The present study determined neurological manifestations in vitamin B12 deficient children in a tertiary care centre in Rajasthan. \u0000Methods: This hospital based observational analytical study was conducted in Department of Pediatrics, Dr. S. N. Medical College, Jodhpur after approval of Institutional Ethics Committee (IEC). The study population included Vitamin B12 deficient (<250 picogram/dl) children during September 2019 to August 2020. All children aged 6 months to 5 years admitted in pediatric ward with anaemia (Hb< 11 gm/dl according to WHO Criteria) were screened for serum vitamin B12 levels out of which children having serum vitamin B12 level <250Pg/dl were included in the study. Results: More than half (63.8%) of the children aged between 6 months and 15 months. Male to female ratio was 1.58:1. 78.7% were rural and 60% were from lower socioeconomic class. 85% of the children’ family had vegetarian dietary habits. The most common clinical feature was hyperpigmentation of knuckles and skin (85% each) followed by depigmented and sparse hair (70%), and chubby face (42.5%). 65% of the children were found to have neurological manifestations. The most common neurological manifestation was developmental delay (60%) followed by hypotonia (38.7%), exaggerated reflexes (31.2%), and abnormal movements (20%). Conclusion: There is an urgent demand for early evaluation of the children so prevent further prevent developmental growth of the children. \u0000Keywords: Neurological manifestations, vitamin B12, developmental delay

This widely circulated perception has become a reason for many expectant mothers to either completely decline to consider c-section or suffer in prolonged labor that could be fatal to either the mother or the baby or both. \u0000Methods: This study follows several aspects of the triangulation method of research. It adheres to the case study approach by mixing qualitative and quantitative methods. It uses purposive sampling by selecting 19 women who delivered by c-section at Bay Regional Hospital, Baidoa, Southwest State of Somalia. \u0000Results: About 58% had believed c-section as ‘very risky’ or ‘risky’ prior to undergoing the procedure; after delivery 89.5% thought it was either ‘very safe’ or ‘safe’, while none of the respondents considered it to be ‘unsafe’ or ‘very unsafe’ anymore. \u0000Conclusion: Women’s perception of c-section changed dramatically after undergoing the procedure as compared to their belief before the surgery. It is also remarkable that they would not hesitate in their next delivery by c-section and that they would recommend it to other women in order to avoid fatal complications. \u0000Keywords: childbirth, health research, labor complications, medical health

Due to conflicting reports regarding the clinicohistological spectrum of malabsorption, this study was conducted to study the spectrum of malabsorption. \u0000Objective: To study the morphologic spectrum of duodenal biopsies and stool microbiological profile in patients with chronic diarrhoea and malabsorption. \u0000Materials and Methods: The study included 106 patients aged above 18 years, presenting with chronic diarrhoea or malabsorption. Duodenal mucosal biopsy obtained was evaluated for villous atrophy, crypt architecture and Intraepithelial lymphocyte count (IEL) by H&E and immunohistochemistry for CD3. Stool samples collected were subjected to wet mount and culture examination. Serological testing for antibodies to coeliac sprue were also performed. \u0000Results: This descriptive study included 57 males and 49 females. Duodenal biopsy spectrum consisted of chronic duodenitis (48), duodenitis associated with autoimmune conditions (20), histomorphology of coeliac sprue (5), duodenitis with villous atrophy (14), only increased IEL (11) and miscellaneous (8). CD3 stained sections (n=90) offer a slightly better advantage over H & E-stained sections in detecting IEL. Stool microscopy and culture were positive in 6 cases. Coeliac sprue antibodies were positive in 1 case with Marsh type 1 lesion. \u0000Conclusion: Duodenal biopsy is a useful parameter in the study of malabsorption which must be adjuncted by microbiological and serological tests. Most common etiology of TEN is drug induced. Though Anaphylactic [Type I Hypersensitivity] reactions have been reported with Iron Sucrose, we present first case of TEN [Type IV Hypersensitivity], associated with Iron Sucrose along with Anti-tuberculosis treatment [ATT], to our knowledge. Iron Sucrose is considered relatively safer than other iron formulations. After 48 hours of administering Intravenous Iron Sucrose [20 mg in 200 ml N.S] infusion [first day], patient developed TEN symptoms [third day]. He was taking first line ATT drugs since past one month for Pulmonary Tuberculosis. On basis of history, 80-90% Body Surface area involvement, positive Nikolsky sign and laboratory findings, patient was diagnosed with drug induced TEN [fifth day]. For TEN management, ATT was withdrawn and patient was treated with intravenous Dexamethasone, Piperacillin/Tazobactam, Metronidazole, Pantoprazole, Intravenous fluids with dermatologically supportive local applications and patient was monitored. Patient died due to septic shock despite all resuscitative measures [seventh day]. Causality assessment (WHO UMC scale and Naranjo scale) was ‘Possible’ for both Iron Sucrose and ATT drugs. No improvement in patient’s condition after withholding ATT and short duration between Iron sucrose and symptoms of TEN onset suggests an increased likelihood of TEN induction by Iron sucrose. However, either ATT or Iron Sucrose could trigger TEN in presence of other drug. Improved understanding by physicians of these relatively unknown life-threatening ADR’s and interactions can aid in early identification and enhanced vigilance in future scenarios. \u0000Keywords: WHO UMC Scale, SCORTEN score, Naranjo scale, Anaphylactic reactions
