Demographics, best-corrected visual-acuity (BCVA), IOP, antiglaucoma medications, complications and interventions were recorded. IOP spikes were defined as >30 mmHg or >10 mmHg above baseline within two months postoperatively and classified as hyperacute (0-3 days), acute (4-9 days), or subacute (10-60 days). Surgical success was defined as IOP: 5-21 mmHg with ≥20% reduction from baseline. Primary outcomes included spike characteristics, associated factors, and success rates.</p><p><strong>Results: </strong>Seventy-one eyes (33.8%) developed IOP spikes, with a mean IOP of 33.9±6.5 mmHg and duration of 2.0 ± 2.6 days. Hyperacute spikes had more fibrinoid reaction (p=0.026) and higher total success (p=0.049); acute spikes had higher maximal preoperative IOP and longer hyphema duration (p=0.02, p=0.006, respectively); and subacute spikes had prolonged topical corticosteroid use, longer axial length, and prior vitrectomy (p=0.003, p=0.04, p=0.004, respectively). Total surgical success was 91.9%, with higher rates in the non-spike group (99.3% vs. 77.5%, p<0.001). Fibrinoid reaction, prolonged topical corticosteroid use, and prior vitrectomy were associated with postoperative IOP spikes.</p><p><strong>Conclusion: </strong>Postoperative IOP spikes are significantly correlated with surgical failure following GATT. Their timing might reflect differences in underlying mechanisms. Ocular lesions were categorized as possible, probable, or proven, according to previously established definitions. The primary outcome measure was prevalence of ocular lesions. Secondary outcomes included the classification of ocular lesions and mortality.</p><p><strong>Results: </strong>Among the 198 patients with fungemia, 115 underwent fundus examination. Ocular lesions were observed in 40 patients (34.8%), with 16 classified as probable and 24 as possible. Among those with positive ocular lesions, 5 (12.5%) complained of ocular symptoms, whereas 21 (52.5%) had impaired consciousness. Impaired consciousness was associated with an increased prevalence of ocular lesions (odds ratio [OR], 2.70). Bedside ophthalmic consultations were associated with the classification of ocular lesions (OR, 0.0485). Positive ocular lesions and echinocandin administration were associated with mortality (ORs, 3.180 and 4.140, respectively).</p><p><strong>Conclusion: </strong>Our study results demonstrate that impaired consciousness is an independent risk factor for ocular lesions. Moreover, the presence of ocular lesions was determined to be an independent factor associated with mortality, highlighting the importance of detecting these lesions in systemic management. All patients underwent ELDR with intubation for lacrimal pathway obstruction after EKC.</p><p><strong>Results: </strong>We included 14 boys and 13 girls with unilateral obstructions (20 right and 7 left). The mean age at EKC onset and treatment was 23.4 ± 24.1 and 58.9 ± 42.4 months, respectively, and the mean interval from onset to treatment was 35.9 ± 29.0 months. Nineteen cases had a single blockage (common canalicular obstruction [CCO], sac-duct junction [SDJ], punctal obstruction, and canalicular obstruction in 7, 10, 1, and 1 case, respectively); five had two blockages (CCO and SDJ in 4 cases and SDJ and the lower end of the duct in 1 case), and three had diffuse blockages (2 with diffuse proximal nasolacrimal duct obstruction [NLDO] and 1 with CCO and diffuse proximal NLDO). All patients had tube stents, with a mean intubation period of 64.0 ± 26.3 days. Outcomes were classified as good, fair, and poor in 25 patients, 2 patients, and none, respectively.</p><p><strong>Conclusion: </strong>CCO and SDJ were the primary lacrimal pathway obstruction sites following EKC in children. Gender differences in all grades were evaluated in regard to subjective and objective refractive error (RE), axial length (AL), corneal keratometry, higher-order aberrations (HOAs), and a questionnaire regarding environmental factors of myopia progression.