{"title":"Effective Ayurvedic Management of Steroid Resistant Nephrotic Syndrome - Case Study","authors":"Dr Sarita Pradip Gaikwad","doi":"10.18535/jmscr/v11i10.05","DOIUrl":"https://doi.org/10.18535/jmscr/v11i10.05","url":null,"abstract":"Nephrotic syndrome presents with heavy proteinuria (3.5 g Proteins/ 1.73 m2 body surface area/24 hour), dyslipidaemia/ hypercholesterolemia, hypoalbuminema, edema and hypertension. If left untreated or undiagnosed, there is progressive damage to glomeruli causing renal failure. Heavy proteinuria is the most characteristic feature of this syndrome. Several studies have noted that higher the 24-h urine protein excretion, more rapid is the decline in GFR. It may be primary or secondary to other systemic diseases. Case study: This 2 year old male child developed Nephrotic syndrome and was placed on steroids, used to relapse when steroids are withdrawn, and subsequently developed steroid resistance. He was poorly managed by modern line of treatment for last 10 years. The case had developed Cushingoid features due to long term use of steroids. This case was successfully managed by Ayurvedic treatment principles of Deepan-Pachan, Vatalunoman and Mutral chikitsa. Steroids were withdrawn gradually and completely. There was no relapse. He was free of proteinuria. His Cushingoid features disappeared. Edema and Ascites also disappeared. This case study is the example of importance of alternative medicine in treating steroid dependent/resistant cases. When modern treatment has definite limitations to treat such cases, it is ray of hope to thousands of poor sufferers of Steroid resistant Nephrotic syndrome, finding them an effective alternative of Ayurvedic treatment. IIH most commonly affects women of child-bearing age and overweight women2 . However, men, women of all ages and children of both genders are affected3 . Previously the terms like pseudo tumor cerebri and benign intracranial hypertension were used; however, in view of impending loss of vision it cannot be considered benign. Hence the term IIH is used nowadays. The diagnosis of IIH was made using modified Dandy criteria which includes presence of raised intracranial pressure, papilloedema, normal magnetic resonance imaging (MRI) and elevated CSF opening pressure with normal CSF composition. This study aim to correlate the infarct location and functional outcome of the patient. Materials and Method: This is a Prospective study done at MES Medical College, between December 2020 to June 2022, total 44 patients were admitted and taken MRI with a diagnosis of acute middle circulation ischemic stroke within 48 hours of symptom onset. Location of infarct is correlated with clinical outcome using a modified Rankin score taken at 45 days follow up. Results: Most of the infarct were located in the right hemisphere. {"title":"Emperipolesis, Apoptosis and Hemophagocytosis during Acute Epstein-Barr Virus (EBV) infection: A Case Report","authors":"B. Later, he developed left-sided cervical lymphadenitis. Subsequently, he also developed palatal petechiae. Absolute lymphocyte count (ALC) was raised (ALC 20,262 cells/mm³). Hepatosplenomegaly was also detected. Clinically, the current case was diagnosed as a case of infectious mononucleosis. Possibility of infection by Epstein-Barr virus was considered. FNAC was done. Blood-mixed aspirate showed monocytoid cells with mild pleomorphic nuclei, and clumped chromatin. Inconspicuous nucleoli and abundant eosinophilic cytoplasm were seen. Foci of apoptosis were also seen. Frequent mitoses were seen. Findings were suggestive of acute reactive lymphocytic hyperplasia with hemophagocytosis and apoptosis. EBV DNA was detected by RT PCR with linear reporting range of 3.3×104 IU/ml. Herewith, we describe a case of infectious mononucleosis with emperipolesis and hemophagocytosis. It is linked to high mortality and morbidity. Cholinergic syndrome, intermediate syndrome, and syndrome of delayed polyneuropathy are the three primary syndromes that characterize OP poisoning. Organophosphates are strong toxins that are frequently used as pesticides in agriculture. Patients who have consumed organophosphorus may exhibit subclinical symptoms of acute pancreatitis. Correct biochemical analysis and clinical correlation assist in diagnosis. In the study, we described a 24-year-old female who suffered necrotizing pancreatitis after OP poisoning. Busy working life, indifference to ideal living, addiction to yummy junk food and lack of physical activity are causing many health hazards. Due to changes in lifestyle and dietary habits, the prevalence of several diseases has increased, among which diabetes can be identified as one of them. Diabetes is a chronic disease that affects people of all ages. There are several forms of diabetes. Type 2 is the most common. The number of people in Bangladesh who have type-2 diabetes has increased dramatically making it one of the country's major health problems. The rising prevalence of diabetes in Bangladesh is associated with overweight, age, family history of diabetic, physical activity, eating habit of high carbs food etc. So, we collected 320 individuals affected or take medication or checks themselves in any Diabetic Center in Gopalganj district for treatment or any suggestions. The aim of the study is to explain the status of diabetes affected peoples on average result and to find out any certain factor which relates to the disease. The variables included the age of the patient, job category, exercise time, meal plan and some other important factor that is considered to be related to diabetes. The dependent variable was chosen as the blood glucose level of the respondents. We used chi-square test and logistic regression to find any significant relation that can be described. Logistic regression analysis identified some important factors that significantly affect the blood glucose level of the respondents, namely average sleep time, rice for breakfast, diet, red meat, smoking and age. Pterygium is mostly prevalent on the palpebral aperture mostly in the nasal side. It is thought that the predominantly nasal location is related to reflection of light from the nasal bridge on to the nasal conjunctiva.
