{"title":"Refractory bullous pemphigoid during treatment with pembrolizumab in the first-line treatment of advanced non-small cell lung cancer","authors":"Renata Olech, Monika Rychlik-Grabowska, Sławomir Mańdziuk","doi":"10.5603/fd.100000","DOIUrl":"https://doi.org/10.5603/fd.100000","url":null,"abstract":"Dermatological toxicity is one of the most common immune-related adverse events (irAEs) of treatment with immune checkpoint inhibitors (ICIs). Bullous pemphigoid (BP) is a rare and serious complication of these drugs that can be difficult to establish, as its initial symptoms may be indistinguishable from mild skin lesions. This paper presents the case of a 68-year-old patient who developed BP after receiving one of the ICI therapies, pembrolizumab, for advanced non-small cell lung cancer (NSCLC). After approximately 7 months of therapy, a grade 3 skin toxicity in the Common Terminology Criteria for Adverse Events (CTCAE) occurred in the form of rash and pruritus. Pembrolizumab was then held and prednisone and antihistamines were introduced. When dermal toxicity improved to grade 1, pembrolizumab was resumed and prednisone was kept at a dose of 10 mg. Immunotherapy was discontinued 3 months later, after the recurrence of grade 3 skin toxicity symptoms. When the patient developed blisters filled with clear fluid, dermatologists suspected pembrolizumab-induced bullous pemphigoid. Bullous pemphigoid was subsequently confirmed using a direct immunofluorescence test and histopathological examination. The patient’s skin condition improved after the use of steroid therapy and methotrexate, and the cancer process stabilized for over one year. Cancer progression and deterioration of the patient’s general condition were observed approximately 4 months after the termination of pembrolizumab therapy. The paper also discusses the key aspects of ICIs-induced BP, especially pembrolizumabinduced BP in the first-line treatment of metastatic NSCLC. Early diagnosis of skin lesions and the initiation of appropriate treatment may lead to better outcomes for patients and prevent disruptions in immunotherapy. Patients with DLE often exhibit photosensitivity, scarring, and a higher susceptibility to skin cancers. This systematic review aims to comprehensively evaluate the existing literature to better understand the relationship between DLE and cutaneous squamous cell carcinoma (cSCC). A systematic search of major medical databases was conducted to identify relevant studies published between 1978 and 2023. Studies were included if they explored the association between DLE and cSCC and reported epidemiological data, clinical features and outcomes. Evidence shows a correlation between DLE and squamous cell carcinoma. The study aimed to find literature about DLE and squamous cell carcinoma (SCC) and analyse potential risk factors. It acts by inhibiting protein synthesis inbacteria, primarily targeting the peptidyl transferase centre in the bacterial ribosome. It exhibits bacteriostatic activity, inhibiting bacterialgrowth, and at higher doses, it can be bactericidal. In the treatment of acne vulgaris, clindamycin not only exerts direct antibacterialeffects but also possesses anti-inflammatory and immunomodulatory properties. It reduces the growth of Cutibacterium acnes and inhibitsthe production of proteins and lipases, which contribute to skin inflammation. Clindamycin also enhances bacterial opsonization andphagocytosis and reduces neutrophil chemotaxis. Combination therapy with benzoyl peroxide can help minimize antibiotic resistance.Topical clindamycin, often in combination with benzoyl peroxide or retinoids, is recommended in treatment for mild to moderate papulopustularacne. In hidradenitis suppurativa clindamycin improves disease control and reduces cutaneous lesions, particularly superficialones like papules and pustules. Various topical preparations containing clindamycin are available commercially, including gels, lotions, andcombination products with tretinoin or benzoyl peroxide. Additionally, the registration of clindamycin as a pharmaceutical raw materialallows for compounding personalized formulations, providing a cost-effective alternative. This study aimed to find out people’s motivations for getting tattoos and when they decide to get them. Material and methods: This was a cross-sectional questionnaire study developed through the cooperation of a psychiatrist and a dermatologist. More than 400 volunteers with tattoos were assessed. The majority of analyzed subjects started tattooing in their twenties (mean age ± SD: 21.6 ± 5.9 years). Results: The majority of subjects indicated aesthetic reasons as the main reason for tattooing (44.5% women vs . 29.6% men; p < 0.05). The most popular tattoo motifs were animal motifs (18.9% women vs . 10.4% men; p < 0.001), plant motifs (18.0% women vs. 1.8% men; p > 0.05) and lettering (18.0% women vs . 15.3% men; p > 0.05). A total of 423 (91.5%) respondents planned to get more tattoos in the future. Conclusions: Based on the study results, it can be concluded that tattooing is currently mainly done to mostly performed to beautify the human body.
