In order to understand the real burden of this problem, we aimed to investigate theincidence rate of the concomitant ABM in neonates with febrile UTI. Methods: This is a retrospective study reviewing the medical records of all neonates who werepresented to the Emergency Department (ED) in a tertiary hospital, Jeddah, Saudi Arabia from 1 Mayto 31 July 2019 and diagnosed with UTI to look if they had a diagnosis of acute bacterial meningitis ornot. Result: from 159 pediatric patients presented with UTIs, there were 3 (1.9%) patients with bacterialmeningitis, two of them were males. For the neonatal age, 49 neonates presented with UTI, only onepatient (2%) was diagnosed with bacterial meningitis. The isolated organisms that caused meningitiswere: yeast, Klebsiella pneumoniae (K. pneumoniae), Escherichia coli (E.coli). Conclusion: Even though our findings suggest that the incidence of ABM among neonates who haveUTI is relatively low, it still exists, and clinicians should consider them at the same time cliniciansshould balance between benefit and harm of doing lumbar puncture to every febrile neonate. Ultrasound is requested by different specialties for diagnosis of UDT before referring to a pediatric urology clinic. Objectives: To evaluate the trends for Ultrasound Scan (USS) requests by particular specialties before referral to a surgical team and to assess the sensitivity and specificity of ultrasound in accurately localizing UDT compared to per operative findings and its impact on the management plan. Methods: We reviewed all the patient between 1st of January 2014 to 1st of January 2019, who were referred to our paediatric urology department for UDT and had an USS testes before referral. Patients with ambiguous genital, hydrocele and redo orchidopexy were excluded. We recorded age at referral, referring specialty, their findings on clinical examination, the USS findings, and per operative findings. The results were expressed as mean/median, percentages, sensitivity, specificity and positive and negative predictive value. Results: We received 670 referrals for UDT to our clinic during this period. Among these 207 patients (31.04%) had an USS done before referral to us. There were nearly equal number of referrals made by the main specialties, which included neonatology, general paediatrics and family medicine. All the decisions as regards to surgery were based on the clinical examination in outpatient and the surgical approach of either open or laparoscopy was based on examination under anaesthesia irrespective of USS findings. The sensitivity and specificity of ultrasonography for correctly diagnosing a testis in the groin was 87.79% and 72.13%, in the abdomen 70.73% and 96.08% and for vanished testes 87% and 94.8% respectively when compared to per operative findings. USS missed diagnosed 43 out of 101 normal testes as either in the groin n=41(16.87%) or abdomen n=2(2.81%). This practice cost our hospital US$ 10350 (US$ 50 per scan). Conclusion: The use of ultrasound does not aid significantly in the accurate diagnosis or management of undescended testes and incur unnecessary additional cost to the health system. The rapid growth of children during the first few years and the highest nutritional requirements make children at high risk as they are considered a critical period for future developmental ability. Aim of the study: To demonstrate the real impact of congenital heart disease on children nutritional status. Patients and methods: All children involved in this study were diagnosed with congenital heart disease and were symptomatic, they were 48 patients, their age (1-18 months), and the study was performed during their preoperative period. The studies conducted from the first of March 2019 to the 30 of November 2019 at AL-Diwaniya city were seeking medical advice at the outpatient clinic. History, complete physical examination, investigations (blood investigations, chest X-ray, ECG, echocardiogram) were performed for all. Growth centiles for weight and height were measured for all of them. Wellcome classification were used to evaluate their nutritional status while waterlow classification we used it to demonstrate the stages of acute and chronic malnutrition. Results: Male to female ratio was 2:1 (32 males, 16 females), 26 presented with cyanotic CHD while 22 were with non-cyanotic defects. When we evaluated patients for severe childhood under nutrition, the severely acute malnutrition ratio with wasting was 66.6% while chronic malnutrition according to growth stunting was 83.3%. Material and Methods: This pilot exploratory study involved determining TUG values of Saudi preschool-aged children with typical development. Differences in the mean and standard deviation of TUG values for each age group, gender, height, and Body Mass Index (BMI) were tested. Results: During the study period, 61 typically developed Saudi preschool children aged 4.37 ± 1.10 were tested, with 55.7% of the sample being male. TUG values ranged from 4.74 to 5.20 seconds across all age groups, with time taken tending to decrease for older children in the study group. No significant differences between males and females in TUG timing (P=0.81) were observed. Materials and Methods: 73 children were operated on for LAIO. 35 children were in a comparison group (comprehensive treatment by traditional methods) and 38 children were in the main group. In the main group was used an anti-adhesive solution which containing sodium hyaluronate and decamethoxin to prevent recurrence of Adhesive Intestinal Obstruction (AIO). The following surgical interventions were performed: Adhesiolysis, adhesiolysis and resection of the small intestine, adhesiolysis and making of the ileostomy, elimination of the cause without total adhesiolysis, elimination of the cause without total adhesiolysis and with resection of the small intestine, elimination of the cause without total adhesiolysis and with making of ileostomy. The Adhesion Process (AP) in the abdominal cavity was evaluated by the type of adhesions and their projection on the anterior abdominal wall. Results: The most common causes of LAIO were adhesive conglomerates (32.88%), moorings (23.28%) and visceral-visceral adhesions (21.92%) in lesser