Bashir, Mohid Rehman","doi":"10.25301/jpda.313.110","DOIUrl":"https://doi.org/10.25301/jpda.313.110","url":null,"abstract":"OBJECTIVE: To determine the frequency of patients with various anatomical forms of the hard palate for complete denture fabrication and to compare the mean distortion in heat cure denture base polymer in millimetres in different hard palate forms. METHODOLOGY: Informed consent was taken from total of seventy six patients and divided into low and medium hard palate forms. Dental casts were prepared from alginate impression and poured in type III stone and reference point R was marked on the deepest part of the posterior palatal seal area at the junction of hard and soft palate on each cast. After curing, the cast along with the cured denture base was retrieved from the flask. After 48h of curing cycle the distortion was measured in millimetres from R to R' via traveling microscope. RESULTS: Among seventy six participants the frequency of low palate forms was 45 (59.2%) and medium palate forms was 31 (40.8%). For each sample three readings were taken R1, R2 and R3. The mean distortion measured in low hard palate form was 0.52mm with a standard deviation of 0.18, the mean distortion measured in medium hard palate form was 0.76mm with a standard deviation of 0.27, which were clinically significant with a p-value of 0.0001. CONCLUSION: The hard palate forms has direct influence on retention of maxillary complete denture in posterior palatal area. {"title":"Efficacy of Calcium Hydroxide and Mineral Trioxide Aggregate in the Formation of Dentin Bridge – A Randomized Controlled Trial","authors":"Rafia Ruaaz","doi":"10.25301/jpda.313.114","DOIUrl":"https://doi.org/10.25301/jpda.313.114","url":null,"abstract":"OBJECTIVE: This study is to compare the dentin bridge thickness achieved using calcium hydroxide and MTA using radiographs. METHODOLOGY: Single blinded randomized controlled trial conducted in the Operative Dentistry department at Fatima Jinnah Dental College and Hospital, Karachi. A total of 100 premolar and molar teeth with class I and II cavities were included in this study. The study participants were assigned into two groups, A and B of 50 participants each. Under local anesthesia, Group A was indirectly pulp capped with Calcium hydroxide (Dycal) and Group B received Mineral Trioxide Aggregate as an indirect pulp capping material. Both groups were then restored with Glass Ionomer Cement. Radiographic follow up was carried out at three and six months to determine mean dentin thickness of reparative dentin bridge. RESULTS: Statistical analysis was performed using SPSS v 23. Independent Sample t-test was applied to evaluate the formation of dentin bridge formation using Ca(OH)2 and MTA at 3 months & 6 months, the outcomes were highly significant (p-value<0.001). Paired sample t-test was applied to evaluate the difference in dentin bridge formation at three months and 6 months, the results were highly significant (p-value <0.001). CONCLUSION: Statistically significant difference was observed in the dentin thickness of reparative dentin bridge amongst the two groups after three months and six months. A greater success rate was noted in the MTA group as compared to the Ca(OH)2 group after 6 months. METHODOLOGY: A cross-sectional study was conducted from May 2017 to January 2018 among 190 conveniently sampled practitioners from four dental clinics of tertiary care hospitals of Karachi. The study population consisted of dental practitioners that included graduates and post graduates of either gender with an experience of two years and above while practitioners refusing to give written informed consent were excluded from the study. The questionnaires were distributed and collected on same day by the principal investigator. All the collected data were analyzed on Statistical Package for Social Sciences version 20. RESULTS: A majority of dental practitioners (>90%) were aware that contact with blood may transmit pathogenic microorganisms; goal of infection control is to eliminate the transfer of microorganisms; steam autoclave sterilizes by using steam under pressure; disinfection of dental chair, clinic, and dental office; wearing gloves, face masks and having protective eye wear on while checking patients; changing gloves, extraction instrument, saliva ejectors and burs in operative dentistry after checking each patient is necessary for a dental practitioner. Moreover, the awareness scores of dental practitioners were found to be significantly different across categories of experience (p=0.008). CONCLUSION: A majority of dental practitioners was aware about most measures of infection control. Further confirmation of study results by studies with larger sample sizes is recommended. REVIEW: Over the past few decades lasers have become a popular alternative to conventional methods owing to the advantages they carry such as decreased cellular destruction and tissue swelling, minimal bleeding, enhanced visualization of surgical sites and reduced requirement for suturing. Among the various lasers in dentistry, diode lasers are currently the most commonly used, with applications extending from soft tissue procedures to photo-activated disinfection of periodontal pockets, endodontics and laser-assisted tooth whitening. With its versatility and numerous benefits, small size, ease of operation and, relative and cost-effectiveness when compared to other lasers, the diode laser is proving itself as a valuable addition to dental setups. METHODOLOGY: This was an analytical cross-sectional study which was conducted in Out Patient Department of Peshawar Dental College, Peshawar from February to April 2019. The sample comprised of 180 individuals with a mean age of 15-25years selected from a population visiting Peshawar Dental College that fulfilled the inclusion criteria. JPEG file format of OPG images were graphically analyzed by Autodesk AutoCAD software 2017 