All clinicians face a diagnostic dilemma because of the clinical polymorphism and the multisystemic sarcoidosis symptoms. After the lungs, the skin is the organ most frequently impacted by sarcoidosis. Before, during, or after systemic involvement, dermatological symptoms can occur, and the kind of skin lesion can have a bearing on the prognosis. Additionally, compared to a biopsy of visceral organs, a skin lesion biopsy is easier to perform and less intrusive. Therefore, there are some instances in which cutaneous symptoms can help with the diagnosis and outlook of a systemic illness. A high index of clinical suspicion is needed for early diagnosis and timely management, which can improve the quality of life for those patients and reduce morbidy and mortality due to this disease. {"title":"Giant Chorioangioma in a Patient with 36 Weeks of Pregnancy: A Case Report","authors":"B. Mehrotra","doi":"10.18535/jmscr/v11i10.04","DOIUrl":"https://doi.org/10.18535/jmscr/v11i10.04","url":null,"abstract":"Abstract Present report relates to a 40-year-old patient with 36 weeks of pregnancy and non-trophoblastic benign placental neoplasm chorioangioma (placental hemangioma). The incidence of chorioangioma has been described as 0.5 to 1.0 % of all pregnancies. Tumors may be >4 cm in diameter; such tumors are called giant tumors. Smaller tumors are generally asymptomatic. However, clinically-significant chorioangioma occur less frequently; incidence varying from 1 in 3500 to 1 in 9000 pregnancies. Giant chorioangiomas are very rare tumors; only a few cases have been reported. Feto-maternal complications like fetal growth retardation, anemia and hydrops may develop in a patient with chorioangioma. Further, fetal loss may be as high as ~ 40 %. Increased incidence of chorioangiomas may occur at high-altitude pregnancies, suggesting role of hypobaric hypoxia in pathogenesis of chorioangioma. When the Left ventricular ejection fraction (LVEF) percentage reduces to less than 35%, it indicates the presence of severe systolic dysfunction. Both general anaesthesia and regional anaesthesia carry potential risks due to these patients' low cardiac output status. Hemodynamic fluctuations associated with anaesthesia can be detrimental in these patients. A clear intraoperative plan should be designed to manage the hemodynamics of these patients. Here we report two cases of ischemic heart disease with low ejection fraction posted for orthopedic surgeries. Graded epidural anaesthesia was used as a sole anaesthetic technique. The patients were hemodynamically stable in the intraoperative as well as postoperative period. The neurologic disorders associated with thyroid dysfunction span the entire spectrum of neurology. Symptoms range from disorders of emotion and higher cognitive function to movement disorders, neuromuscular diseases and a range of rarer yet significant neurologic sequalae. Myopathy is the most common of the peripheral neurologic manifestations in both hypothyroidism and hyperthyroidism. Compressive mononeuropathies, namely carpal tunnel syndrome, are a major feature in hypothyroidism. At the level of the central nervous system, psychosis, personality change, mood disorder, confusion, dementia, coma, and seizures can occur in either hypothyroidism or hyperthyroidism. Comprehensive management approach by surgical intervention by drainage and debridement of the affected area and long- term multidrug anti-tubercular therapy, resulting in complete healing of the sinuses. This case report highlights the importance of considering atypical pathogens in the differential diagnosis of surgical site infection and comprehensive management strategy. Left ventricular thrombus (LVT) is an important complication in patients with ischemic heart diseases and in those with dilated cardiomyopathy and systolic heart failure. The diagnosis of left ventricular thrombus remains important since anticoagulation will reduce the risk of systemic embolization and stroke. Despite advances in other imaging modalities, echocardiography remains the most important tool for diagnosis and risk stratification in patients predisposed to develop left ventricular thrombi. We are presenting a case report of 62 years old gentleman who had suffered from anterior wall myocardial infarction (AWMI) in the recent past, and now has developed LVT which was detected by contrast tuned imaging (CTI) technology based contrast echocardiography. This is a first case report of detection of LVT by this ingenious technology. Objectives: this systematic review aims to compare and evaluate the efficacy of conservative (non-surgical) and surgical approaches in treatment of CTS. Methods: Using predefined keywords related to this condition, an extensive search of medical databases, including PubMed and Cochrane library, was carried out to find and select randomized controlled clinical trials evaluating the effects of conservative treatment compared to surgical treatment released up to August 2023. The quality of the included research was rigorously evaluated, and data were extracted. After reviewing pertinent papers for relevance and quality, data was gathered for analysis.
