Many studies have been done to find the hypomagnesemia prevalence and its effects on patients regarding mortality and morbidity. So we have undertaken this study to know the effects of Hypomagnesemia in critically ill patients admitted in medical critical ward. And it is an observational study. Aims and Objectives: To study the level of serum magnesium in critically ill patients and to correlate its effects with patient outcome in terms of length of stay in ICU, need for ventilator support, duration of ventilator support, APACHE II score. To detect any electrolyte abnormalities associated with hypomagnesemia. Results: In our study, on admission in ICU, 55.3% patients had hypomagnesemia, and patients with hypomagnesemia have mean duration of stay in ICU was 8.2 days, longer duration on mechanical ventilator i.e. 6.3 day and APACHE II Score of 15.7 and more frequently patients were in sepsis (25.9%), 15.3 % had cardiovascular abnormality. Patients with hypomagnesemia were more frequently associated with Diabetes Mellitus (34%) and they were having higher mortality rate (48.9%). Conclusion: In the critically ill patients Hypomagnesemia was prevalent in higher rate. And it was associated with a higher mortality rate in them. And the requirement of ventilator support and duration on ventilator was significantly higher in hypomagnesemic patients. Hypomagnesemia was more commonly associated with sepsis, diabetes mellitus. Earlier plasma cholinesterase level was used to assess the severity of poisoning. Presently serum amylase is being recommended as a better indicator of severity. Aims and Objectives: The present study was undertaken to estimate serum amylase levels in acute organophosphate poisoning andto correlate serum amylase levels with outcome of patient . Methods : This cross sectional study was conducted in Shri Vasantrao Naik Government Medical College and Hospital, Yavatmal. A total of 120 patients with organophosphorus poisoning admitted to the hospital during the study period of 1st January 2017 to 30 th November 2017 were included in the study. Biochemical evaluation which includes serum amylase, blood glucose, urea, creatinine, and liver function tests were analyzed. Results: In OP poisoning patients , the amylase levels were significantly elevated at the time of admission (178.21 U/L) and have shown a gradual remission with proper treatment. The mean amylase level in severely poisoned patients was 294.8 U/L. The bad prognostic factors, very well correlated with serum amylase levels were Pinpoint pupil- 297 U/L, Fasciculations-309 U/L, Severe secretions-321 U/L, CNS depession-334 U/L, Respiratory failure-359 U/L and Convulsions-398 U/L. The overall mean value of serum amylase was significantly higher in non-survivors Vs survivors (482.46 U/L Vs 148.34 U/L, p<0.0001). Methanolic and aqueous extract of aerial part of plants was prepared and tested for the presence of different phytoconstitutents. The in-vitro antioxidant potential of extract were evaluate by different in-vitro methods. TLC of extracts were performed by using various solvent systems. The present study revealed that the MELC and AELC contained flavonoids, glycosides, phenols, saponins, steroids and terpenoids However, alkaloids were detected only in MELC. Tannins were absent in methanolic as well as in aqueous extracts. The in-vitro antioxidant potential MELC and AELC was evaluated by DPPH and FRAP. In both methods MELC possesses good antioxidant activity as compared to AELC. Dried and crushed leaves of Baccaurea ramiflora and Microcos paniculata were defatted with petroleum ether and then extracted with alcohol. The alcoholic extract at the doses of 100 mg/kg, and 200 mg/kg body weight was subjected to evaluation of ulcer protective activities in experimental animal models. Tuberculosis of the central nervous system is not uncommon. Meningitis and tuberculoma are the two most important manifestations of tuberculosis of the CNS. Intracranial tuberculomas can present as solitary lesion or multiple lesions. Many atypical radiological findings of tuberculomas are known. They may be indistinguishable from cranial abscess or primary brain tumour. In India, tuberculoma should be considered as a differential in patients with intracranial mass lesions. We present a case of intracranial tuberculoma mimicking a high grade gliomas on magnetic resonance imaging and clinical presentation. A 58-year-old male presented with one-month history of epilepsy. Cranial magnetic resonance imaging showed a in right gangliocapsular region peripheral ring-enhancing lesions with central necrosis and one of the lesion showing thick wall enhancement and highly raised choline/creatinine rat. There was a strong suspicion of glial tumour. On CSF studies of the patient, 100% lymphocytes were noted and no malignant cells were seen. There has been a rapid increase in the prevalence of diabetes worldwide, developing countries being the worst hit. Recent studies have revealed a trend of occurrence of diabetes mellitus at a younger age.This implies morbidity and mortality due to diabetes will occur at an early age among large proportion of individuals. We aimed to determine the prevalence of diabetes mellitus among young medical students of MGIMS Sevagram and risk factors associated with diabetes.Methods: This cross-sectional study was conducted between August 2010 and December 2012 in MGIMS Sevagram, a medical college located in central India. Total of 700 students, aged 17-35 years, were studied using a modified STEPs questionnaire.After an overnight fast, blood samples for fasting blood glucose and lipid profile were collected. Statistical analysis was done using STATA 13 software.Results: The prevalence of diabetes mellitus was found to be 2.36%. Impaired fasting glucose or prediabetes was present in 17.57%. Control of diabetes was very poor among study subjects as only 33.3% of diabetics had their fasting blood sugar under control. .After multivariate logistic regression analysis diabetesrisk was significantly higher among those aged >30 years {OR-11.32 (95% CI 3.43-37.32)}, those with low physical activity level {6.99 (95% CI 1.52-32.08))}, those with abnormal waist circumference {OR-3.70 (95% CI 1.07-12.81)} and those who had hypertriglyceridemia {OR-3.12 (95% CI 1.02-9.51)}.Conclusion: A significant proportion of young medical students