However, its performance may depend on the quality of requests submitted by primary care physicians. To evaluate dermatological TE activity in a regional network and to assess the impact of continuing education in dermatology and dermoscopy on the quality and efficiency of TE requests. We conducted a retrospective single-centre study of all TE requests received between January 1, 2022 and December 31, 2024 at the Centre Hospitalier Intercommunal d'Alençon-Mamers using the Omnidoc platform. After standardization, 3,266 TE cases were analysed. Data from TE requests were cross-referenced with patient records when in-person consultation occurred. Request quality and TE performance were compared between trained and untrained general practitioners. General practitioners accounted for 99.2% of TE requests, mainly for diagnostic purposes (92.4%). The most frequent indications were skin cancer screening (59.6%), inflammatory dermatoses (9.7%), and infections (9.5%). TE provided rapid specialist input (median response time: 13 hours), with only 4% of requests rejected due to insufficient information. TE avoided face-to-face consultations in 51% of cases, with high diagnostic concordance (92.4%). Training significantly improved request quality. This resulted in more efficient specialist responses. TE is an effective tool to optimize dermatological care pathways but is highly dependent on the requesting physician's level of training. Integrating structured education in dermatology and dermoscopy transforms TE into a high-performance, learning healthcare model. Investment in training is therefore essential to maximize the clinical impact of TE.</p>","PeriodicalId":11968,"journal":{"name":"European Journal of Dermatology","volume":"36 4","pages":"339-344"},"PeriodicalIF":1.9,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148900158","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objectives: This study aimed to update CSU epidemiology and assess refractoriness to initial pharmacotherapy and related determinants in Italian primary care. A cohort of patients aged 15 years and older with newly diagnosed CSU was identified from Italian primary care data from January 1, 2002, to September 30, 2022. Prevalence and incidence were calculated as % and rate per 1000 person-years, respectively. The study investigated occurrence and determinants of refractoriness to initial pharmacotherapy (H1-antihistamines and/or steroids) using multivariable Cox regression analysis. Annual CSU prevalence ranged from 0.41% in 2002 to 0.35% in 2022. The incidence was 2.07-1.93 per 1000 person-years. A cohort of 7,018 patients with CSU was identified during the study period. Of these, 50% experienced refractoriness to initial pharmacotherapy, with most events occurring within two months. Factors associated with increased refractoriness risk included age 55 years and older, autoimmune thyroiditis, asthma/COPD, unspecified allergies, and immunosuppressive drug use, including steroids. Patients who developed refractoriness were 52% more likely to be referred to dermatologists or allergists compared to non-refractory patients. These findings show that many patients, particularly those with certain coexisting conditions, do not respond well to initial CSU pharmacotherapy. By identifying these patients early through a decision support system, GPs could promptly refer them to specialists for second-line therapy evaluation.</p>","PeriodicalId":11968,"journal":{"name":"European Journal of Dermatology","volume":"36 4","pages":"353-361"},"PeriodicalIF":1.9,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148900138","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

PDT remains an effective treatment for actinic keratoses/field cancerization with increased interest in delivery of treatment indoors using artificial daylight sources as well as in home-administered daylight PDT. Sustained clearance has remained an important marker of the success of PDT for superficial and thin nodular basal cell carcinomas and updated data is presented. Objectives: To investigate the relationship between the late-to-early acute phase VZV immunoglobulin (Ig)G antibody (Ab) titre ratio (VZV IgG L/E ratio) and pain intensity after 90 days from onset in patients with HZ. In this hospital-based retrospective cohort study, we analysed the data of patients who met the following criteria between July 2019 and September 2024: 1) those clinically diagnosed with HZ within seven days from onset; 2) those in whom VZV IgG Ab titres were measured twice in the early and late acute phases; and 3) those in whom pain intensity was assessed at the earliest time point after 90 days from onset, using the numerical rating scale (NRS). VZV IgG Ab titres were measured by enzyme-linked immunosorbent assay. A total of 21 patients were included in the analysis. The VZV IgG L/E ratio negatively correlated with NRS score after 90 days from onset (ρ= -0.53). When PHN was defined as pain associated with HZ (rated as 3 or more on the NRS), persisting or appearing for more than 90 days after the onset of HZ, the receiver operating characteristic curve analysis revealed a cut-off value of the VZV IgG L/E ratio for PHN to be 1.8, with 100% sensitivity and 93% specificity. Our results suggest that the VZV IgG L/E ratio could serve as a novel PHN indicator.</p>","PeriodicalId":11968,"journal":{"name":"European Journal of Dermatology","volume":"36 4","pages":"345-352"},"PeriodicalIF":1.9,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148900147","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
