Various medical and surgical techniques have been developed to address this condition and restore a more youthful and rejuvenated appearance. In terms of medical therapies, non-invasive options such as topical creams containing retinoids, peptides, and hyaluronic acid have shown promise in reducing mild malar bags. These substances help improve skin texture, promote collagen production, and enhance skin elasticity, thereby reducing the appearance of swelling and puffiness. Additionally, injectable treatments such as dermal fillers have been utilized to camouflage malar bags and create a smoother contour. By strategically placing fillers in the surrounding areas, a skilled practitioner can achieve a more harmonious facial appearance. In recent years, advancements in radiofrequency have also shown promise for treating malar bags. Laser skin resurfacing procedures can help tighten the skin, stimulate collagen production, and reduce the appearance of swelling and wrinkles in the malar area. For patients seeking more lasting results, surgical techniques provide viable options. Blepharoplasty, a surgical procedure aimed at correcting eyelid issues, can be performed to remove excess skin and fat deposits in the lower eyelid, effectively addressing malar bags. Additionally, newer techniques like endoscopic surgery allow for minimal incisions and reduced scarring, making them appealing to patients concerned about visible postoperative marks. Although numerous medical and surgical techniques are available, the choice of treatment depends on factors such as the severity of the condition, patient preferences, and the expertise of the medical practitioner. To enhance prevention and quantify the true burden of melanoma, better understanding of thickness patterns and associated characteristics is crucial. Previous studies have been limited to report trends and address risk factors of thickness in specific melanoma subtypes in the Greek population. We investigated associations between epidemiological characteristics and thickness for the two most common melanoma subtypes and the trends in thickness over a twelve-year period.</p><p><strong>Methods: </strong>A retrospective study of 1201 patients with histologically confirmed primary nodular and superficial spreading melanoma (SSM) diagnosed from 2010 to 2021 in \"Andreas Sygros\" Hospital of Cutaneous and Venereal diseases was conducted. Multiple regression was performed to examine the association of variables of interest with melanoma thickness.</p><p><strong>Results: </strong>SSM thickness significantly increased by 2% per year (percent of change: 2.0, 95% CI: 0.2, 3.7) from 2010 to 2021, while a similar tendency for nodular melanoma (NM) thickness was indicated. Age at diagnosis was demonstrated to be a predictor of thickness for both subtypes. When considering all confounders, overall sun exposure was inversely associated with SSM thickness (PC: -6.2, 95% CI: -12.4, 0.5) and a similar association was indicated for NM (PC: -9.3, 95%CI: -21.1, 4.2).</p><p><strong>Conclusions: </strong>These results indicate an upward trend of SSM thickness and the associations of age at diagnosis and overall exposure to UV with thickness of both subtypes. Photodynamic therapy (PDT) emerges as a promising supplement or an alternative to address complicated cases. In this review, we comprehensively review PDT's safety and efficacy in treating cutaneous infections.</p><p><strong>Evidence acquisition: </strong>A PubMed systematic review with search terms for PDT treatment, hair, skin, and nail infections.</p><p><strong>Evidence synthesis: </strong>There were a total of 43 studies on the use of PDT in cutaneous infections which discussed the treatment of viral (N.=20), bacterial (N.=11), fungal (N.=9), and protozoal (N.=3) infections. There is evidence for using PDT, mostly 5-aminolevulinic acid (5-ALA) and methyl aminolevulinate (MAL), in the treatment of cutaneous infections. Most evidence for viral infections involved treatment with 5-ALA PDT in condyloma acuminatum (CoAc), verruca vulgaris, and molluscum contagiosum. In bacterial infections, 5-ALA and MB PDT have been used to achieve complete remission in refractory Pseudomonas and atypical mycobacteria infections without recurrence. In onychomycosis, MAL PDT achieved a 40.9% cure rate and MB PDT showed a 77.8-100% cure rate with no reports of recurrence. Parasitic infections, such as leishmaniasis have also been successfully treated with both 5-ALA and MAL PDT.</p><p><strong>Conclusions: </strong>PDT is a promising treatment option for cutaneous infections, with growing evidence for its utility in treating cutaneous bacterial, viral, fungal, and parasitic infections, particularly those that fail standard treatments. Despite its widespread use, the mechanism of action of IVIG in treating autoimmune diseases remains a topic of debate. IVIG is derived from the plasma fractionation of a large pool of donors, primarily consisting of the IgG isotype. Its main mechanisms of action involve neutralizing circulating autoantibodies via the F(ab')2 portion, inhibiting complement-mediated tissue destruction, and reducing the half-life of circulating autoantibodies through the Fc portion. This paper explores the growing utilization of IVIG as an off-label therapy in dermatological autoimmune or immune-mediated diseases, including autoimmune bullous disease (AIBS), dermatomyositis (DM), lupus erythematosus (LE), systemic sclerosis, scleromyxedema, Stevens-Johnson Syndrome (SJS), toxic epidermal necrolysis (TEN), pyoderma gangrenosum (PG), and necrobiotic xanthogranuloma (NXG). In this context, the sole large prospective, randomized trial was the 2022 ProDERM study, which demonstrate efficacy of IVIG in improving cutaneous manifestations among 95 DM patients compared to the placebo group. Moreover, although considered off-label, the use of IVIG is regarded as the first-line therapy for patients with scleromyxedema. As a first line of therapy, IVIg is only approved for Kawasaki Disease (KD) in the setting of vasculitis. Keloids can be difficult to treat because of their tendency to recur after treatments, including after excision. There is a myriad of treatment modalities used, which can be used as monotherapy or in combination with surgical excision or other treatment methods.