havediabetes and prediabetes. This study is an eye-opener, as a large proportion of young medical students having diabetes are not able to control their diabetes, despite adequate knowledge and treatment opportunity. Method: This double blind, comparative study was conducted among 200 postmenopausal osteoporotic leprosy patients. The enrolled participants were screened for various diagnostic parameters. The participants were randomized in a 1:1 ratio to receive either oral Ibandronate (150 mg single tablet- once monthly) or oral Risedronate (150 mg single tablet- once monthly) for a period of one year. Results: Present study showed that treatment with both Ibandronate and Risedronate increased hip BMD, and Z-test with repeated measurements was used to examine the significance of the longitudinal changes in the BMD. The difference in the efficacy of the two drugs in increasing hip BMD might be attributed to the greater efficacy of Ibandronate in reducing bone turnover than that of Risedronate. Conclusion: Once monthly Ibandronate has a potential to decrease the severity of osteoporosis as compared to once monthly Risedronate. It is usually asymptomatic, but it sometimes can cause different sinonasal symptoms. The severity of the symptoms is related to associate abnormalities like agger nasi cells, onodi cells, haller cells, concha bullosa and inferior turbinate hypertrophy. Methods:Patients were retrospectively selected by a computer-assisted search of all the reports of CT scans performed with a clinical symptom referable to the sinonasal region, headache, nasal obstruction, recurrent sinusitis, chronic sinusitis, acute sinusitis, allergic sinusitis in S, Nijalinagppa Medical College and Hanagal Shri Kumareshwar Hospital & Research Centre between December 1, 2016 and November 30, 2017 to detect the presence of deviated nasal septum. A total of 524 CT studies were identified. Of these cases, studies with malignancy, motion artifacts, Traumatic fractures, polyps, post-op status were excluded. The study design did not involve any patient contact hence no approval by the local ethics committee was required. Results:The sinusitis was identified in 54 patients (10.4%). The age range was 8 to 75 years with a mean age of 33.5 years. Majority of the patients in our study belonged to 21 to 30 year age group (n=21). 36 were males, 18 were females. Midline nasal septum was seen in 31 patients, among them 14 had sinusitis and 17 showed clear sinuses. Right sided septal deviation was seen in 12 patients. Among them 11 had sinusitis and one patient showed clear sinuses. Left sided septal deviation was noted in 9 cases. Among them 6 had sinusitis and 3 showed no signs of sinusitis. S shaped septum was seen in 2 patients. Septal deviation with spur was seen in 3 patients. Sinusitis was seen in 14 patients with midline septum (45.1%). In 17 patients with midline septum there was no sinusitis (54.9%). Right sided septal deviation with associated sinusitis was seen in 11 of the total 12 cases. There was statistically significant association of septal deviation and sinusitis in our study. Conclusion:The association of midline nasal septum with sinusitis is seen in 44.1% of the cases in our study. The association of deviated nasal septum with sinusitis is seen in 81% of the cases in our study. Design: A 4 months long cross-sectional study was conducted at Sports Authority of India during pre-competition period. Method: 83 national and international level players from Sports Authority of India with age 18-25 years were selected as participants and body fat percentage was estimated using both the techniques following strict adherence to pre-measurement criteria. Descriptive statistics were calculated for all the parameters. Bland-Altman analysis was carried out to compare the methods. Result: Internal consistency of the data was confirmed by Pearsons correlation value of 0.739 at p?0.05. Mean body fat percentage from anthropometry came out to be 13.085.00 and from BIA 14.789.29. Mean difference between the body fat percent values between anthropometry and BIA was found to be 1.706.53. Limits of agreement from Bland-Altman plot were found to be clinically significant with values from -14.51 to 11.11 at 95% confidence interval suggesting large discrepancies.Discussion: Based upon the findings of the study, it is not advisable to use anthropometry and BIA as interchangeable methods of body fat estimation however results need to be validated with further studies in future. Out of the two methods, BIA involves certain pre-measurement criteria like players falling in normal BMI ranges with emptied bladder, no vigorous training and food beverage intake before assessment. Methods: Total 75 neonates with clinically identified seizure episodes were enrolled in the study.EEG was done for 73 neonates and USG skull was done for all, whereas CT/MRI brain study was undertaken only if indicated by history, examination or other basic investigations. The findings of these investigations were correlated with various demographic parameters like gestational age, gender and birth weight and clinical parameters like age of onset and type of seizure, etiology of seizure, presence of status epilepticus, need for anticonvulsants and mortality. Results: A greater percentage of neonates with 1 st seizure occurring before 72 hours of life were found to have an abnormal EEG (14.70%), USG (14.28%) and CT/MRI (50%) as compared to later age of onset (5.12% and 5.00% and 34.6% respectively). Multifocal clonic and multiple types of seizures have a greater association with EEG abnormalities as compared to other types. There was a statistically significant association between clonic neonatal seizures and USG as well as CT/ MRI abnormalities.EEG, USG and CT/MRI studies revealed a higher incidence of abnormalities in patients with status epilepticus and found a statistically significant correlation between presence of status epilepticus and CT/MRI abnormalities. Most cases of metabolic convulsions do not require anticonvulsants. Conclusion: Interictal EEG provides a useful non-invasive test to diagnose neonatal seizures due to diffuse brain lesions. USG is a rapid, inexpensive and sufficiently accurate modality in evaluation of neonatal brain pathology and in the diagnosis of various lesions that occur in the first weeks of life.
