{"title":"Operation STICK Improves Vascular Access Outcomes in a Specialty Team: A Quasi-Experimental Study.","authors":"Steven Matthew Gibson, Jessica Sarlabous, Lauren Adair, Amit Bahl","doi":"10.1097/NCQ.0000000000000801","DOIUrl":"10.1097/NCQ.0000000000000801","url":null,"abstract":"<p><strong>Background: </strong>Education and training may improve outcomes within an experienced vascular access team.</p><p><strong>Purpose: </strong>The purpose of this study was to measure the impact of a standardized peripheral intravenous catheter insertion educational program (Operation STICK) on vascular access outcomes.</p><p><strong>Methods: </strong>A quasi-experimental study design was used. Adult inpatients with difficult intravenous access (DIVA) requiring ultrasound-guided insertion by a vascular access specialty team were eligible. Data were collected before and 7 months after the educational training program.</p><p><strong>Results: </strong>A total of 54 subjects were included (24 pre- and 30 post-implementation). Significant improvements were found for mean peripheral intravenous catheter dwell time (3.62 to 14.97 days, P < .001), completion of therapy (58.3% to 96.7%, P = .001), and first-stick success rate (79.2% to 100%, P = .013). Critical appraisal of evidence is a fundamental component of evidence-based practice (EBP) and is also needed to evaluate the quality of quality improvement (QI) projects.</p><p><strong>Problem: </strong>Currently available EBP or QI critical appraisal tools can be challenging for students learning the critical appraisal process and for practicing clinicians who desire access to a standardized EBPQI approach to inform health care decision-making. The content was organized through a scoping review of 4 databases. Twelve health professionals participated in focus groups and 10 experts participated in content validation. The content validity coefficient (CVC) was calculated for agreement between experts.</p><p><strong>Results: </strong>Three patient safety culture domains were identified: organizational, professional, and patient and family participation. The experts rated the definitions favorably, with high CVC scores (>0.87), indicating good agreement. Nurses reported a moderate to heavy burden regarding care for those with diarrhea.</p><p><strong>Methods: </strong>A pre-post implementation design was used to evaluate the impact of evidence-based interventions for diarrhea screening and prevention.</p><p><strong>Interventions: </strong>A diarrhea risk screening tool and countermeasures were developed through an extensive literature review. Tools were integrated into the electronic health record system.</p><p><strong>Results: </strong>The rate of diarrhea decreased from 37.14% to 17.33% ( P = .000). A secondary aim explored potential effects of misinformation on EBP.</p><p><strong>Methods: </strong>Descriptive qualitative inquiry was performed with a focus group of 6 clinical nurse leaders.</p><p><strong>Results: </strong>EBP expectations were identified, discriminating between novice entry level and advanced-level nurses. Participants emphasized asking questions, linking evidence to practice, and acknowledging that evidence changes over time. All advanced-level nurses were expected to apply, lead, and teach EBP. The Task, User, Representation, and Function framework guided the research.</p><p><strong>Results: </strong>The survey participants were moderately satisfied with the SM. Opportunities to design this system to be more efficient and maximize patient safety were identified.</p><p><strong>Conclusion: </strong>Improving training and workflow can aid in incorporating SM into clinician's daily routines, focusing on enhancing user satisfaction. The current standards of postoperative care rarely consider frailty status.</p><p><strong>Local problem: </strong>There was no standardized protocol to optimize specialized postoperative care for frail patients at an academic medical center.</p><p><strong>Methods: </strong>A quasi-experimental pre-/postimplementation study design, using the Reach, Effectiveness, Adoption, Implementation, Maintenance implementation framework, was utilized.</p><p><strong>Interventions: </strong>A frailty-specific postoperative order set (FPOS) was developed, including tailored nursing care, activity levels, and nutritional goals.</p><p><strong>Results: </strong>There were significant improvements in nurse's self-reported familiarity with frailty ( P = .003) and FPOS awareness ( P < .001).
