Gite, Dr. Jitendra Patel, Dr. Punita Pant","doi":"10.32553/ijmsdr.v6i5.936","DOIUrl":"https://doi.org/10.32553/ijmsdr.v6i5.936","url":null,"abstract":"Aims and Objective: To study response after giving neoadjuvant chemoradiation in patients with locally advanced rectal cancer with Cap capecitabine 825 mg/m2 daily PO Bid with radiation in dose of 45Gy/20#/4 weeks and to study to the clinical profile of different patients with locally advanced rectal cancer and to assess the toxicity of patients treated with neoadjuvant chemoradiation \u0000Materials and Methods: A prospective observational study was conducted in 60 patients from October 2018 to April 2020 with locally advanced Rectal carcinoma which were proven histopathologically. Neoadjuvant chemoradiation was planned with Capecitabine 825mg/m2 PO Bid with radiation to a dose of 45 Gy/20#/4 weeks. Radiological response assessed after 6 weeks of completion of treatment and then surgery was planned according to response. \u0000Results: Thirty-seven patients received definitive surgery. Pathological complete response was observed in 1 patient, near complete response in 7, partial response in 27 and poor or no response was seen in 2 patients. Among 37 patients who have undergone surgery ,30% of patients received sphincter preserving surgery. Radiation induced acute skin and acute lower gastrointestinal were seen. Capecitabine induced diarrhea, hematological toxicities and few patients of hand foot syndrome were seen. \u0000Conclusion: In locally advanced rectal cancer, preoperative radio chemotherapy with Capecitabine improves local control and reduces the risks of acute and late toxicity as compared to postoperative radio chemotherapy . It is non-invasive and non-ionizing and the images are highly sensitive and specific with excellent soft tissue contrast. It gives excellent images of anatomic structures differing in proton density and other tissue characteristics. It can be used to assess intracranial and extracranial lesions particularly those involving the soft tissues. \u0000MRI has the shortcoming of being prone to magnetic susceptibility difference artefacts, caused by the presence of metallic materials such as dental restorative materials, implants and orthodontic appliances. All substances when placed in a magnetic field are magnetized to a degree which varies according to their magnetic susceptibility. Non precious dental alloys have the potential of causing image deformation or image voids. \u0000Dentists are not the only professionals implanting metal devices. Heart pacemakers and defibrillators, aneurysm clips, cochlear implants, insulin pumps, vascular stents and artificial joints are widely used in various branches of medicine. In some cases, it is not possible to carry out MRI due to extensive dental hardware with a high ferromagnetic content. Severe image distortion or an inability to perform the MRI scan due to dental restorations is a rare problem, but cannot be completely eliminated. It is characterised by periodic fever with chills due to rupture of erythrocytes to release the progeny parasites. It is marked by release of variety of cytokines like IL-6, IL-12, IFN-Ɣ, TNF—α etc. Uric acid is one of the emerging inflammatory markers in malaria that demands attention. One of the proposed mechanisms is that there is accumulation of uric acid and its precursor, hypoxanthine in the infected erythrocytes. These are released into the blood on rupture of the schizont.  \u0000Aim: To find association of serum uric acid level with severity of malaria.Settings and Design: This was a hospital based longitudinal study conducted at a tertiary care centre in Kolkata, India. \u0000Materials and Methods: We measured the plasma levels of uric acid and various inflammatory markers {Ferritin, C-reactive protein(CRP), LDH, C3, C4} in eighty eight  patients admitted with microscopically proven malaria (severe or non-severe type). The levels of uric acid were compared with the disease severity and the inflammatory markers stated above. \u0000Statistical Analysis used: The data was analyzed using MedCalc software and Microsoft Excel 2010 and further graphically plotted. \u0000Results: The serum uric acid levels were raised in 18.51% of patients with severe malaria compared to only 4.91% with non-severe variety (p= 0.04). The uric acid levels demonstrated a positive correlation with CRP (r=0.3334, p=0.0015); procalcitonin & ferritn (r=0.3701, p<0.0005). However, it was negatively correlated with C3 (r= -0.3780, p= 0.0003) and C4 (r=0.3180, p<0.005). A univariate regression analysis supported our results to establish the correlation. However, multiple logistic regression analysis demonstrated significant association between serum uric acid levels on day 1 and C3 decrement as a marker of disease severity. \u0000 Conclusion:  Thus, it can be concluded that there is a definite association between the severity of malaria and plasma uric acid levels. There is a crucial need for new and dependable methods for recognition and confirmation of victims in mass disasters. Forensic Odontology employs various methods like rugoscopy, dental labelling, DNA analysis from dental pulp, bite marks analysis, etc. Currently it embraces the study of enamel rods end patterns. Ameloglyphics is an analysis of the end patterns of the enamel rod. These patterns have been found to be unique to the individual teeth of the same individual, as well as to different individuals. A potential association between the type of enamel rod end pattern and the incidence of dental caries can help to predict the individual's susceptibility to caries and to implement preventive measures. Individual identification is becoming more important than ever in today's world. To achieve regeneration and optimal healing of the bone different bone substitutes can be used. A 25 year old male patient reported with the chief complaint of mobile teeth in maxillary anterior tooth region  with 21 labially placed. He had a history of trauma 5 years back. The clinical and radiographic diagnosis of infected periapical cyst with 21,22; Endodontic treatment was performed with 21, 22 followed by periapical curettage. Platelet rich fibrin (PRF) was mixed with Metronidazole gel and used as a regenerative biomaterial in the periapical defect. 6 months follow up shows satisfactory healing and regeneration of periapical region.  There is considerable clinical interest in using PRF in combination with Metronidazole Gel.
