It usually affects women in the reproductive age group. In men, it is associated with gynecomastia. While the exact etiology is unknown, PASH is considered hormonally responsive. The mammography and ultrasonography findings are nonspecific. Grossly, PASH is a well-circumscribed, firm, rubbery mass with solid, homogenous, gray-white cut surface. On histologic examination, it is characterized by the presence of open slit-like spaces in dense collagenous stroma. The spindle cells express progesterone receptors and are positive for vimentin, actin, and CD34. PASH lesions should be surgically excised if increasing in size or associated with symptoms, also in cases of suspicious imaging findings or other lesions are synchronously diagnosed that require excision. In other small incidentally detected asymptomatic cases observation with clinicoradiological follow-up can be considered. It is associated with a good prognosis with no increased risk to develop breast cancer. Hence, the operating surgeon and pathologist should be aware of this not so uncommon entity so that patients are not subjected to unnecessary excision and subsequent physical and mental morbidity associated with it. Malnutrition and deficiencies can be prevented or treated by optimal medical nutrition therapy, yet limited specific knowledge about micronutrients remains among clinicians. Further knowledge on trace elements remains miniscule as compared to vitamins. Practical recommendations based on available micronutrient guidelines are therefore required regarding procedures for determination of requirements and, especially for gastrointestinal diseases as this is the commonest encountered problem that can impair enzymatic as well as biochemical processes and lead to multiple complications including death. Here we have tried to give consensus based recommendations to help practicing dieticians and clinicians making decision regarding specific micronutrient deficiencies and their implications in gastrointestinal diseases. In low-middle-income group countries (LMICs), patients are mostly undernourished. They need cost-effective, easily available and acceptable nutritional support. The study has been undertaken to evaluate the cost-effectivity of homemade recipes vs commercially available nutritional formulations for patients on liquid diet. Recipes for both vegetarian and nonvegetarian were designed using low-cost and locally available ingredients to fulfil the daily calorie requirement of ≈2,000 kcal (30 kcal/kg for a 65 kg adult) with ≥90 gm of protein (1.5 gm/kg). Alongside, five commercial nutritional formulations were evaluated to meet the above requirements. The caloric (380 ± 80.1 kcal/100 gm, 260–474 kcal) and protein contents (32.6 ± 10.9/100 gm, 15–49 gm) in commercial supplements were variable. For providing 2,000 kcal with minimum of 90 gm protein, the cost of commercial formulations ranged from INR 745 -3,461/day (1,700 ± 946). Comparatively, the vegetarian and non-vegetarian homemade recipes were estimated to cost just INR 135/day and INR 182/day respectively. For the 2000 kcal diet, the difference in nutrient contents of commercial vs homemade preparations were non-significant, while the cost was significantly lower with homemade recipes (p=0.03). Homemade recipes designed and suited to individual needs could meet the dietary requirements for patients on liquid diet, be highly cost-effective and thereby especially suit patients from resource constraint settings, like LMICs. Lack of sunlight exposure is the major cause of vitamin D deficiency, as very few foods contain natural vitamin D. Several epidemiological and random clinical trials have reported a close association between low vitamin D levels and cardiovascular disease development. This paper aims to discuss vitamin D deficiency and its role in developing cardiovascular disease. Vitamin D is an essential fat-soluble vitamin which plays an important role in skeletal maturation. Vitamin D deficiency is highly prevalent globally and is linked to a spectrum of cardiovascular disorders and is thought to promote atherogenesis. We have done a thorough review of the literature using a Pubmed search. Different keywords like vitamin D deficiency, cardiovascular diseases, coronary artery disease, atherosclerosis, heart failure were used to write the review. Here we report according to current evidence, vitamin D deficiency is associated with a higher risk of cardiovascular diseases. However, further study is necessary to evaluate whether vitamin D supplementation has a beneficial effect on cardiovascular outcomes. An observational analysis on mortality and survival rates were conducted in patients with head and neck squamous cell carcinoma (HNSCC) that received cancer treatment ONS plus 2 g of eicosapentaenoic-acid (EPA) versus standard treatment (EN). Twenty-six participants were available (81.3% of the original cohort [n=32]) in the ONS-EPA 2 g and 24 (75% of original cohort [n=32]) in the ONS-standard groups. Mortality at five years was 50% in the ONS-EPA 2 g and 58.3% in the ONS-standard group but with longer survival time in the ONS-EPA 2 g than the ONS-standard group (35 versus 18 months, respectively). A trend toward a longer survival time five years was observed in participants who received ONS with a high EPA content compared to those who received standard-formula ONS. Results may represent a possible long-term benefit of using high EPA supplementation during active cancer treatment to address malnutrition and positively influence clinical outcomes. Intestinal failure is defined as significant deficit in gut absorption of micro and macronutrients such that intravenous supplementation is necessary to sustain life. If absorption is inadequate, however intravenous supplementation is not required, it is called intestinal insufficiency. Short bowel syndrome and entero cutaneous fistula are the most common conditions leading to intestinal insufficiency and failure. These conditions in the initial phase may lead to significant loss of fluids and electrolytes and require multidisciplinary team management. Definite plan for restoration of adequately functioning bowel after stabilization of patient is important. Based on evidences, recommendations for nutritional management in such patients were thoroughly discussed and approved by the Gastrointestinal nutrition core committee of the Indian Association of Parenteral and enteral nutrition (IAPEN).
