Reports of precocious puberty with attendant hypertension are rare. Detection of the cause of hypertension and precocious puberty which can be central or peripheral is a prerequisite for institution of appropriate management. The outcome of the time, resources and efforts expended in obtaining care for this condition is dependent on the knowledge and expertise of the health care provider. Other determinants of outcome include the understanding of the minor concerned, the parental skills and parental resources. We present an 8 year old Nigerian boy with features of precocious puberty and hypertension. We highlight the not so desirable outcome and challenges associated with previous and current management of this patient. Hopefully, this report will increase awareness on this condition and improve management practices amongst health practitioners in similar settings.","PeriodicalId":11183,"journal":{"name":"Current Pediatric Research","volume":"25 1","pages":"623-626"},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70028170","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Setting-The study was conducted in tertiary care hospital of North India. Participants-Hundred children, 3-12 years of newly diagnosed epilepsy (generalised or focal motor). Intervention-Children were randomly allocated to receive oral valproate or levetiracetam in a dose of 20 mg/kg/day after initial loading dose (n=50 in each group). Drug levels were estimated by ELISA method at steady state, end of therapy and in case of any seizure activity. Side effect and repeat seizures were recorded till 6 months. Outcome measures-No repeat seizure activity for 6 months, side effect profile, achievement of therapeutic drug levels at steady state and 6 months were studied. Results: Two groups were comparable in basic demographic data. In 6 months, 8 children in valproate group and 12 in levetiracetam gr oup had r epeat seizur e (p=0.317). Mean time to first seizur e was similar in two groups (p=0.930). Mean survival time was 145.16(127.39 ± 162.93) days with levetiracetam and 159.80(145.77 ± 173.82) with valproate. The hazard ratio of valproate for treatment failure was lower. There was 38% more reduction in hazard of treatment failure with valproate compared to levetiracetam though not statistically significant (p=0.297). Overall side effects were comparable (p=0.341). Behavioural side effects were significantly more with levetiracetam (p=0.021). Conclusion: First line therapy with oral levetiracetam or valproate is comparable in efficacy and side effects in childhood epilepsy.","PeriodicalId":11183,"journal":{"name":"Current Pediatric Research","volume":"1 1","pages":"357-361"},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70028269","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objective: Study the prevalence of short stature in transfusion dependant beta thalassemia in children aged 5 years-18 years and its correlation with transfusion frequency. Materials and Methods: This is a cross sectional study performed at pediatrics department, government medical college, and Patiala included 93 cases of transfusion dependant beta thalassemia. Anthropometric data was recorded and height of every child was plotted on Indian academy of pediatrics 2015 growth charts. Short stature defined as height less than 3rd percentile. Data were collected, managed, edited, entered, and analyzed by Statistical Package of Social Sciences (SPSS) software. Receiver operating curve was analyzed for frequency of transfusion and short stature. The level of significance taken for the entire statistical test was a P-value of <0.05. Results: 64 males and 29 females were included aged 5 years-18 years with mean age of 10.09 ± 3.4 years. 41 (44.1%) cases were short stature, including 33 (80.5%) males and 8 (19.5%) females. Mean age of short stature cases was 11.9 ± 2.9 years compared to 8.6 ± 3.1 years in cases with normal height (p-value<0.05). ROC curve between short stature and transfusion frequency showed cut off of 16 days. We observed a significant correlation between frequency of transfusion and the incidence of short stature (P=0.001). Conclusion: Short stature is highly prevalent among multi-transfused beta thalassemia children and hence it is recommended that every patient should be sort out for short stature.","PeriodicalId":11183,"journal":{"name":"Current Pediatric Research","volume":"25 1","pages":"725-728"},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70028632","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Parental self-regulation is critical for maintaining positive parenting practices. However, to the best of our knowledge, existing parental trainings for ODD do not focus on the enhancement of parental reflective functions. In the present study, we compare two models of behavioral Parent Training (PT) for duration of six months: one, already consolidated, is based on the principles of Applied Behavioral Analysis (ABA), the other, innovative, integrates with the model of the regulatory and reflective parental function of Fonagy. Outcomes revealed that the innovative training was more effective as compared to the ABA training shopping that an improvement into the ability of parents to imagine the subjective experience of their developing children increases the development of children's self-regulation.","PeriodicalId":11183,"journal":{"name":"Current Pediatric Research","volume":"2 2 