Evaluation of Post-Cementation Sensitivity After Cementation with Bio-Active Luting Cement - A Randomized Clinical Control Trial 评价生物活性Luting骨水泥固接后的敏感性-一项随机临床对照试验

The sensitivity to cold stimulus after cementation can be indicative of a gap present below the crown or a marginal gap connecting to opened tubules which emerge into the pulp. A pulp fluid outflow occurs as a result of the contraction of dentinal fluid in this gap which causes postoperative sensitivity. Thus luting agents should form complete seal between fabricated restoration and prepared tooth.Objectives The current study was conducted to clinically evaluate post-cementation hypersensitivity of bioactive luting cement in a randomized clinical control trial using resin modified glass ionomer cement RMGIC as the control.Methods According to standard tooth preparation protocol the teeth were prepared to receive Porcelain fused to metal PFM bridges on both sides of the mouth. PFM final restorations were cemented using Bioactive and RMGIC luting cement. Sensitivity was tested by using compressed airbiting pressure tests and cold water and was evaluated by using Schiff scale with 0-3 scores before cementation soon after cementation one week and three months post cementation.Results Almost all the patients responded to hypersensitivity at pre-cementation and baseline with compressed airbiting pressure tests and cold water. Pharmacological treatment improves the symptoms but increases the adverse effects of the latter and worsens the situation again. Keeping this in mind non-pharmacological interventions are currently used or under supervision to bypass the complications of pharmacological treatment. This article aims to review the prevalence incidence causes signssymptoms and most importantly the success rate of non-pharmacological interventions. Articles in PubMed Cochrane database and Web of Science were analysed till date to establish the effectiveness of different modalities such as acupuncture electrostimulation hyperbaric oxygen HBO therapy and low-level laser therapy LLLT etc. Newer treatment modalities such as gene therapy and stem cell therapy are under trial. Apart from alcohol consumption, tobacco and arecanut chewing, other risk factors for oral cancer include chronic infection and inflammation, poor nutritional status, chronic trauma to the oral soft tissues etc. Studies have also found that the risk of developing oral cancer may increase with periodontal diseases like periodontitis. Chronic periodontitis is a multifactorial, inflammatory disease of the periodontium primarily caused by the pathogenic microorganisms present in dental plaque biofilm, ultimately resulting in loss of the tooth. An induction of carcinoma by such chronic inflammatory disease appears possible since the involved inflammatory mediators and bacterial toxins in the disease have shown to have a potential for transformation into malignancy. Therefore, establishing a connection between chronic periodontitis and oral carcinoma would be of great interest. Moreover, it seems judicious to take measures to improve oral hygiene in order to minimize the cancer risks. Pharmcoviliglance is a important tool in health care field by helping in assessment, monitoring and discovery of newer drugs for the benefit of patients. Pharmcovigilance also plays role in checking drugs interaction among other drugs and adverse drug reaction of any drug which can cause harm to the humans. Dental field is involved in various drug uses in treatment of oral conditions. Dentists are aware of drugs side effects but they should be aware of adverse drug reactions of other drugs. It is possible that loss of preload is the root cause for screw loosening. Several factors affect the connection between torque and preload. Neurofibroma usually occur on skin and is rare in the oral cavity. In the literature intraoral neurofibromas are reported in 7% of the cases. Clinically neurofibroma appears as discrete, nonulcerated nodule or diffuse mass of tissue which tend to be of the same color as the normal mucosa. Neurofibroma is usually asymptomatic, but sometimes pain and paresthesia may be associated with it. Clinically intraoral neurofibromas resemble many other soft tissue lesions. Hence, recognizing and diagnosing intraoral neurofibromas is necessary for careful planning of conservative treatment and follow up to rule out intraosseous extension and malignant transformation. Surgical excision with regular follow up is the treatment of choice. They are most effective at high inflammation sites and are a potent activator of T regulatory cells. Hence this study aimed to assess gingival crevicular fluid levels of IL-35 in healthy subjects and chronic periodontitis patients and to compare the levels of IL-35, before and after scaling and root planing in chronic periodontitis patients. Methods: Forty subjects were selected and categorised into 3 groups. Group 1: Periodontally healthy individuals, Group 2A: Chronic periodontitis patients and Group 2B: Group 2A patients who were evaluated 6 weeks after scaling and root planing. Gingival bleeding index, probing pocket depth and clinical attachment level were recorded. Gingival crevicular fluid samples were collected and stored at -800 C till they were subjected to analysis by Enzyme Linked Immunosorbent Assay (ELISA). Results: On intergroup comparison, the difference in mean IL-35 levels and clinical parameters were highly statistically significant between Group 1 and Group 2A while between Group 1 and Group 2B were not significant. On comparing Group 2A and Group 2B, the mean differences were highly statistically significant P (<0.001). Significant reductions in all parameters were noted in Group 2B. Conclusion: Significant reduction of IL-35 levels in chronic periodontitis patients, 6-8 weeks after scaling and root planing and their presence in periodontally healthy subjects suggest the role of IL-35 in controlling the inflammation and their protective role in maintaining periodontal health. Key message: IL-35 has an effective role in controlling the inflammation and in maintaining periodontal health. They may occur either in the crown, between the crown and root, or in the root. Although not so common, impacted tooth exhibiting root dilaceration pose a diagnostic and treatment challenge to the clinician. Common causes of treatment failures in such cases are primarily related to procedural errors such as ledges, fractured instruments, canal blockages, zip and elbow creations. Knowledge of tooth anatomy and its variations is essential for the success of endodontic treatment. A detailed and thorough assessment of preoperative radiographs along with careful approach yielded into a safe and a successful endodontic treatment of such teeth. YEN angle is also used as a sagittal dysplasia indicator after its introduction in 2009. The aim of our study was tocorrelate ANB, Beta and Yen angle with soft tissue profile angle in class I and class II patientsTotal of 140 lateral cephalograms of class I and class II patients were selected based on soft tissue profile angle. In each cephalogram, ANB, Beta and YEN angle were measured and compared with soft tissue profile angle. Statistical analysis carried out.
