{"title":"Prognosis Prediction of Patients with Chronic Heart Failure by Platelet Distribution Width and Serum Iron Metabolism Indicators.","authors":"Hong Deng, Daxing Wang, Rui Yang","doi":"10.7754/Clin.Lab.2025.250101","DOIUrl":"https://doi.org/10.7754/Clin.Lab.2025.250101","url":null,"abstract":"<p><strong>Background: </strong>We aimed to investigate the correlations of platelet distribution width (PDW) and serum iron metabolism indicators with cardiac function and inflammatory response in patients with chronic heart failure (CHF) and their predictive values for poor prognosis.</p><p><strong>Methods: </strong>Eighty-four patients with CHF treated from August 2022 through September 2024 were retrospectively selected as the HF group, while eighty-eight healthy people with normal cardiac function during the same period were selected as the control group. PDW, serum ferritin (SF), transferrin saturation (TS), cardiac function indicators, and inflammatory response were compared between the two groups.</p><p><strong>Results: </strong>The levels of interleukin-6 (IL-6), C-reactive protein (hs-CRP), and tumor necrosis factor-α (TNF-α) in the HF group were significantly higher than in the control group (p < 0.05). The results of the Pearson correlation analysis revealed that left ventricular ejection fraction (LVEF) was negatively correlated with PDW and positively correlated with SF and TS. The IL-6, hs-CRP, and TNF-α levels all had a positive correlation with PDW and negative correlations with SF and TS (p < 0.05). It is more commonly observed in children and less likely in adults. We report a case of an elderly man with elevated squamous cell carcinoma-associated antigen combined with multiple nodules in both lungs, initially diagnosed as lung cancer and eventually diagnosed as multiple foreign bodies in both lungs by bronchoscopy.</p><p><strong>Methods: </strong>Appropriate laboratory tests, chest enhanced CT, and bronchoscopy.</p><p><strong>Results: </strong>Squamous cell carcinoma-associated antigen (SCC) was elevated to 6.73 ng/mL, and an enhanced chest CT showed multiple nodular ground glass shadows in both lungs. Bronchoscopy indicated that smooth green foreign bodies were visible in the opening of the upper lobe of the left lung, smooth red foreign bodies were visible in the opening of the lower lobe of the right lung, irregular white foreign bodies were visible in the opening of the anterior segment of the upper lobe of the right lung, and the surrounding granulation tissue was hyperplasia. After the removal of the foreign body, the airway became unobstructed, and the patient's blood oxygen saturation was significantly improved.</p><p><strong>Conclusions: </strong>The clinical manifestation of tracheobronchial foreign body in the elderly are not typical, so they are often missed or misdiagnosed. The bronchoscope is very important in diagnosing and treating the trachea and bronchial foreign body. Gynostemma pentaphyllum, a prominent traditional Chinese medicine, is clinically utilized to treat various conditions such as diabetes mellitus, hepatic fibrosis, hypertension, hyperlipidemia, and other disorders. This report details a case of a cerebral infarction patient who developed significant thrombocytope-nia following treatment with Gynostemma pentaphyllum. This case underscores the importance of monitoring platelet counts in patients using Gynostemma pentaphyllum.</p><p><strong>Methods: </strong>The patient's platelet counts were measured using a fully automated blood cell counter. Her whole blood cell concentrations were assessed before and after discontinuation of Gynostemma pentaphyllum.</p><p><strong>Results: </strong>Initial blood tests revealed leukocyte and platelet concentrations of 3.27 x 109/L and 6.0 x 109/L, respectively, after the patient had used Gynostemma pentaphyllum continuously for over three months. Post-discontinuation, two subsequent blood tests showed leukocyte counts of 5.26 x 109/L and 5.52 x 109/L, and platelet counts rebounded to 156 x 109/L.</p><p><strong>Conclusions: </strong>Gynostemma pentaphyllum may induce thrombocytopenia. The Sigma metric imprecision and bias were calculated by using internal and external quality control data, respectively, then the Sigma metric and quality goal index (QGI) was calculated to assess and improve the performance of laboratory process system.