Acne in transgender patients: disease management and treatment recommendations from a group of experts on acne.

Conversely, in transwomen (men to women), feminising hormones may reduce acne but cause skin dryness and irritation which may be further exacerbated if mild acne persists, requiring topical treatments. Gonadotropin-releasing hormone analogues suppress endogenous sex hormones, thus avoiding the onset of acne in prepubertal individuals. To identify the specific issues of acne in transgender patients and propose suitable medical care for dermatologists. A group of five experts on acne reviewed and discussed 68 publications regarding the issue of acne in transgender patients. Key words included \"acne+transgender\", \"acne+transgender+treatment\", \"transgender+skin diseases+treatment\", \"transmen+acne+treatment,\" and \"transwomen+acne+treatment\". The gender affirming process in transgender patients requires a multidisciplinary approach involving endocrinologists, psychiatrists, cardiologists and dermatologists, especially if the hormonal treatment causes or worsens acne. Treatment of \"transgender\" acne differs from \"classic\" acne treatment. The administration of masculinizing hormones frequently causes moderate to severe acne which requires the administration of oral treatments involving specific conditions of use and, conversely, the use of feminising hormones reduces the risk of more severe forms of acne. However, mild to moderate acne may persist, requiring topical treatments that may further increase skin dryness and irritation. The risk of relapse after stopping treatment may be very high and justifies a maintenance topical treatment. The use of ceramide-enriched emollients is mandatory in all patients. According to previous reports, in addition to MBTPS2 variants, variants in SREBF1 (encoding SREBP1) can also cause IFAP syndrome. SREBF1 variants can also result in hereditary mucoepithelial dysplasia (HMD). These two diseases exhibit some similar clinical features. We report two cases of IFAP syndrome with atypical clinical features associated with the c.1670G>A variant in the SREBF1 gene, and review the clinical characteristics of all reported cases of IFAP syndrome and HMD patients with SREBF1 variants to date. Whole-exome sequencing was performed for the two patients, and immunohistochemistry was performed on samples from psoriatic-like plaques on the right lower limb of one of the patients. A PubMed search was conducted to identify all patients with IFAP syndrome and HMD with SREBF1 variants. A missense variant c.1670G>A in SREBF1 was identified in our two patients. The heterozygous SREBF1 variant was not identified in their parents. Immunohistochemistry of samples from the psoriatic-like plaques on the lower limb from one of the patients showed enhanced staining for IL-17A and S100A8, with reduced nuclear translocation of SREBP1. We describe two cases of IFAP syndrome without apparent photophobia, one of which exhibited severe psoriasis-like plaques limited to the extensor sides of both lower limbs. Immunohistochemical results of the lower limb lesions showed partial resemblance to psoriatic lesions. To evaluate the diagnostic accuracy for common non-cancerous skin diseases and the clinical applicability of an ML model in practical telemedicine. A prospective, multi-centre, diagnostic accuracy study including patients with common dermatoses, between October 2022 and July 2023, was performed. The top three diagnoses (Top 1, Top 2 and Top 3) from the AI system, trained to recognize 25 common dermatoses based on skin lesion images and medical data, were compared to diagnoses by two dermatologists (gold standard) to calculate the AI model's diagnostic accuracy, sensitivity, and specificity. Two versions of the AI software were evaluated: version 1 (V1) and version 2 (V2) with and without medical supervision (MS), referring to the use of metadata to control diagnostic predictions. Seventy participants and 195 photographs were included. The sensitivity and specificity of the Top 3 algorithm were 88% and 90%, respectively, for V2, with a significant improvement compared with V1. For V1, diagnostic accuracy was 0.57 (0.46;0.69) for Top 1, 0.70 (0.59;0.81) for Top 2, and 0.81 (0.72;0.91) for Top 3. For V2, diagnostic accuracy was 0.69 (0.58;0.79) and 0.71 (0.61;0.82) without and with MS, respectively, for Top 1; 0.87 (0.79;0.95) for Top 2; and 0.90 (0.83;0.97) for Top 3. Adjuvant treatment decisions of patients with stage IIB/C and III melanoma in routine clinical practice.

However, patients' decisions sometimes deviate from the advice of physicians. This retrospective study aimed to evaluate adjuvant treatment decisions, influencing factors, and outcomes for melanoma patients with stage IIB/C and III receiving routine care at the Vivantes Skin Cancer Center, which operates across three locations in Berlin, Germany. We analysed tumour board recommendations, patient treatment decisions, potential determinants, and outcomes for melanoma patients with stage IIB/C and III treated at our centre from January 2019 to December 2022 (stage III) or September 2023 (stage IIB/C). Descriptive statistics, multivariate regression, and Kaplan-Meier analyses were employed. A total of 356 cases involving adjuvant treatment decisions were identified. The tumour board recommended adjuvant treatment in 76.3% of all cases and with an option of modern adjuvants in 95.2%. For stage IIB/C, the recommendation rate significantly increased after approval of PD-1 inhibitors (90.2% vs. 35.5%, p<0.001). Given the option of approved modern adjuvants, 73.8% of patients decided to take adjuvant therapy, among them 81.7% with stage III, but only 43.1% with stage IIB/C (p<0.001). Major determinants of tumour board recommendations and patients' decisions were tumour stage and age, and the patients' most common counterargument was fear of adverse events. Adjuvant therapy led to significantly improved recurrence-free survival in patients with stage III (p=0.016).
