Cow's milk protein allergy is a topical issue in pediatrics. This disease affects 2-3% of young children. Methods. The study included 30 children with a confirmed cow’s milk protein allergy and a prescribed milk elimination diet. The children were observed for 36 months. The molecular profile was evaluated by detecting the major components of milk (Bos d 8, Bos d 5, Bos d 4), minor (Bos d 6), and cross-reactive with serum albumin (Fel d 2, Can f 3). Results. The study group included 46.7% of boys. The mean age of the subjects was 14.5±3.18 months. Nine of the 25 analyzed factors were identified in the course of our work by the method of logistic regression with progressive selection, which in combined action affect the formation of tolerance to cow's milk proteins in allergic children. There are three clinical examples of calculating the prediction of the complete tolerance development in the elimination diet in children with cow's milk protein allergy. Conclusions. Independent predictors that reduce the likelihood of the tolerance development in the elimination diet, have identified: skin test to milk, Bos d 8, Bos d 5, Bos d 4, Bos d 6. Independent predictors of the tolerance development in the elimination diet are: milk-specific IgE, total IgE, Fel d 2, Can f 3. Low probability of tolerance development by elimination therapy is a recommendation for prescribing treatment to a patient by specific oral tolerance induction.","PeriodicalId":140699,"journal":{"name":"Proceedings of the Shevchenko Scientific Society. Currently, diagnostic tests use purified nucleic acids from clinical samples. This purification step adds time, cost, and affects the quality of testing. Multiple attempts to remove the purification step were reported. Results: We report a protocol for the direct detection of genetic markers in saliva. The protocol is based on collection of saliva in a solution containing a detergent and ethanol, and is compatible with isothermal amplification (LAMP), real-time RT-PCR and RT-PCR. SARS-CoV-2 and GAPDH markers were used as reference markers. We observed that mild detergents allow efficient detection of markers (e.g. GAPDH and SARS-CoV-2), while strong detergent, e.g. sodium dodecyl sulfate, inhibited the PCR reaction. Under these conditions, saliva samples can be stored for 24 h at +40C or -180C with preservation of the markers. Storage at room temperature led to deterioration of marker detection. Snap heating of saliva samples at the time of collection, followed by a storage at the room temperature, provided partial protection.Conclusions: The protocol presented in this report describes collection and storage of saliva for direct detection of genetic markers and is compatible with PCR and LAMP tests.","PeriodicalId":140699,"journal":{"name":"Proceedings of the Shevchenko Scientific Society. Current approaches to reducing the ulcerogenicity of nonsteroidal anti-inflammatory drugs are aimed primarily at eliminating the damaging effect on the gastric mucosa, and according to the literature, the use of proton pump inhibitors can even worsen the course of enteropathy. Objectives. To characterize the influence of cryopreserved placenta extract on the state of secretory and motor-evacuation activity of the stomach under the conditions of experimental meloxicam-induced gastropathy and enteropathy. Methods. Serial randomized placebo-controlled studies were performed on nonlinear rats and mice using a model of small intestine-induced gastrointestinal lesions. Gastric secretion in rats, gastric evacuation function and intestinal motor function in mice were studied with the help of standard methods. Results. The administration of meloxicam leads to a statistically significant increase (p <0.05) in the volume of gastric secretion by 31.6±7.3%, an increase in total acidity (p=0.002) by 7.7 [7.7; 16.6]% and a statistically significant increase (p<0.05) in peristaltic activity by 18.9% relative to intact animals. The administration of cryopreserved placenta extract led to a statistically significant (p=0.002) decrease in the ratio of free and total acidity by 2.3 times, with a statistically significant decrease (p<0.05) by 14.5% in peristaltic activity relative to intact animals. The combined threaded administration of meloxicam and cryopreserved placenta extract was accompanied by a decrease in the ratio of free and total acidity by 43.0±3.0%. Conclusion. The combined application of placental cryoextract and meloxicam eliminates meloxicam-induced hypersecretion of gastric juice and gastric hypermobility. Infectious diseases have been a threat during all periods of human existence. Primary measures to protect against extremely dangerous pathogens included quarantine, observation and vaccination. Vaccination was crucial in the fight against smallpox - the only disease, which was eradicated on a global scale. The aim of the work was to identify the main stages of development and application of vaccines for the protection against extremely dangerous infections (EDI) and the contribution of Ukrainian scientists in the development of vaccines for EDI. Methods. An analysis using the Search Strategy of narrative reviews of literary sources and Internet resource was conducted to systematize data about the application of immunobiological preparations to create an active immunity against several actual EDI. The participation of scientists with Ukrainian roots in the creation of vaccines against this group of diseases is indicated. Results. Smallpox (variolation in China in the 11th century) is known as the first disease against which specific protection was created. Until the 20s century, the first five vaccines against EDI were developed: smallpox, rabies, anthrax, cholera, and plague. In the 20s century, the list was