These observations highlight the virus's potential impact on cardiovascular health, necessitating vigilant monitoring and timely interventions to mitigate complications. Study Objective: This research aims to assess electrographic abnormalities and troponin elevation in COVID-19 patients within Bangladesh. Methods: Conducted retrospectively at a tertiary medical college and hospital, our study enrolled 100 consecutive adult COVID-19 patients. We analyzed baseline characteristics, lab results, and serial ECGs. Troponin I concentrations were measured using the Abbott Architect method (normal range: ≤0.05 ng/mL). Troponin levels were categorized as normal (0.00-0.05 ng/mL), mildly elevated (0.05-1 ng/mL), and significantly elevated (>1 ng/mL). ECGs were reviewed and interpreted. Results: The majority of patients (71%) were males aged 41-50. Common COVID-19 symptoms included dyspnea and cough (75%), followed by fever, fatigue, and malaise (80%). Gastrointestinal symptoms were seen in 20%, and 10% experienced anosmia or dysgeusia. Comorbidities included diabetes mellitus (65%), hypertension (60%), CKD (10%), coronary artery diseases (13%), and other cardiovascular diseases (5%). Abnormal LFTs were observed in 30%. Initial troponin levels averaged 0.18 ± 1.75 ng/mL, with a maximum of 2.55 ± 17.90 ng/mL. Mortality occurred in 25% of cases. Conclusion: Our findings suggest that ECG repolarization abnormalities may serve as markers of clinical severity, even in cases with mildly elevated troponin values in COVID-19 patients. Its epidemiology depends on the serum sodium used to define hyponatraemia, which varies from study to study. It can be an early or even first sign of important underlying disease. It can be complicated by two types of neurological disorders cerebral oedema and osmotic demyelination syndrome. It is associated with increased morbidity and mortality rates in hospitalized patients, although it remains unclear whether these associations are mainly due to the underlying disease, direct effects of hyponatraemia, or a combination of both. Aims and Objectives - to study clinical profile of hyponatremia in hospitalized subjects. To study the aetiology of hyponatremia in hospitalized subjects. To assess morbidity and mortality due to hyponatremia. Materials and Methods - 100 patients with hyponatremia were studied. Most common symptoms were Lethargy and tremors, followed by headache, nausea, altered sensorium, convulsion and coma, in order. Hypervolemic hyponatremia is most common followed by euvolemic hyponatremia and hypovolemic hyponatremia. Euvolemic subjects had propensity to develop more severe and symptomatic hyponatremia. Dilutional hyponatremia is the most prevalent underlying pathophysiology of hyponatremia followed by diuretic use, SIADH, GI loss, Hypothyroidism, extra renal/GI loss and Cerebral salt wasting in order. Congestive cardiac failure was the most common cause of hypervolemic hyponatremia, followed by cirrhosis of liver and renal failure. Acute gastroenteritis was most common cause of hypovolemic hyponatremia. Through a detailed analysis of clinical cases, we explore the complexities associated with TB reactivation post-cancer therapy and the hurdles in timely diagnosing lung cancer in tobacco users. Three cases are presented: A 60-year-old male with a history of pulmonary TB, bidi smoking, and a diagnosis of adenocarcinoma after presenting with persistent cough and imaging findings suggestive of lung cancer. A 57-year-old male, post-cancer treatment, presenting with cough, diagnosed with TB after immunosuppression, highlighting diagnostic challenges in this population. A 63-year-old female with a prolonged cough and blood streaks in sputum, not responding to anti-TB treatment, ultimately diagnosed with lung cancer due to tobacco use. These cases underscore the need for vigilant monitoring and early intervention in post-cancer individuals vulnerable to latent TB reactivation, comprehensive diagnostic approaches for immunocompromised patients, and heightened awareness of the association between tobacco use and lung cancer. The study emphasizes the importance of thorough assessments, timely interventions, and collaborative efforts among healthcare providers to optimize outcomes in patients with overlapping health concerns. In conclusion, this study sheds light on the complexities of managing patients with a history of cancer, TB, and tobacco use, advocating for a holistic approach in diagnosis and management to improve patient outcomes and quality of life. Further research and clinical guidelines are warranted to refine treatment strategies and enhance the care of individuals facing these dual health challenges. She was operated. Biopsy was taken from the left iliac crest and histopathological examination was done. Tumor measured 3×2×2 cms approximately. It was chalky-white ball-like. Its surface was lobulated. Microscopically, it showed small and large areas of necrosis and calcification. Areas of mild-to-moderate lymphocytic infiltration were seen. Fibrosis and hyalinization were seen. Bony trabeculae and osteoid formation were also seen. The patient was finally diagnosed as a case of primary tumoral calcinosis of sacroiliac region. However, its practical