extent visceral-parietal and omentalvisceral adhesions, accordingly 10.96% and 10.96%. In main group recurrent AIO was seen in 3 of 38 children (7.89%) and resolved conservatively. In comparison group recurrent AIO occurred in 7 of 35 children (20%). In comparison group, 4 of 7 children required relaparotomy. The method of cause eliminating was used at AIO without total adhesiolysis using sodium hyaluronate for the operated children. When children were observed between 1 and 7 years, recurrence of AIO was in 1 patient who resolved conservatively. Conclusion: Mesothelium damaging leads to the emergence of the primary AP of the abdominal cavity with spreading to the surrounding structures and projection on the areas of the anterior abdominal wall. In operations for LAIO in children, it is advisable to use the operation of the cause eliminating (separation or resection of the conglomerates, cutting of the mooring) without total adhesiolysis in combination with intraoperative use of sodium hyaluronate solution with decamethoxine. The safety and efficacy of Holmium: YAG laser lithotripsy makes it the intracorporeal lithotripter of choice. Aim of Study: To assess the effectives of ureteroscopy using Holmium: YAG lithotripsy in the treatment of ureteric stone in pediatrics. Patients and Methods: 30 children (20 boys and 10 girls) with ureteric stones were treated by semirigid ureteroscope using Holmium: YAG laser lithotripsy between October 2018 to May 2020. Mean patient age 3.6 years (range 8 months to 14 years). Mean stone size 12.8 mm (range 7-20 mm). Preoperative evaluation done including urinalysis, abdominal ultrasonography, plain radiography and blood investigations such as complete blood count and renal function test. Non contract abdominal CT scan used in some cases. Results: The stone free rate was 100% in one session. The mean operative time was 37.3 (25-60 minutes). No significant intraoperative or postoperative complications were encountered. Background: Young children in India suffer from some of the highest levels of stunting, underweight, and wasting observed in any country in the world. The levels of over-nutrition are also on a rise. Prevention of child malnutrition require diets providing adequate energy and essential nutrients to promote catch-up growth, strengthen resistance to infection, and support normal mental, physical and metabolic development. Methods: This is an observational study, where an oral nutritional supplement is given to 776 children, aged 2-12 years, for 6 months along with normal diet. Anthropometric parameters (height, weight and BMI) are assessed at baseline, 3 and 6 months. The z score for height, weight and BMI is used to analyze the results using Khadilkar growth chart, Indian Pediatrics 2009. Results: A total of 763 subjects are included in the analysis. Each child is grouped according to age bracket (2-3 yrs; 4-6 yrs; 7-9 yrs; 10-12 yrs). After consumption of nutritional supplement for 6 months, z-scores for height, weight and BMI are shown improvement in almost all age groups, compared to baseline. The improvement is significant in weight and BMI z-score. The standard deviation scores from expected increase in mean of weight, height and BMI is well within the permissible range. No adverse event is observed. Abdelsalam","doi":"10.35841/0971-9032.25.2.355-357","DOIUrl":"https://doi.org/10.35841/0971-9032.25.2.355-357","url":null,"abstract":"Objectives: Laparoscopic-Assisted Percutaneous Endoscopic Gastrostomy (LAPEG) is a wellrecognised technique used for the placement of gastrostomies in patients with complex medical conditions or postsurgical difficult anatomy. We introduced LAPEG in our centre to improve outcome and increase safety in our complex paediatric population. We aim to review the outcome and complications of LAPEG in children since our change in practice. Methods: A retrospective tertiary centre review over 9 years (September 2010- September 2019) was conducted. Children under 16 years undergoing LAPEG were reviewed for risk factors and major complications. Results: 76 patients were identified, 44 males. Median age was 1 year (1 month–14 years), median weight was 8.4 kg (2.8-33.9 kg). A third was less than 7 kg and a third were in Paediatric Intensive Care Unit at the time of the procedure. A quarter had underlying congenital heart disease, a quarter had previous abdominal surgery and 15% (11/76) had American Society of Anaesthesia score of 4. The median procedure time was 29 min (18-83 min). No patient required conversion into open surgery. There was no adjacent bowel or organ injury, fistula formation, intraperitoneal leek, bleeding and no complications requiring surgical intervention. Three patients (3.9%) experienced early tube dislodgement. All dislodged gastrostomies were successfully replaced at bedside. Conclusions: LAPEG in our study was a safe method for gastrostomy insertion particularly in our complex high-risk patients. Dental profession falls under very high risk category of virus transmission because of their close contact with the patient and production of aerosols during dental procedures. Therefore, to sustain dental practice, the dentists must find out different approaches which can minimize the risk of infection. One such approach is Minimal Invasive Dentistry (MID). Nurses working in inpatient department along with especially labor room and neonatal intensive care unit should have proper knowledge regarding newborn resuscitation. The present study aimed to assess the knowledge regarding neonatal resuscitation among nurses working in inpatient department of selected wards in Gandaki Medical College Teaching Hospital and Research Center (GMCTHRC). Descriptive, cross sectional study design with purposive sampling technique was used to collect the data from 130 nurses. Semi structured self-administered knowledge questionnaire for knowledge was used for data collection. The collected data were analyzed using descriptive and inferential statistics. The study revealed that, more than half of the respondents 64.6% had inadequate knowledge. There was significant association between knowledge level and working unit (p=0.013). Most of the nurses were found to have inadequate level of knowledge.