</p><p><strong>Results: </strong>In the girls in all grades, corneal keratometry was steeper and AL was shorter (p<0.05), coma-like and total aberration (in 6mm) corneal HOAs, coma-like, spherical-like, and total aberration (in 6mm) ocular HOAs were larger (p<0.05), in addition, only spherical aberration (in 4mm) corneal HOAs were smaller. In all grades, no gender-related differences were found in myopia prevalence (Grade 1: boys = 6.1%; girls = 6.5%, Grade 9: boys = 60.4%; girls = 65.4%) as well as RE. The questionnaire findings revealed that in all grades the girls spent more time reading and less time playing mobile-phone-app games (p<0.001).</p><p><strong>Conclusions: </strong>In Japanese school children, AL was shorter in the girls than in the boys, although, no gender-related differences were observed in myopia prevalence. The steeper cornea in girls might be associated with that discrepancy, and partially with gender differences of HOAs. This study aimed to examine the disease classification of binocular diplopia in patients aged over 40 years old and examine the treatment methods and prognosis of SES.</p><p><strong>Study design: </strong>Retrospective observational case series.</p><p><strong>Methods: </strong>We reviewed the medical records of all new patients aged 40 years or older with binocular diplopia who visited the International University of Health and Welfare, Atami Hospital, between January 2020 and December 2022. Clinical causes of diplopia were analyzed based on age and sex; binocular alignment, treatment types, and post-treatment outcomes were documented for patients with SES, age-related distance esotropia, and cyclovertical strabismus.</p><p><strong>Results: </strong>Among the 199 patients with binocular diplopia, SES was identified as the most common cause, affecting 95 (47.7%) individuals. Women comprised 52.6% of SES cases and 49% of non-SES cases (p=0.67). The mean age of onset was 71.2 ± 10.8 years for binocular diplopia, 76.2 ± 6.5 years for SES, and 66.6 ± 12.0 years for other etiologies (p<0.001). SES treatments included surgery (48.4%, n=46), prism glasses (35.9%, n=34), and observation (15.8%, n=15). Both techniques have a history of over 100 years and are the gold standard approaches for the treatment of nasolacrimal duct obstructions. Alternatively, researchers have also attempted lacrimal passage reconstruction using various stents to restore the patency of the lacrimal passage, with nunchaku-type silicone tubes showing good surgical results according to several published reports. However, in cases in which the procedure required blind manipulation, the results were largely dependent on the surgeon's skill. Under such circumstances, the dacryoendoscope was introduced at the beginning of this century and is currently widely used. In other fields, especially in gastroenterology, the introduction of gastrointestinal endoscopes that enable observation and treatment of lesions under direct observation has dramatically improved the treatment of gastrointestinal disorders. Preoperative, postoperative 1st week, 1st, 3rd and 6th months ACF were measured. At the same visits, mCVI and pCVI were calculated. The relationship between phacoparameters and postoperative ACF, mCVI and pCVI values were evaluated by correlation analysis.</p><p><strong>Results: </strong>Fifty eyes of 50 patients were included in the study. Postoperative 1st week ACF was significantly higher than preoperative ACF (p<0.001). ACF decreased significantly from postoperative 1st week to 6th months. Postoperative 1st month and 3rd month mCVI were significantly higher than preoperative mCVI (p<0.001, p=0.04, respectively). It was observed that pCVI reached its peak value in the 1st postoperative week and decreased to base value in the postoperative 6th month. A strong positive linear correlation was found between total cumulative dissipated energy (CDE) and the difference between postoperative 1st week and preoperative ACF (p<0.001, r=0.68).