The aim of this study was to evaluate the effect of the presence of hernia sac and defect size on the prognosis of CDH in Tabriz children’s hospital. Methods: In this prospective study, all the infants with the diagnosis of CDH that were admitted to the Tabriz children’s hospital since 2016-2019 were enrolled. The presence or absence of a hernia sac was evaluated, and the largest diameter on diaphragmatic defect was measured before the repair. Postoperative outcomes, including instability, duration of mechanical ventilation, mortality rate, or hospital discharge were obtained and analyzed. Results: Over the study period, 30 patients with CDH met the inclusion criteria, consisting of 15 males and 15 females. Fifteen cases were diagnosed during the first 24 hours of life and 16 cases experienced hypoxia in the first hour after birth. Defect size was significantly smaller in survived patients (P=0.041). Postoperative death occurred in 9 (42.85%) patients without a hernia sac. Results showed that none of the cases with hernia sac expired (P=0.035). Conclusion: In our study, the presence of the hernia sac was related to a lower postoperative mortality rate. Defect size in survived group was also smaller. It can be suggested that the presence of hernia sac and lower diameter of defect size act as good prognostic factors in the outcome of patients with CDH.","PeriodicalId":17071,"journal":{"name":"Journal of Research in Clinical Medicine","volume":"11 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-03-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82155195","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The incidence of thrombotic complications in mild disease is not clear and probably few cases have been identified. We reported a case of a 60-year-old man with no previous history admitted to our unit for splenic infarct and a floating clot into the descending aorta without signs of severe disease. Several mechanisms to clarify prothrombotic state have been described. However, the exact prevalence of thromboembolic phenomenon is probably underestimated. There is no consensus about the treatment and the indications for preventing these complications in non-hospitalized patients. Thrombotic events should be suspected also in healthy patients with symptoms suggestive for coronavirus disease infection but without severe COVID-19 pneumonia. Further analysis should be performed to stratify the risk in nonhospitalized patients and the indications for prophylactic treatment.","PeriodicalId":17071,"journal":{"name":"Journal of Research in Clinical Medicine","volume":"18 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-02-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80498494","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This systematic review is designed to qualitatively analyzed the interaction between autophagy and aging in various organs of animal models. Methods: Based on our primary search, 9478 identified and following screening ultimately 80 full text were included to proceed further analysis. Next, by using SPSS software, data analysis of autophagy and aging related markers as well as autophagy alternation over the course of aging was performed. Results: Despite of debatable results, we established that the majority of studies showed that the autophagy process reduced in different aged organs significantly. Conclusion The outcomes showed that autophagy induction during aging were inferior to those reports that indicated the therapeutic potential of autophagy. Taken together, it should be considered that autophagy inducers could be counted as anti-aging agents.","PeriodicalId":17071,"journal":{"name":"Journal of Research in Clinical Medicine","volume":"48 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-02-09","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79452585","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Vitamin D affects thyrocytes by decreasing TSH stimulated iodide uptake and cellular development. In hypothyroidism, hypovitaminosis D are due to malabsorption from the intestine and the other mechanism is inactivation of vitamin D. Thyroid hormones induce erythropoietin gene expression causing increase secretion of erythropoietin. Iron deficiency decreases activity of thyroid peroxidase enzyme. Hypothyroidism can cause microcytic anemia due to malabsorption of iron and menorrhagia. Hypothyroidism is a common but under-recognized and under-diagnosed condition in the Persian Gulf countries. Moreover, the effect of hypothyroidism on vitamin D and iron levels were inadequately evaluated. This is important to identify bi-deficiency so that, doctors can treat them earlier and can reduce the deficiency-related complications. There are plenty scopes for study on bi-deficiencies status of hypothyroid population.","PeriodicalId":17071,"journal":{"name":"Journal of Research in Clinical Medicine","volume":"28 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81249823","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: The patients were evaluated in two groups according to both their skiing experience and use of protective equipment. The characteristics of the patients, such as age, gender, injury area, and injury type were statistically evaluated according to the groups. Results: A total of 191 patients, 68 (35.6%) female and 123 (64.4%) male, were evaluated. There was a significant difference between the skiing type of the patients with and without skiing experience (p=0.001). When the patients were evaluated according to the presence of protective equipment, 25 (59.5%) patients who had no experience did not wear protective equipment, and this rate was statistically significant compared to the experienced group (p=0.001). Concerning the diagnoses of the patients according to injury areas, joint dislocation and soft tissue trauma were mostly detected in the upper extremities, bone fractures in the lower extremities, and organ injury in the thorax. Conclusion: Shoulder and wrist traumas were the most common injuries among the patients with skiing experience while tibial fractures were mostly seen in those without such experience. Tibia fractures were mostly observed in the patients using protective equipment, and skin incisions in those that did not use protective equipment. Therefore, in winter sports injuries, patients’ skiing experience and protective equipment use should be questioned and more attention should be paid to the above-mentioned injuries.","PeriodicalId":17071,"journal":{"name":"Journal of Research in Clinical Medicine","volume":"13 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80300679","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: for this purpose, 60 patients who did surgery for middle- (n=35) and lower-thoracic esophageal cancer (n=25) were recruited. Then, preoperative plasma D-dimer levels were measured in a quantitative and qualitative manner and their relation with postoperative outcomes in each type of cancer was assessed for six months. Results: With regard to the middle-thoracic esophageal cancer, complications were observed in 14 cases and deaths were reported in six individuals. Considering the lower type, morbidity was found in 11 cases and mortality was seen in five patients. In both types of cancer, plasma D-dimer levels were significantly higher in patients with complications or deaths compared with those who were not. Sensitivity, specificity, as well as positive and negative predictive value (P/NPV) of plasma D-dimer levels were 90.91%, 78.57%, 76.92%, and 91.67% for diagnosing morbidity of the lower-thoracic esophageal cancer and 80%, 85%, 57.14, 94.44% for predicting mortality in this type of cancer. Moreover, the given values were equal to 71.43%, 76.19%, 66.67%, and 80% for diagnosing complications of the middle-thoracic esophageal cancer and 83.3%, 72.41%, 38.46%, 95.45% in predicting deaths induced by this type of cancer. Conclusion: It was concluded that elevated plasma D-dimer levels could be accompanied by adverse events and early poor postoperative outcomes for esophageal cancer.","PeriodicalId":17071,"journal":{"name":"Journal of Research in Clinical Medicine","volume":"10 3 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-12-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75005630","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: Contrast-enhanced CT scans of the patients were retrospectively reviewed regarding PPAA. The patients with PPA were classified into BD, HSS, and idiopathic groups according to the etiology. The groups were compared for demographical features including age and gender, multiplicity, distribution thrombosis and accompanying pulmonary artery embolism(PAE), and deep venous thrombosis(DVT) history. Results: A total of 30 PPAA (25.4±13.4 (11-62) mm) were detected in 10(2.3%) (mean age 39.8±22-1.0[8-73] years, female/male:3/7) among 4391 patients reviewed. In 7 patients multiple aneurysms were detected while in 3 a solitary lesion was seen. Most commonly lower lobes (right 8-left 8, 53.2%) involvement was observed. A thrombosis was detected within 19(63.4%) aneurysms. Among 10 patients with PPAA 4(40%) ones have BD, 2(20%) HS and 4(40%) idiopathic origin. In 5(50.%) patients there was accompanying PAE and 3 (30%) DVT history. Patients with BD nad HSS tended to have multiple lesions than with idiopathic origin. Accompanying PAE was observed in 2(100%) of HSS, 2(50%) BD, and 1(25%) patient in the idiopathic group. A DVT history was recorded in 2(100%) of HSS, 1(25%) BD. None of in the idiopathic group had a DVT history. The only rupture was observed in the HSS group. Conclusion: Vasculitic diseases lead to PPAA, including BD and HSS are more likely to be associated with complications and additional morbid conditions than idiopathic processes.","PeriodicalId":17071,"journal":{"name":"Journal of Research in Clinical Medicine","volume":"37 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-12-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78332511","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Therefore, proper initiations of nursing interventions are crucial for ensuring early stabilization after birth. The present study aimed to evaluate the impact of a multi-method training program on knowledge and practices in neonatal nurses. Methods: This quasi-experimental research was carried out with a pre-test–post-test and intervention-control groups design. The population consisted of 60 nurses working in the neonatal and neonatal intensive care unit (NICU) of two hospitals in Iran. The samples were selected using the convenience sampling technique. A researcher-made questionnaire was used. The validation and reliability of the questionnaire were confirmed. The nurses received a multi-method training for two weeks. They were evaluated three months later. Results: The mean knowledge score obtained in the pre-test was 30.33 ± 2.01, and it was47.73 ± 6.82 three months after the intervention. There was a statistically significant difference in the trend of changes in the scores of the two groups (F = 125.72, P < 0.001). The mean performance score in intervention group before the intervention was 60.63 ± 5.69 and after the intervention was 81.77 ± 6.37, indicating a significant difference in the trend of changes(F = 90.285, P < 0.001).The correlation between knowledge and practice was significant at the 0.01 level (r = 0.544,P = 0.002). The previous in-service training, individuals’ education, and thermoregulation knowledge were determined as predictors of performance (r = 0.651, P = 0.013). Therefore, clinically differentiating these diseases is difficult. We investigated the diagnostic value of 99m Tc-TRODAT-1 SPECT in this setting. Due to the fact that this modality has some limitations in imaging small organs like the sub-regions of basal ganglia, we also evaluated the use of anatomical MR imaging along with functional SPECT imaging in parkinsonism. Methods This follow-up diagnostic test evaluation study was performed with 40 patients with the clinical presentation of parkinsonism, and 10 healthy subjects as controls. After administration of the radiopharmaceutical, SPECT images were acquired, then co-registered on MRI. Uptake values were evaluated in basal ganglia semi-quantitatively. Results In this study, 99mTc-TRODAT-1 SPECT was able to differentiate essential tremor and healthy subjects from progressive supranuclear palsy (PSP) and Parkinson’s disease (PD) with a sensitivity of 76.47% and specificity of 100% at a cut-off of 0.53; however, findings were not significant in differentiation of PD from PSP (p ˃0.05), and the results were similar in SPECT and co-registered MRI/SPECT images. In evaluation of the uptake pattern in basal ganglia, the lateralization of decreased uptake was only seen in PD; and in PSP, the dysfunction was bilateral in all patients. Conclusion 99mTc-TRODAT-1 SPECT is sensitive and specific in diagnosing basal ganglia dysfunction; however, 99mTc-TRODAT-1 SPECT alone or co-registration on MRI are not adequate in differentiation of the etiologies of basal ganglia dysfunction.","PeriodicalId":17071,"journal":{"name":"Journal of Research in Clinical Medicine","volume":"109 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-11-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79590512","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