supplemented by vaccines against typhus, yellow fever, tick-borne encephalitis, tularemia, brucellosis, coxiellosis (Q-fever), hemorrhagic fever with renal syndrome (HFRS). The introduction of new technologies in the 21st century allowed improving existing preparations and creating new ones against Ebola viral disease (EVD), COVID-19 as EDIs relevant ones for the pandemic potential. Outstanding scientists with Ukrainian roots contributed to the creation of vaccines against EDI: V. Khavkin, D. Samoilovich, V. Zhdanov. Conclusion. Protection of the population against EDIs is limited by the number of available effective vaccines. This virus can cause severe acute respiratory syndrome (SARS). In this study, we reviewed proteases of SARS-CoV-2 based on related articles published in journals indexed by Scopus, PubMed, and Google Scholar from December 2019 to April 2020. Based on this study, we can claim that this coronavirus has about 76% genotype similarity to SARS coronavirus (SARS-CoV). Also, similarities between these two viruses have been found in the mechanism of entry into host cells and pathogenicity. ACE 2, the angiotensin convertase enzyme 2, plays a role in the Renin-Angiotensin-Aldosterone system (RAAS) and blood pressure regulation. Some mechanisms have been reported for the role of ACE 2 in the pathogenicity of SARS-CoV-2. For example, the interaction between the ACE 2 receptor and spike protein mediated by TMPRSS2, Cathepsin B/L, and other enzymes is responsible for the entry of the virus into human cells and pathogenicity. Some host cell endosomal enzymes are necessary to cleavage coronavirus spike protein and cause binding to their common receptor. It was dedicated to the discovery of X-rays made on November 8, 1895 by the German physicist Wilhelm Conrad Röntgen. Having taken into account the contribution of other scientists, the precondition of the given epochal, yet unexpected, discovery was, first and foremost, the work of the Ukrainian scientist Ivan Puluj. It was Puluj who laid the foundation for X-ray science. He explained the nature of X-rays, discovered that they can ionize atoms and molecules, and defined the place of X-ray emergence and their distribution in space. In 1881, Puluj constructed a cathode lamp (“Puluj’s tube”) which was fundamentally a new type of light source. In the same year, in recognition of this discovery, Puluj received an award at the International Exhibition in Paris. Investigating the processes in cathode-ray tubes, Ivan Puluj set the stage for two ground-breaking discoveries in physics, namely X-rays and electrons. Puluj used his cathode lamp in medicine as a source of intense X-rays which proved to be highly efficient. The exact date of the first X-ray images received by Puluj remains unknown. High-quality photographs of the hand of an eleven-year-old girl, taken on January 18, 1896, are preserved. Multiple X-ray images clearly visualized pathological changes in the examined structures (fractures, calluses, tuberculous bone lesions). High-quality images were obtained by means of the anticathode in the design of Puluj’s lamp, which was the first in the world. This crisis has crystalized the prioritization of expenditures of resources for medical research, clinical practice and public health measures in combating this deadly virus. The Johns Hopkins School of Public Health Coronavirus Resource Center has counted a total of 46,168,459 cases and 1,196,891 deaths worldwide (November 1, 2020). The data for Ukraine is 407,573 cases and 7,515 deaths. It is now 10 months since the recognition of the worldwide involvement of the SARS-COV-2 virus as the etiologic agent of this pandemic. Although progress has been made, there is still a large gap in our efforts to find a cure and create an effective vaccine for the world population. A corollary lesson is the need for life-long learning and the acceptance of change in everyday practice. Harvard and Ukrainian Catholic University Professor of business management Adrian Slywotzky develops a succinct idea in his book «David Conquers: The Discipline of Asymmetric Victory». He states that David’s sling is a modest investment that results in a giant return. Such is our modest investment in this scholarly medical journal: Proceedings of the Shevchenko Scientific Society. Medical Sciences. We rely heavily on the social media mechanism of «word-of mouth» to promote our journal and its offerings of current medical breakthroughs and findings. The need to socially distance and limited human movement in open spaces since the COVID-19 outbreak have brought most forms of research to a standstill. While most research incentives have been directed towards research regarding the pandemic, diminished patient visits, have paralysed all the studies requiring personal and physical examination. Majority of the social interactions have been reduced to a screen, and this is also the new practice in the research realm. This unique situation calls for a need to re-examine research practices and reinvent novel methods for quality research. While it is prudent to step up research to understand the disease and its impact on varied conditions, the situation also necessitates a close watch for misinformation, erroneous reporting, and failure of ethical research. Moreover, there is a felt need to derive validated tools for remote assessment to minimise risk to the patients and physicians alike. In this brief, we discuss the perceived changes and potential areas for erroneous research reporting while providing possible solutions for fruitful research in the peri-pandemic period. We also identify new methods of conducting studies and the setbacks that could be faced while carrying out such tasks, including those of methodological, ethical and financial nature.