implication in gastric cancer prognosis or treatment by restoration of mutated p53 function is yet to be fully exploited. Hence, this study with the aim of assessing the yield of p53 expression in gastric carcinoma and its relationship with various clinicopathological parameters was formulated. Objectives: To assess immunohistochemical expression of p53 protein in different histological subtypes of gastric carcinoma cases and to study correlation between p53 protein expression and different clinicopathological parameters like age, gender, tumour location, gross pattern, histological types, degree of tumor differentiation and pathological staging in the above said cases. Materials and Methods: This was a hospital based cross-sectional study done over a period of 18 months on 60 cases of gastrectomies with proven histological diagnosis of gastric carcinoma. Observation and Results: Strong p53 expression was frequent among elderly male patients (>60 years) who had ulceroproliferative tumors, located at pyloric antrum belonging to pTNM stage IIIA. In 87% cases, it was expressed, predominantly in moderately differentiated intestinal type with serosal invasion. Also, cases even with no lymph node metastasis showed p53 expression. Conclusion: Few findings were contradictory to other similar studies e.g. lymph node negative cases showing P53 expression does not support the hypothesis of predicting lymph node status by p53 expression. No significant correlation between p53 expression and different clinicopathological parameters in gastric carcinoma cases was seen. Chronic alcohol use triggers oxidative stress and inflammation that damages liver cells. Glutathione (GSH), a tripeptide consisting of gamma glutamyl cysteinyl glycine possesses a thiol group and participates in oxidation reduction reactions, acting as a principal cellular scavenger of free radicles. GSH is present in large concentrations in the liver, and its endogenous levels are depleted in ALD, exacerbating the condition. Intravenous GSH supplementation has shown promising results in improving liver function and reducing fibrosis markers in ALD patients. Intravenous GSH treatment has demonstrated the potential to reduce oxidative injury and mortality rates in patients with sepsis in the intensive care unit. The versatility of GSH in mitigating oxidative stress, inflammation, and tissue damage and proven safety and tolerability profile make it a valuable adjunct to current treatments. Further research is imperative to comprehensively unravel the therapeutic benefits of GSH injection adjunct to standard of care in patients with ALD and sepsis. Despite significant efforts to address external ventricular drain-related infections (ERIs) in CCUs and other critical care units, current studies suggest a high prevalence rate of external ventricular-related infections, particularly in LMICs and low-resource settings. Material and Method: A mixed-method study was conducted at KNH Critical Care Units with a sample of 81 nurses. Data analysis of Quantitative continuous data was summarized using medians and interquartile ranges, and Categorical data was summarized using frequencies and proportions. Qualitative data was analyzed using manual thematic analysis. Results: Most respondents were between 30 and 40 years old, 43.2%, females were 75.3%, higher diplomas were 54.3%, and 4.9% specialized in EVD care. 48.1% had good knowledge of the EVD care bundle, 46.9% had fair knowledge, and 5% had poor knowledge. Binary logistic regression and chisquare tests showed no factors associated with knowledge of the EVD care bundle (P>0.05 chi-square). A basic diploma and no specialization in EVD care were associated with higher odds of a poor or fair knowledge of the EVD care bundle. Physician appearances, gender, nationality, age, speech and posture were found to affect patient’s first impression(3) . Also, it is found that once the first impression been developed, it will remain unchanged – most of the times- (3) . Gender preferences found to increase patient satisfaction in some settings(4) . For the intimate, and psychosocial issues, it is found to be preferred to have a physician of the same gender as a patient (4) . For the age preference, some studies found that patients preferred the physician to be between 30- 50 years (5) . CT examination revealed a non-succable tumor at anterolateral aspect of dura. Tumor extended from the right cervicomedullary junction upto C7 level. Tumor was excised and multiple soft tissue pieces, together measuring 1.5×1×1 cm were cellected. All the pieces were sectioned. Tumor had a sieve-like and cyst-like appearance. Section showed a circumscribed globular tumor which consisted of proliferated bland spindle cells. Tumor had an edematous stroma and presented a microcystic appearance. The cells had elongated ovoid nuclei and mild eosinophilic cytoplasm. Several thin-walled vascular spaces were also seen in the tumor. Strong positive reaction was obtained with an anti-S100 antibody, suggesting a neural origin of tumor cells. Further, anti-Ki 67 antibody gave mild reaction (Ki67 index was 12%) suggesting benign neoplasm. Anti-CD 34 and anti-D2.40antibodies gave positive reactions (2+) with small blood vessels of the tumor.