1","pages":"744-751"},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70028684","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Research approach was quantitative research approach and research design was pre experimental one group pretest-posttest design. Sample size was 30 and sampling technique was non probability consecutive sampling. Major finds were apparently higher in the post test group in comparison to the pre-test group. The paired ‘t’ test value is found to be 2.16 at the degree of freedom 29 and the corresponding p value is <0.05. Hence, the research hypothesis can be accepted at 5% level of significance and can be inferred that there is significant difference between pre and post-practice level.","PeriodicalId":11183,"journal":{"name":"Current Pediatric Research","volume":"25 1","pages":"436-442"},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70027268","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

UTI sometimes accompanied by bacteremia. Bacteremia is characterising as thedevelopment of a pathogen within the blood culture. Imaging tests, such as a Renal Ultrasound (RUS)and Avoiding Cystourethrogram (VCUG) use to diagnose UTIs. Aim: We aimed to describe RUS and VCUG findings in young infants with bacteremic urinary tractinfection and to determine the association between imaging findings and other variables at a tertiaryhospital in Jeddah, Saudi Arabia. Methods: We retrospectively studied the medical records of 30 infants from both genders who haddiagnosed with bacteremic UTIs at a tertiary hospital in Jeddah, Saudi Arabia. Infants were excludedif they had known urologic abnormalities at the time of presentation, required intensive unite care andinfants with immunocompromised diseases. Data was analyzed by SPSS program. Results: Of the 30 infants, 70% were boys, 50% aged from 0-3 months while the other 50% aged >3months, 14 (46.7%) were Saudis while 16 (53.3%) were non-Saudis, 61.1% were non-febrile, 58.62%had standard creatinine value. All infants 100% underwent RUS, 96.2% of them had RUSabnormalities (18 males and seven females) while only 3.8% (1 male) had typical RUS result. 37.9% ofinfants underwent VCUG, 18.2% (2 miles) of them had normal VCUG, while the majority 81.8% (7males and two females) had abnormal findings. 17.9% presented with VUR. There was no significantassociation between RUS results with sex, age group, creatinine value and urine cultures. On anotherhand, there was a significant association between RUS abnormal result and blood culture. Also, nosignificant association found between VCUG results with sex, age group, creatinine, blood and urinecultures. Conclusion: In conclusion, the prevalence of RUS and VCUG abnormality in our cohort found to behigher than other studies. Objectives: We present series of four cases that showed a very strong demand and need for genetic testing and counselling. Results/Case Presentation: We report four cases of congenital familial disease, each pair consists of siblings with both congenital and acquired heart diseases. The first pair arise from same parents, both males, 8 years and 5 years respectively with both presenting with anomaly from cono-truncal origin. The elder brother has pulmonary stenosis and the younger had truncus arteriosus. The former had surgery and was doing well but the former awaits surgery. The second pair arise from same biological parent. Both are females, 8 years and 6 years old respectively, both had cardiomyopathy, they are now awaiting cardiac transplant. Regrettably, none of the familial cases had any genetic assay or counselling because of lack of funds and facilities. Conclusion: From these cases, it is very expedient to include genetic counselling and chromosomal analysis as part of our daily practice in our locale. In Kazakhstan, as well as throughout the world, in recent years, bronchial dysplasia is increasingly common in children. However, the problems of bronchopulmonary dysplasia in Kazakhstan are not fully understood. This study will focus on monitoring the medical and social care provided to children with bronchopulmonary dysplasia and their families in Almaty. Methods: The study involved 34 children with bronchopulmonary dysplasia and their families and 64 children without bronchopulmonary dysplasia and their families. The research was carried out using a specially developed questionnaire. Results: The average age of parents of children with BPD was higher than in the control group. It turned out that 52.9% of fathers and 5.9% of mothers of sick children smoke. Families with two or three children were more common (p=0.011). The share of families with disabled children was 17.6%. Despite this, the monthly income of families is low. The main reason for bad family relations is the child's illness (29.4%). After the birth of a child, only 29.4% of mothers worked in a profession, while the majority of mothers were engaged in household chores (64.7%). 47.1% of fathers had a job; 5.9% were unemployed. It turned out that the parents have good potential to rehabilitate the sick child, but they cannot get all the necessary medical services in the clinic. It turned out that only 17.6% of clinics have pulmonologists, despite the fact that sick children need regular follow-up by a pulmonologist. Conclusion: The data obtained made it possible to identify the necessary medical and social problems for children with BPD. It is clear that such children and their families need government support.
