Chem. 31, 1779-1789 (1985)), total bilirubin was determined in 77 neonatal serum samples (concentration range 63-444 mumol/l, average value 227 mumol/l). Four of the routine methods (Jendrassik & Grof's (Biochem. Z. 297, 81-89 (1938)) method, Hertz's (Scand. J. Clin. Lab. Invest. 33, 215-230 (1974)) method, the bilirubinometer procedure, and the method employing 2,5-dichlorophenyldiazonium (Scand. J. Clin. Lab. Invest. 29, Suppl. 126, Abstr. 11. 12. (1972]) gave values that were generally higher than those of the reference method. In contrast, the results from Vink's (Clin. Chem. 34, 67-70 (1988] direct spectrophotometric method differed only negligibly from those of the reference method. The accuracy of Jendrassik & Grof's method, and to a limited extent that of the 2,5-dichlorophenyldiazonium method, can be improved by redetermination of the molar absorption coefficient, or by using a standard containing a matrix of human albumin, with an assigned value determined by the reference method. It was found that Hertz's direct spectrophotometric method can be replaced by that of Vink. The accuracy of the bilirubinometer results could be improved only by using calibrators with assigned values specific for the bilimeter, or by calibration with a serum pool.</p>","PeriodicalId":15649,"journal":{"name":"Journal of clinical chemistry and clinical biochemistry. The ELISA assay is based on polyclonal (rabbit) antihuman cholinesterase and a highly specific monoclonal (mouse) antibody, with a commercially available peroxidase-conjugated (rabbit) antibody directed against mouse immunoglobulins as the signal carrier. The photometer response was linear up to an absorbance of 2.8. The maximal CV of photometric imprecision was 0.5% for the wavelength pair 340/405 nm within the absorbance range 0.9 to 2.4. For the 13 analytes in our study, mean within-run imprecision was less than 2%, and mean between-day imprecision less than 2.5%. The results obtained with the Hitachi 717 instrument correlated closely with those of comparison instruments. Linearity for the various tests was high and exceeded the manufacturer's claims. No drift was detected during an 8-hour work period; carry over could not be detected under the chosen experimental conditions. The analytical evaluation which was performed in line with the ECCLS recommended kit evaluation protocol showed a marginal improvement in precision with the new assays. There was a substantial improvement in the theoretical limit of detection utilising the IRMA kits although it may prove difficult to realize this improvement in practice with individual samples because of differing protein matrices. The evaluation also demonstrated the degree of parallelism and range of linearity of both kits as well as inaccuracies when compared with the International Reference Preparation for parathyrin (IRP 79/500) as the accepted standard. The demonstration of lack of agreement between measured and kit assigned results for standards when cross-over studies between kits were performed may highlight a possible contributing factor to inaccuracy. Alternatively there may be a difference of antisera avidity within the kits for intact parathyrin. Whilst minor differences in sample stability were demonstrated between the kits, the sample stability was much improved compared to that for intact parathyrin measurement by RIA. Both for the whole blood cell count and the leukocyte differentiation we obtained intra- and inter-assay coefficients of variation fully comparable with those for other cell counters. The carry-over between samples was negligibly small. The linearity range was broad, which implies a universal application in routine haematological analysis. The numerical results from the Sysmex NE-8000 and the Technicon H-6000 corresponded closely, with exception of the mean corpuscular volume and the haematocrit, whereas these parameters showed excellent agreement between the NE-8000 and the E-4000. In the differentiation of leukocytes, the NE-8000 and H-6000 showed good correlation and conformity for lymphocytes, neutrophils and eosinophils; for monocytes and basophils the conformity was poor. We also found a good correlation between the NE-8000 and E-4000 for lymphocytes and neutrophils, whereas the correlation was less for the middle cell ratio and the sum of monocytes, eosinophils and basophils. Furthermore, we compared Receiver Operating Characteristic curves for the different leukocyte subtypes on the NE-8000, the H-6000 and the E-4000, and investigated the flagging frequency in inpatients, outpatients and blood donors.</p>","PeriodicalId":15649,"journal":{"name":"Journal of clinical chemistry and clinical biochemistry. We now describe a study on the analysis in heparinized venous and heparinized finger stick blood. Cholesterol assays in venous serum and plasma, and in finger capillary blood and plasma (all with heparin) were compared. Finger capillary blood was obtained in two ways: 1. in heparinized capillaries; 2. in a Becton Dickinson microtainer that could be centrifuged for separation of the plasma. Venous blood was obtained in plain tubes (for serum) and heparinized tubes to obtain venous whole blood and also venous plasma. We did not, on average, find large differences between the concentrations of cholesterol in the various materials. The regression equations for finger capillary whole blood and venous whole blood however show higher slopes than the others. Fifteen small cell lung carcinomas, 24 non-small cell lung carcinomas and 96 benign pulmonary diseases were investigated. Lung biopsies or bronchial washings were obtained from about 75% of the patients, including all patients with neoplastic diseases. Serum neuron-specific enolase was measured by a recently introduced enzyme-immuno assay (WaKo NS-Enolase EIA-II testkit). The results obtained with this kit were similar to those based on RIA assays. Receiver Operating Characteristic curves (ROC curves) were constructed for comparison of the discriminating ability of neuron-specific enolase and carcino-embryonic antigen in small cell lung carcinomas and non-small cell lung carcinomas. For small cell lung carcinomas the sensitivity and the specificity of neuron-specific enolase (cutoff value: 10 micrograms/l) were 87% and 88%, respectively, and for carcino-embryonic antigen values 60% and 77% were obtained. There was no correlation between neuron-specific enolase and carcino-embryonic antigen in small cell lung carcinoma patients. The diagnostic value of neuron-specific enolase and carcino-embryonic antigen in non-small cell lung carcinomas is illustrated by sensitivities of 13% and 58%, respectively. An extensive literature survey is included to allow comparison with other studies. In 13/21 patients with chronic progressive external ophthalmoplegia the muscle mitochondrial DNA was shown to be heteroplasmic. Further mapping by use of several restriction enzymes yielded large deletions in muscles from 10/13 chronic progressive external ophthalmoplegia patients. Most of the deletions spanned large parts of the mitochondrial genome, leading to loss of mitochondrial genes encoding several subunits of the respiratory chain complexes I (NADH-dehydrogenase), IV (cytochrome c oxidase) and V (ATP-synthetase), as well as of several tRNAs. Comparison of the mapping data with the histochemical and biochemical results did not provide a clear correlation between the location of the mitochondrial genetic defects and the functional deficiencies of the affected respiratory chain complexes. In the majority of patients with chronic progressive external ophthalmoplegia, but without a family history of the disease, restriction analysis reveals large mutations of the mitochondrial genome, while other methods are necessary for the localization of defects in all cases with maternal transmission of the disease. The same holds true for all other kinds of mitochondrial myopathies based on defects within the nuclear DNA or on derangements of the \"cross-talk\" between the nuclear and the mitochondrial genomes.</p>","PeriodicalId":15649,"journal":{"name":"Journal of clinical chemistry and clinical biochemistry.
