Immune checkpoint inhibitor-related TEN (irTEN) is much rarer than typical TEN, but mortality rates associated with irTEN are much higher than those for TEN. TEN and irTEN should therefore be managed in a different manner. To investigate biomarkers that could be used to predict outcomes and therapeutic efficacy associated with irTEN and TEN. Seven patients with TEN and two patients with irTEN were included in the study. The seven patients with TEN were treated with varying combinations and sequences of high-dose corticosteroids, intravenous immunoglobulin, pulse corticosteroids and plasma pheresis. Four of the seven TEN patients developed severe complications, and one patient eventually died due to cytomegalovirus disease. We found that, for TEN, a high neutrophil-to-lymphocyte ratio (NLR) at baseline was associated later onset of severe complications, and a subsequent decrease in NLR after starting immunosuppressive therapy was associated with a beneficial treatment response. In contrast, the opposite was found for the two irTEN patients; a low NLR at baseline and an increase in NLR after starting treatment were associated with poor outcome and a beneficial treatment response, respectively. To date, perfumes have been the most well-known counterfeit cosmetics. The aim of this study is to demonstrate counterfeit sunscreens without any photoprotective efficacy. Two creams bought online were tested. Firstly, sun protection factor (SPF) and protection factor ultraviolet A (PF-UVA) efficacy indices were determined using an in vitro method. Secondly, the dosage of UV filters was determined by high-performance liquid chromatography. The SPF and PF-UVA values were equal to 1, indicating a lack of photoprotection. The creams studied do not contain any UV filter and therefore pose a significant risk to users when exposed to the sun. {"title":"Improving the patient care pathway and multidisciplinary management are major challenges in Hidradenitis suppurativa treatment.","authors":"Ziad Reguiaï","doi":"10.1684/ejd.2025.4859","DOIUrl":"https://doi.org/10.1684/ejd.2025.4859","url":null,"abstract":"<p><p>Hidradenitis suppurativa (HS) is a chronic inflammatory disease resulting from the close interplay between genetic predisposition, dysregulation of the mechanisms involved in autoinflammatory pathways, dysfunctional response to bacterial biofilm changes, and environmental factors that potentially exacerbate the disease. Its cutaneous manifestations may represent just the tip of the iceberg, as numerous comorbidities may be associated with the disease. The risk of occurrence of inflammatory bowel disease is three times higher than that in the general population. At least 5% of patients also have spondylarthritis, and the risk of cardiovascular events and death from myocardial infarction is significantly increased. Psychiatric co-morbidities, ranging from severe anxiety to depression and suicidal risk, also contribute greatly to the disease burden. The painful and chronic nature of the flares and the highly conspicuous appearance of HS lesions, whether inflammatory or permanent scarring, can lead to social isolation and addiction, particularly to analgesics. Despite its high frequency, easy diagnosis, severe impact on patients' daily lives, and the discovery of new treatments, many patients still experience prolonged diagnostic delays, therapeutic inertia, and chaotic, inappropriate management.