version to calculate the mesiodistal widths of permanent maxillary anterior teeth. Independent t test was used to compare mesiodistal teeth dimensions between males and females. RESULTS: Mesiodistal widths of permanent maxillary anterior teeth, in males, were greater than females but the difference in their mean values were statistically insignificant (P>0.05). The mesiodistal width of permanent maxillary right lateral incisor in males was larger (8.023± 1.083 mm) than females (7.853± 1.319 mm) but the difference was statistically insignificant (P>0.05). CONCLUSION: It can be concluded that gender had statistically non-significant association with the mesiodistal widths of permanent maxillary anterior teeth. It is a safe procedure with high success rate. One of the main issues with root canal treatment is post-operative pain. There are many factors which can cause pain and distress to patients. If the clinician follows the guidelines properly and use the anxiety reduction protocol, then the procedure is not painful. RCT can be performed in multiple visits or it can be done in single visit. Present research was conducted to compare the single visit versus multiple visits endodontic treatment in teeth with pulpal necrosis in terms of post-operative pain frequency. METHODOLOGY: This study was a randomized controlled trial conducted at Department of Operative Dentistry, Fatima Memorial Hospital, Lahore, over a period of six months. Total number of participants was 302 and the teeth were lower and upper first and second molars with pulp necrosis and they were were equally assigned in two groups of 151with equal number of men and women. Single visit RCT was done in Group A patients, in which the endodontic procedure was performed in one day and multiple visit RCT was done in Group B, in which endodontic treatment was done in two or more visits. RESULTS: In necrosed teeth single visit endodontic treatment 20.5(%) resulted in less frequent post-operative pain than in multiple visit endodontic treatment (26.5%). CONCLUSION: Single visit endodontic treatment is a better technique in terms of pain control as compared to multiple visit in teeth with pulp necrosis. The outcome of the study will help in determining if the routine consideration of ANB angle while the lack of defined thresholds for convexity of soft tissue profile justified for choosing the treatment modality. METHODOLOGY: Lateral cephalograms of 141 skeletal class II subjects (ANB>4°) were obtained. Angular parameters including soft tissue angle of facial convexity (SA-FC) and angle ANB were determined. Gender dimorphism of the variables was assessed by Mann Whitney U test. Correlation between SA-FC and angle ANB were determined utilizing Spearman's correlation coefficient. RESULTS: The angle SA-FC and ANB depicted moderately positive correlation (r = 0.662, p<0.001). Gender dimorphism exists with increased mean value of ANB (7.88±1.90) and SA-FC (23.22±7.61) in females. CONCLUSIONS: Angle SA-FC depicts moderately positive correlation with angle ANB among skeletal class II subjects, hence suggesting the need of soft tissue guidelines along-with hard tissue parameters for selection of treatment modality. Retention of a complete denture is the major factor to resist the forces of dislodgement along the path of insertion. In complete denture fabrication, fovea palatinae is the guideline for the posterior extension in the maxilla. Fovea palatinae mucous gland secretion helps in the retention by forming a thin layer within the maxillary complete denture. METHODOLOGY: The study was conducted in Sharif College of Dentistry in Lahore, Pakistan on a sample size of two hundred and fifty dentate, partially dentate and edentate patients with different age groups. Clinical examination of the maxilla was conducted for assessment of the vibrating line, whether it was at, anterior to or posterior to the fovea palatinae. The researcher used an indelible pencil to mark the ah line i.e vibrating line and its distance with fovea palatinae was measured. RESULTS: Results were concluded using SPSS Version 23. The fovea palatinae were mostly found to be located at 1.68 mm±1.11 anterior to the vibrating line. Thus, the dentists should assess this distance for the posterior extension of the maxillary denture. CONCLUSION: Fovea palatinae location in relation to vibrating line is an important landmark for posterior palatal seal in a complete denture. In the majority of cases, fovea palatinae were located 1.68 mm±1.11 anterior to the vibrating line. METHODOLOGY:The dental records of all the dental patients (n= 1824) who attended the QDH during first wave of COVID-19 Pandemic peak were retrieved and divided into 3 groups based on diagnostic conditions:1) - non-emergency (NE), 2)- Emergency endodontic (EE), and 3) - Emergency non-endodontic (ENE). The age groups were also divided into 3 group of 0 to 25, 26 to 50, and >50. Categorical variables were reported as %. Chi-square test was used to find inter-group difference. p <0.05 was regarded as significant. RESULTS: Dental records of 1823/1824 patients were analysed. Results suggested that 54.6% of the patients were females and 50.1% of patients belonged to age range 0 to 25. Moreover, 1040 (57%) patients suffered with EE conditions, and 805 (77.4%) with symptomatic irreversible pulpitis. Analysis showed that significantly higher number of females and patients with age < 50 suffered with EE as compared to males and age >50 (p <0.05). Significantly higher number of patients aged >50 years had ENE conditions as compared to 0 to 25 age group (p= 0.007) CONCLUSIONS: Emergency endodontic conditions, especially symptomatic irreversible pulpitis was most prevalent during COVID-19 pandemic. This condition was experienced more by females and patients of age group <50 years. Efficient allocation of resources and cross-training is recommended to facilitate treatment of such patients during the pandemics.