</p><p><strong>Results: </strong>Sigma levels from 21 clinical analytes were found ≥ 6, and the analytical performance of these analytes were at \"world-class\" level. The analytical performance of serum potassium, glucose, bicarbonate, β2-MG, LDH, and direct bilirubin reached \"excellent\" level (5 ≤ σ < 6 and QGI ≤ 0.8), and multiple rules of 13s22sR4s (N = 2, R = 1) with batch length of 450 patient samples were selected as QC schemes. The analytical performance of sodium and TBA also achieved \"good\" level (4 ≤ σ < 5 and QGI ≤ 0.8), so multiple rules of 13s22sR4s41s (N = 4, R = 1 or N = 2, R = 2) with batch length of 200 patient samples were selected as QC schemes. Sigma level of chloride, total protein, total bilirubin, and albumin was < 4 and ≥ 3 (QGI ≤ 0.8), so multiple rules of 13s22sR4s41s8x (N = 4, R = 2 or N = 2, R = 4) with batch length of 45 patient samples were selected as QC schemes. Due to serological test limitations, genotyping is implemented to find appropriate and compatible blood units. This study aimed to determine extended blood group genotypes to identify antibody specificity and to establish appropriate typing for Thai patients with repeated transfusions.</p><p><strong>Methods: </strong>Blood samples obtained from 200 unrelated Thai patients with repeated transfusions, at the Thammasat University Hospital, Pathum Thani, Thailand, were included. The patients had recently received at least three units of donor RBCs within 3 to 12 weeks at the time of blood collection. Extended blood group genotyping using the PCR-sequence-specific primer technique determined KEL*03, KEL*04, DI*A, DI*B, DO*A, DO*B, CO*A, CO*B, CROM*01.12, and CROM*01.-12 alleles.</p><p><strong>Results: </strong>Among 200 patients, no significant difference was found between male and female ratios (p > 0.05). Concerning the antibody production divided by patients' diagnoses, we found that only 2 patients with chronic kidney disease, CKD, (6.3%) had significantly lower antibody detection than patients with thalassemia, hematological and other malignancies, and other disorders (p < 0.05). Based on extended blood group allele patterns among 200 patients and 500 donors, the two common allele patterns were first: KEL*04, DI*B, DO*B, CO*A, and CROM*01.12 (75.0 vs. 71.0%), and second: KEL*04, DI*B, DO*A, DO*B, CO*A, and CROM*01.12 (18.0 vs. 21.6%), consistent with the donor population. As a result, this patient group showed a high chance of identifying matched donors and a low risk of alloimmunization. The prevalence of the DI*A/DI*B heterozygote was observed at 4.6% among 500 donors, corresponding to the high prevalence of anti-Dia in the patient group. Therefore, extended blood group genotypes, particularly the common significant blood group antigens in populations, are helpful among patients requiring repeated transfusions.</p><p><strong>Conclusions: </strong>Extended blood group genotyping among patients with repeated transfusions should be performed to select antigen-negative RBC transfusions. The availability of sensitive methods for its detection can generate false-positive results, requiring a confirmatory test that is both costly and unavailable in all medical laboratories. The objective of the study was to determine, using the two automated systems (Architect and Alinity i (Abbott)), a cutoff value at which HBsAg is considered positive without the need for a confirmatory test and to assess the role of anti-HBc antibodies as an indirect marker of qualitative HBsAg confirmation.</p><p><strong>Methods: </strong>The study included 328 HBsAg positive (≥ 1 S/CO) and repeatable samples tested on Architect (Abbott) or Alinity i (Abbott) using the microparticle chemiluminescence immunoassay technique (CMIA). The samples were confirmed by the Architect or Alinity i HBs Ag qualitative II Confirmatory neutralization test, with concomitant detection of anti-HBc antibodies in the majority of patients.</p><p><strong>Results: </strong>Out of the 223 repeatably reactive samples tested on Architect, 173 (77.6%) were true positives and 50 (22.42%) were false positives. For samples processed on Alinity i, out of the 131 tested, 105 (78.9%) were true positives and 26 (21.1%) were false positives. A specificity of 100% was obtained, with an HBsAg index value of 14.04 for Architect and 24.93 for Alinity i. Regarding the results of the anti-HBc total antibodies assay, they were concordant with the results of the confirmatory test in 89.62% and 95.03% for Architect (Abbott) and Alinity i (Abbott), respectively.</p><p><strong>Conclusions: </strong>Our study showed concordance between the Architect and Alinity i (Abbott) automated systems. According to ROC curve, the neutralization test is particularly relevant for HBsAg index values between 1.15 S/CO and 24.93 S/CO. Dysregulation in uric acid production or excretion can result in hyperuricemia and gout. Several proteins play important roles in uric acid metabolism. Among them, GLUT9, a key protein for uric acid excretion, has garnered significant attention, particularly for two SNPs (rs3733591 and rs1014290) on its encoding gene SLC2A9. However, their relationship with gout and hyperuricemia in the Han Chinese population has not been researched.