</p><p><strong>Conclusion: </strong>Particularly in the early period after phacoemulsification, ACF, mCVI and pCVI increased. The increase in ACF lost its significance at the third month and the increase in mCVI and pCVI remained significant, which may indicate that the inflammatory effect of cataract surgery lasts longer in the posterior segment. {"title":"Efficacy and limitations of amniotic membrane transplantation in cases of severe ocular surface disorders: retrospective analysis over a 21-year period.","authors":"Kohei Harada, Hideki Fukuoka, Koji Kitazawa, Yulia Aziza, Tsutomu Inatomi, Tomoyuki Hino, Go Horiguchi, Satoshi Teramukai, Shigeru Kinoshita, Masafumi Uematsu, Takashi Kitaoka, Chie Sotozono","doi":"10.1007/s10384-025-01209-z","DOIUrl":"10.1007/s10384-025-01209-z","url":null,"abstract":"<p><strong>Purpose: </strong>To examine the reoperation rate of amniotic membrane transplantation (AMT) and clarify the risk factors for AMT reoperation in severe ocular surface (OS) disorders (OSD).</p><p><strong>Study design: </strong>Retrospective cohort study.</p><p><strong>Participants: </strong>We reviewed the medical records of all AMT cases between April 1998 and June 2019 at the Department of Ophthalmology, Kyoto Prefectural University of Medicine, Kyoto, Japan.</p><p><strong>Methods: </strong>Primary diseases and AMT reoperation rates were examined. In severe OSD cases (Stevens-Johnson syndrome, ocular cicatricial pemphigoid, and chemical/thermal burns), preoperative OS Grading Score (OSGS), surgical procedures combined with AMT, and risk factors for reoperation were investigated and assessed.</p><p><strong>Results: </strong>Over a period of 21 years and 3 months, 750 AMTs were performed on 664 eyes of 594 cases. AMT was repeated on 51 of those 664 eyes (7.7%), and most frequently performed on 25 out of 196 eyes (12.8%) afflicted with severe OSDs. In severe OSDs, OSGS was significantly higher in the reoperation group compared to no-reoperation group (P<0.05), suggesting corneal epithelial defects, conjunctival hyperemia, trichiasis, mucocutaneous junction involvement, and corneal opacity as being risk factors for re-AMT (univariate analysis). In logistic regression analysis, only conjunctival hyperemia was a risk factor, with odds ratios (OR) of 2.65 (95%CI: 1.34-5.22, P=0.005). AMT combined with cultivated or donor corneal epithelial transplantation reduced reoperation risk with an OR of 0.92 and 0.63, respectively.</p><p><strong>Conclusions: </strong>In severe OSD cases, the effect of AMT is limited. {"title":"Evaluating efficacy of 0.125% atropine using a myopia progression machine learning model.","authors":"Chang Yoon Han, Sa Ra Kim, Dae Hee Kim","doi":"10.1007/s10384-025-01212-4","DOIUrl":"10.1007/s10384-025-01212-4","url":null,"abstract":"<p><strong>Purpose: </strong>To investigate the usefulness of a machine learning (ML) model that can predict the natural course of childhood myopia in evaluation of the inhibitory effects of 0.125% atropine on the progression of childhood myopia.</p><p><strong>Study design: </strong>Retrospective, cohort study.</p><p><strong>Methods: </strong>A retrospective analysis was conducted on children treated daily with 0.125% atropine eye drops. The children were classified into 6-, 12-, 18-, 24-, and 30-month group based on the treatment duration. Spherical equivalents (SE) at the last treatment time point were compared with the pretreatment and ML-predicted SE. The myopia suppression rate due to treatment was calculated based on the first- and ML-predicted SE.</p><p><strong>Results: </strong>A total of 771 eyes (402 boys and 369 girls) from 397 children were included. The participants' mean age was 8.0 ± 1.5 years. The first SE of -2.87 ± 1.67 diopters (D), treatment led to a mean SE of -3.44 ± 1.90 D, showing a significant reduction in myopia progression compared to the ML model's prediction of -4.12 ± 1.75 D. Except for the 6-month group, the last SE was statistically significantly less myopic than the predicted SE, indicating that treatment suppressed progression compared to the natural course. The mean myopia suppression rate was 53.5%, with definite differences between the groups.</p><p><strong>Conclusion: </strong>Treatment with 0.125% atropine may suppress myopia progression in children compared with the ML child myopia prediction model.
