Although Cognitive Behavioral Therapy (CBT) and pharmacotherapy are considered first-line treatments, not all patients respond sufficiently. The case of a young woman demonstrates that a psychodynamic short-term therapy over five weeks can lead to marked symptom reduction (Y-BOCS: 39.5 → 19). In particular, the assessment of decision-making capacity in individuals with mental disorders presents significant challenges. This case report describes a patient with recurrent depressive disorder and histrionic personality disorder who died during day-clinic psychiatric treatment with the support of a right-to-die organization. The external psychiatric assessment was based on a single video consultation conducted twelve days prior to her death. The case illustrates how depressive symptomatology and ambivalent treatment attitudes may complicate the evaluation of a self-determined wish to die. {"title":"[Children exposed to interparental intimate partner violence: An examination of the health care situation in trauma outpatient clinics].","authors":"Lina Specht, Lea Bentz, Marc Giesmann, Isabella Flatten-Whitehead, Miriam Rassenhofer, Ingo Schäfer, Sibylle Maria Winter, Julia Schellong","doi":"10.1055/a-2835-8912","DOIUrl":"10.1055/a-2835-8912","url":null,"abstract":"<p><strong>Objective: </strong>The new Social Compensation Act extends the care to relatives of victims of violence, including children exposed to interparental intimate partner violence (i-IPV).The German health care situation for rapid interventions in trauma outpatient clinics (TOC) remains unclear.</p><p><strong>Method: </strong>A nationwide survey in N=104 participating TOC (N=234 invitations), examined the care of exposed children. At a child-TOC (c-TOC) the psychopathology and treatment success (N=71 children) were assessed.</p><p><strong>Result: </strong>Domestic violence is frequent cause of consultation in adult TOC (a-TOC) but only 30.3% of the children were referred to c-TOC. There 74.7% were impaired in psychosocial functioning, 59.1% received a F43-diagnosis of ICD-10. 57.7% reached symptom improvement.</p><p><strong>Conclusion: </strong>Children exposed to i-IPV are underrepresented in a-TOC. Data on expectations of psychiatric patients in Germany are lacking.</p><p><strong>Methods: </strong>108 patients from three psychiatric outpatient clinics completed a semi-structured questionnaire. Data were analyzed using descriptive statistics and qualitative content analysis.</p><p><strong>Results: </strong>Patients expected increased transparency, improved preparation and follow-up, and more intensive communication with clinicians. Reported challenges included comprehension difficulties and emotional burden. Concerns about data security and workload were also expressed. The overall estimators of the meta-analyses across all model projects were calculated.</p><p><strong>Results: </strong>69,711 patients were analyzed (including 6,726 children). Adults in model projects had 4.5 fewer inpatient days in the first year (95% CI: -6.8; -2.3 days). There were no significant differences in sick leave days (-1.2 days; 95% CI: -7.2; 4.9 days). This study examines implementation models, incentives, prerequisites, challenges, and effects.</p><p><strong>Methods: </strong>Interviews with clinic representatives (n=13) and delegation partners (n=8) were analyzed using content analysis.</p><p><strong>Results: </strong>Three models emerge: cross-sectoral concepts, overcoming geographic barriers, and personnel substitution. Incentives include reimbursement and service expansion. Cooperation requires visibility, contracts, cost calculation, and information exchange. Challenges concern workflows, coordination, and team development. Prevalence follows a U-shaped pattern across the life course, with especially high rates in vulnerable groups. Its emergence reflects an interplay of individual, relational, community and structural factors. Longitudinal evidence links loneliness consistently to depression, anxiety, suicidality, cardiometabolic disease, cognitive decline and mortality. Targeted interventions can reduce loneliness, though the growing use of generative AI as emotional support introduces ethical and psychosocial concerns.
