Its etiopathogenesis is not fully understood. Koebner’s phenomenon is considered to be a potential triggering factor, which may explain the localization of skin lesions in flexural and intertriginous areas that are not exposed to ultraviolet. A case of a 32-year-old female patient is reported, in whom the umbilicus was the only site involved by LPPI. Dermatoscopy revealed irregular pinkish structureless areas intermingled with whitish lines, grey dots, and a grey-black structureless area. The observed features did not allow a clear diagnosis. Due to the unknown duration of the lesions and technical difficulties with videodermoscopic monitoring, the lesion was excised with a 1-mm margin. These behavioural problems may occur in older individuals as well. That is why, there is an obvious need for further development in that area of dermatology, which should also include discovering new therapies. In men, the diagnosis of androgenetic alopecia is quite simple, and the treatment usually involves incorporating minoxidil and/or finasteride. Switching from topical minoxidil to its systemic form as well as from finasteride to dutasteride may give better results in some patients. In general, male pattern hair loss (MPHL) is a chronic disease that may demand pharmacological treatment in the long term, which is often unlimited. Therefore, and also considering the patients’ concerns involving the systemic usage of 5α-reductase inhibitors as well as the limitations of current therapies, some specialists use topical finasteride, dutasteride in mesotherapy and many other additional modalities. When it comes to female pattern hair loss (FPHL), in most cases it is also a chronic entity, which especially in younger women demands performing extended lab testing in the search for potential hyperandrogenism. The primary objective of this study was to assess the safety and efficacy of EFFACLAR H ISO-BIOME cream and washing cream as an adjunct to pharmacological treatment (local and/or systemic) administered for at least 12 weeks in patients with mild to very severe acne vulgaris. Material and methods: This observational study was conducted in patients (n = 306) aged 14 years and over with mild to very severe [Global Acne Severity Scale (GEA) 1–5] acne vulgaris. Participants were instructed to apply the EFFACLAR H ISO-BIOME cream and washing cream twice daily for at least 12 weeks along with prescribed anti-acne therapy (topical agents, oral isotretinoin, or systemic antibiotics/ /spironolactone combined with topical agents). Results: The treatment regimen led to an improvement in GEA scores in 88.9% of all patients (p < 0.05). Statistically significant changes were observed in the reduction of sebum secretion, number of papules and pustules, postinflammatory hyperpigmentation, and erythema (p < 0.05). The vast majority of dermatologists and patients rated the tolerance of the products as very good (97% and 94%, respectively). The quality of life of most patients improved by the end of the study. Usually, it is a mild, self-limiting condition. The severe, erythrodermic clinical appearance is rare and often demands differential diagnosis with other more serious skin conditions. We report a case of severe seborrheic dermatitis in an infant. A 5-weeks male infant presented with erythrodermic scaling lesions and exfoliation of the outermost layer of the epidermis, which had appeared after birth. Greasy scales were observed on the scalp, eyelids, and face accompanied by inflammation of the eyelids. Moreover, erythematous, well-demarcated lesions were noticed in the neck folds, behind the ears, in the axillary region, and diaper area. Considering the severe clinical appearance, additional tests such as skin biopsy and genetic analysis were performed to exclude other possible causes such as atopic dermatitis, Langerhans histiocytosis, congenital ichthyosis, and psoriasis. Based on clinical presentation and additional test results, infantile seborrheic dermatitis seemed to be the most probable diagnosis. The treatment including 1% tannic acid, 0.5% erythromycin eye cream, clotrimazole cream, hydrocortisone cream, and emollients was started in the hospital with a good response. After a month of therapy, the patient was re-admitted for the follow-up, with further improvement of the skin condition. The aim of this study is a five-year retrospective analysis of epidemiological and clinical aspects of patients hospitalized for urticarial. Material and methods: Analysis of medical records of patients hospitalized for urticaria at the department of dermatology. The clinical and epidemiological data were analysed and compared to a control group composed of 137 healthy individuals’ body mass index (BMI) and sex-matched. Statistical analysis was performed using the chi-square test, a statistically significant difference was at p < 0.05. Results: In the analysed period 137 patients were hospitalized for urticaria, 93 females (68%) and 44 males (32%), with mean age 48.53. Thirty-eight per cent of patients have been previously diagnosed with an allergy. Over 60% of patients (60 women, 18 men) suffered from acute and 38% from chronic urticaria. A causal factor was identified in 62% of all cases, the most common were drugs (48%). Urticaria patients had significantly higher levels of C-reactive protein (CRP), neutrophils and glucose than controls (all p < 0.05). The patients were treated with antihistamines (94%), glucocorticosteroids (81%), and one patient was classified for omalizumab treatment. Conclusions: Urticaria still poses a challenge as the cause often remains unknown which highlights the need for novel diagnostic methods and interdisciplinary treatment. Patients suffering from psoriasis in the adult population are predisposed to the development of cardiovascular diseases, metabolic syndrome and its components, type 2 diabetes, and inflammatory bowel diseases. In almost a third of patients, the first symptoms of psoriasis begin in childhood. Early identification of risk factors for comorbidities or detecting them at an early stage of advancement can reduce the probability of their development in adulthood. Psoriasis can have a negative impact on mental health, quality of life and functioning in school life. Its mechanism of onset still needs to be fully understood. There has been an increasing number of studies and reports confirming the beneficial influence of eradication of Helicobacter pylori on the course of the disease. It has been recognized that the bacterium leads to the activation of the inflammatory immune response, resulting in the induction of symptoms similar to rosacea. Another thesis suggests a close connection between the gut–brain–skin axis, which relates to the influence of normal microbiota and gut health, and dermatological diseases. {"title":"Hereditary alpha tryptasemia: literature overview on the genetic trait and its clinical manifestations","authors":"Agnieszka Rydz, Magdalena Lange","doi":"10.5603/fd.96810","DOIUrl":"https://doi.org/10.5603/fd.96810","url":null,"abstract":"Hereditary Alpha Tryptasemia (HαT) is a genetic condition characterized by an increased number of copies of the TPSAB1 gene, resulting in elevated basal serum tryptase levels and an increased risk of anaphylaxis, especially in individuals with IgE-dependent allergies or systemic mastocytosis. The severity of clinical symptoms can vary and is influenced by the number of extra TPSAB1 gene copies, suggesting a gene-dose effect. Approximately two-thirds of individuals with HαT show minimal or no symptoms. The remaining individuals with HαT may present with Hymenoptera venom allergy, flushing, urticarial/angioedema, irritable bowel syndrome, gastrointestinal reflux, hypermobility, neuropsychiatric symptoms and dysautonomia. Recent studies revealed that α-tryptase which forms complexes with β-tryptase activate protease-activated receptor-2 (PAR2) receptors. Activation of these receptors may lead to hypotension, muscle contraction, inflammation, and trigger neuropeptide secretion, and in consequence, result in mast cell degranulation. This cycle of activation and degranulation may potentially contribute to the development of mast cell activation syndrome (MCAS). Mast cell activation syndromes are defined by systemic, severe and recurrent mast cell activations, usually in the form of anaphylaxis. Hereditary/ familial MCAS is a specific subtype of MCAS, which is associated with HαT. Diagnostic work-up for HαT includes determination of basal serum tryptase level and the presence of additional TPSAB1 gene copies using droplet digital polymerase chain reaction. Further research is needed, to explore the relationship between HαT and MCAS, as well as to determine if there is a distinct form of hereditary MCAS which is independent of HαT.