</p><p><strong>Methods: </strong>This study investigated 498 individuals, including 300 patients with hyperuricemia or gout and 198 healthy controls. Serum uric acid levels were measured, and the genotypes of rs3733591 and rs1014290 were determined using the multicolor melting curve analysis (MMCA) method. These results were subjected to statistical analysis.</p><p><strong>Results: </strong>This detection result highlights the value of MMCA as a rapid and accurate method for genotyping rs3733591 and rs1014290. Statistical analysis revealed that the proportion of the wild-type (C) allele at rs3733591 and the mutant (A) allele at rs1014290 were significantly higher in patients with hyperuricemia and gout compared to healthy controls. Additionally, individuals with the homozygous mutant genotype at rs3733591 had significantly lower uric acid levels compared to those with the wild-type and heterozygous genotypes, whereas homozygous mutants of rs1014290 exhibited higher uric acid levels. Among the various models based on clinical and laboratory data from 433 participants, the logistic regression model based on eight factors - gender, age, eGFR, WBC, HDL, MCHC, rs3733591, and rs1014290 - demonstrated the best diagnostic performance for gout and hyperuricemia, achieving an AUC of 0.8737.</p><p><strong>Conclusion: </strong>In conclusion, this study demonstrated a close association between SLC2A9 gene polymorphisms and gout as well as hyperuricemia in the Han Chinese population. Both EUCAST and CLSI recommend broth microdilution (BMD) for antimicrobial susceptibility testing of colistin, but BMD is rarely used in routine microbiology laboratories. In search of alternative, more practical methods, some commercial kits were propagated, along with CLSI's recommendation of using Colistin disc elution method. In this study, we aimed to compare colistin susceptibility testing both by MMS, which is a commercial BMD product, and by Colistin disc elution method to BMD using multidrug-resistant Acinetobacter baumanii, Pseudomonas aeruginosa, and Klebsiella pneumoniae.</p><p><strong>Methods: </strong>Multidrug-resistant Acinetobacter baumannii, P. aeruginosa, and K. pneumoniae isolates detected from various clinical samples in Marmara University Pendik Training and Research Hospital´s clinical microbiology laboratory between January 1, 2021, and July 30, 2022, were included in the study. Colistin susceptibilities were determined using the CBDE method, Micronaut MIC Strip colistin assay (MMS), and BMD. The results were compared with BMD as the reference method. Categorical agreement (CA), essential agreement (EA), major error (ME), and very major error (VME) rates were calculated.</p><p><strong>Results: </strong>The study included 185 multidrug-resistant A. baumannii, P. aeruginosa, and K. pneumoniae isolates from various clinical samples. Out of these isolates, 13 (6.5%) were found to be resistant. The MMS test demonstrated a categorical agreement (CA) of 100% for A. baumannii and P. aeruginosa, while the CBDE method achieved a CA of 100% only with A. baumannii.</p><p><strong>Conclusions: </strong>Both the MMS and CBDE tests are compatible with the BMD method. The CBDE, which does not require specialized equipment or advanced techniques, may be more suitable for laboratories with limited resources. Pembrolizumab has shown potential in patients with micro satellite instability-high tumors. We report a rare case of complete remission of metastatic pancreatic cancer.</p><p><strong>Methods: </strong>A 58-year-old male with MSI-H advanced PDAC, who progressed during adjuvant and metastatic first-line treatment, was treated with pembrolizumab showed significant clinical improvement.</p><p><strong>Results: </strong>Pembrolizumab led to complete response. The purpose of the study was to observe the Adriamycin (ADM)-resistance changes of SGC 7901/mdr1 caused by ART, compare it with SGC7901, and explore the cell apoptosis and changes of Bcl-2's expression in the reversal process.</p><p><strong>Methods: </strong>MTT was used to test ART's drug-resistance reversal effect. Fluorescence microscope and flow cytometry were applied to test the apoptosis of SGC7901/mdr1 cells. Western blotting was used to test the expressions of Bcl-2 and caspase-3.</p><p><strong>Results: </strong>It was found that non-cytotoxic ART combined with ADM in different concentrations can produce intensified damage to SGC7901/mdr1. The resistance reversal ratio of ART to drug resistant cells was about 2.31 times, and the relative reversal rate was about 83%. After 48h combination of non-cytotoxic ART and non-cytotoxic ADM, cell apoptosis was observed in morphology. The results of FC showed that the apoptosis ratio of combined group reached 15%.
