Mori","doi":"10.6014/jjsom.26.47","DOIUrl":"https://doi.org/10.6014/jjsom.26.47","url":null,"abstract":"Plasmacytosis circumorificialis (PC), a benign inflammatory mucosal disease rarely found around orifices of the body, is histologically characterized by dense plasma cell infiltration. A 56-year-old woman presented with bandlike redness of the anterior mandibular gingiva with superficial pain. Histopathologically, inflammatory cell infiltration including mainly plasma cells was observed. Immunohistochemical staining revealed these cells were positive for immunoglobulin kappa and lambda light chain. Treatment for periodontal disease and topical steroid application worked well to resolve the symptoms. Seven years before visiting our hospital, she had undergone partial glossectomies and postoperative adjuvant radiotherapy due to squamous cell carcinoma (SCC) of the left margin of the tongue at another hospital. Physical examination at the first presentation revealed tumorous lesions in the right maxilla, left cheek and labial gingiva of the anterior mandible. Biopsies were performed and the pathological diagnoses were SCCs for all biopsies. The patient underwent partial max-illectomy, resection of the buccal tumor, and marginal mandibulectomy simultaneously. We report an unusual case of calcium pyrophosphate dehydrate involving the temporomandibular joint (TMJ). The patient was a 48-year-old male with the chief complaint of pain in the left TMJ. Biopsy was performed under general anesthesia, and a diagnosis of contradiction as tophaceous pseudogout was obtained. The patient was a 24-year-old woman who was referred to our department with chief complaints of healing failure of stomatitis in the oral cavity. Ulcers were found in the tongue and palate, and findings of angina were observed. Both the qualitative and quantitative values of TPHA and RPR were positive. Biopsy from the palate showed positive immunohistochemical staining with anti Treponema pallidam antibody. Flat condyloma was also recognized and diagnosed as secondary syphilis. Ampicillin was administered and the lesion disappeared. Based on low serum zinc, we diagnosed zinc deficiency taste disorder. Dosing of zinc acetate, which has a high zinc content, was initiated and the subjective symptoms improved immediately. However, the patient complained of digestive symptoms as adverse effects, so zinc replacement therapy was continued by changing to polaprezinc.
