model for application of strategic management techniques that will support implementation of organisational changes.
