Disruption of type VII and XVII collagen turnover within the ECM is a contributing factor in the pathogenesis of skin conditions such as atopic dermatitis (AD). To investigate the turnover of type VII and XVII collagens in AD using novel biomarkers of ECM turnover. Type VII (C7M) and XVII (PRO-C17) collagen turnover were measured using ELISAs. The cohort comprised 158 AD patients and 20 matched healthy donors (HD), with 53 of the AD patients treated with topical calcineurin inhibitors. Disease severity was assessed using the SCORAD index, categorizing patients into mild (SCORAD < 25), moderate (SCORAD: 25-50), and severe (SCORAD > 50) groups. AD patients across all severity groups had higher C7M levels (all: p < 0.0001), while the levels of PRO-C17 were higher in mild AD patients versus HD (p = 0.0399). The C7M marker demonstrated an AUC of 0.96, and its levels were decreased in AD patients undergoing topical calcineurin inhibitor treatment (p=0.0073). The turnover of type VII and XVII collagens, measured by the biomarkers C7M and PRO-C17, is elevated in patients with AD. In UVA-induced photoaging, MMP1 expression is increased in dermal fibroblasts, thereby facilitating skin damage and degradation of extracellular matrix (ECM) components, such as type I collagen. To investigate whether UVA influences the level of histone H3 acetylation of the MMP1 promoter via C/EBPα/P300. In this study, skin samples (from both exposed and unexposed areas) were collected from 10 subjects, and a fibroblast photoaging model was concurrently established. RT-qPCR, western blotting, and ChIP-qPCR were employed to assess C/EBPα, P300, MMP1, and COL1A1 expression levels in the dermis and primary fibroblasts. Additionally, ChIP-qPCR was utilized to validate the binding of C/EBPα to the MMP1 promoter region, while co-immunoprecipitation was performed to confirm interaction between C/EBPα and P300. ChIP-qPCR was employed to examine the level of binding of relevant genes to the MMP1 promoter after exposure to light. Transfection with overexpression plasmids and siRNA targeting C/EBPα and P300 was performed, followed by RT-qPCR, western blotting, and ChIP-qPCR, to ascertain whether the regulation of MMP1 by C/EBPα is dependent on P300. We observed increased MMP1 mRNA and protein levels in UVA-exposed samples, which significantly positively correlated with histone H3 acetylation of the MMP1 promoter. This phenomenon may be related to UVA-induced C/EBPα-P300 complex formation, mediated by histone acetylation of the MMP1 promoter. To determine the frequency, clinical features and prognostic significance of acral involvement in the course of MF. MF subtypes, clinical features of acral lesions, and disease stage were documented retrospectively to assess possible associations. We evaluated 28 consecutive patients with acral involvement in a cohort of 773 MF patients (3.6%) over 24 years. The median age of acral involvement was 55 years. None of the patients exhibited isolated acral involvement. In 53.6% of patients, acral lesions were present at the time of MF diagnosis. Classic MF (71.4%) was the most common subtype, followed by folliculotropic (32.2%). The plantar region was the most common localization of acral involvement (53.6%), presenting typically with erythematous squamous/hyperkeratotic papules or plaques, tumours, and diffuse micropapular lesions with erosions. The dorsum of the hand and foot, and palms were mostly involved with tumoral lesions and/or erythematous squamous plaques. Although 64.3% of patients were diagnosed with early-stage MF, most of them (75%) exhibited advanced-stage disease during the course. All lesions in the acral location were associated with the involvement of other body parts in our cases. They occurred with a relatively higher ratio in folliculotropic MF. Acral lesions display a diverse clinical spectrum, including grouped micropapular lesions with erosions in the plantar region. To assess the efficacy and tolerance of a soothing and repairing balm product (Cicaplast Baume B5+, La Roche-Posay, France) when used for the treatment of skin lesions requiring regenerative care, as well as its impact on quality of life (QoL) and patient satisfaction. Patients requiring regenerative care for superficial wounds or irritative dermatitis, or requiring sedative skin care after irritating treatments, such as for actinic keratosis (AK), or skin care following a dermatological procedure were included in the study. Initially, patients' QoL and clinical characteristics were assessed, and the product was then administered appropriately. During a second visit, all clinical and QoL parameters were re-evaluated, and product tolerance rate and satisfaction were assessed. In total, 2,337 subjects participated in the study. After the recommended period of use, a significant improvement in all clinical parameters examined was observed, with ≥64.9% of patients demonstrating an improvement of at least one grade for all assessed clinical symptoms, compared to the initial visit. A significant improvement in all skin discomfort parameters examined was also observed, with ≥81.2% of patients showing an improvement of at least one grade, along with a significant reduction in DLQI and CDLQI mean scores, by 67.7% and 69.8%, respectively (p<0.001), with a high tolerance rate and overall product satisfaction.
