By precisely matching the restoration color with the remaining teeth, the possibility of inconvenience such as a correction or re-making of the same restoration is minimized. Since determining tooth color in everyday practice is done visually by using specially made color selection shade guides, unawareness of the correct method and the conditions under which it should be administered may increase the failure of the therapy.\u0000The aim of our research was to determine whether education and training in determining tooth color influence the choice of precise and correct shade.\u0000The study included a total of (N = 90)dental medicine students who were first tested with the Ishihara color vision test and Farnsworth-Munsell 100 HueColor Vision test to exclude the possibility of an error due to the presence of a color vision anomaly of the eye. We have formed two groups with an equal number of respondents, work and control group. Both groups were tested twice with a special color coding program ToothGuideTrainer WEB-final exam, which uses VITA Linearguide 3D-MASTER® shade guide. During the testing, the working group went through education, in the form of lectures, exercises and training tests for determining the color. The control group did not attend the training. The results from both tests were electronically counted. Mean value and standard deviation were determined. Differences between the work and control group were analyzed with t-test.\u0000The workgroup showed statistically significant higher points after education and training than the control group. The points earned before training did not differ significantly between the two groups.\u0000The education and training for shade matching in dentistry significantly affect the success of selecting the perfect color for future restoration. It is necessary to include more intensive training in determining color within the program of dental medicine studies. The job of the registered nurse requires high professionalism and participation in continuous post-graduate training and courses so that nurses can maintain a high level of competence.\u0000The mentor of future nurses has to be able to identify the different levels of acquired knowledge, skills and competences in trainees.\u0000Aim: The aim of this study is to establish the pedagogical competence of mentors and their readiness and preparation to participate in the process of training and education during the pre-graduation traineeship of the future registered nurse.\u0000Methods and subjects: In order to research the opinion of mentors of their own pedagogical competence and skills to participate in the training process during pre-graduation traineeship of the trainee-nurses assigned to them, we surveyed mentors at the following university hospitals – ‘St. George’ University Hospital, Plovdiv, MPHAL AD Haskovo, ‘Dr. At. Dafovski’ MPHAT Kardzhali, University Hospital – Stara Zagora.\u0000Findings and discussion: The questionnaire included questions to the mentors for the purpose of establishing their opinion of the required pedagogical competences and the need of further training of the mentors, and, if such need is established, the topics that should be covered. Further training of mentors shall facilitate a better quality training process during pre-graduation traineeships. {"title":"BODY FAT DISTRIBUTION AND LIPID PROFILE CHANGES AFTER WEIGHT LOSS – A CASE REPORT","authors":"S. In this case report following subject’s parameters were determined before and after weight loss of 12 kg: body mass index (BMI), body weight (BW), total, trunk and legs fat mass (FM) with dual energy x-ray absorptiometry (DXA) and their ratios legs/trunk and legs/total FM indexes of abdominal fat distribution. Lipid profile was also determined before and after weight loss: total cholesterol (TC), triglycerides (TG), HDL-C, LDL-C, LDL/HDL-C, TC/HDL-C. BW of 63 kg and BMI value of 28.74 kg/m2 before the weight loss lowered to 51kg and 22.55 kg/m2. The percentage difference between the change in BMI and BW was statistically not significant (p=0.782). Legs/total FM index value increase from 0.36 to 0.39 was significant (p<0.025). Legs/trunk FM index value increase from 0.67 to 0.76 was also significant (p<0.043). Legs, total and trunk FM had not significant reduction (p>0.05), but the percentage difference between their changes, during weight reduction was statistically significant (p=0.0001). TC, HDL-C, LDL-C, LDL/HDL-C, TC/HDL-C changes were also significant. The percentage difference between the change in BMI and BW to normal levels was statistically not significant, but the percentage difference between the changes in DXA indexes of visceral, abdominal obesity was significant and it was associated with significant reduction of atherogenic lipid profile indicating reduced atherogenic risk. {"title":"TEETH EXTRACTION WITH ROOT DELACERATION ON THE ORTHODONTIC INDICATIONS","authors":"P. Stanimirov, G. The actual mechanical obstructions in the way of dental eruption can be: root canal infections in the neighborhood tissues, development of the ectopically located dental germs and lack of space for them, anatomically dense structures, ankylosis temporary tooth, and so on.\u0000The purpose of our study is to analyze the causes of dental root dilaceration in patients, the first with the upper central incisor affected, and the second with the lower first premolar affected, as well as the approaches to their orthodontic treatment.\u0000In the first clinical case, the root dilaceration of the lower right first premolar is due to the development of a large cystic formation from a devialized lower right first temporary molar. In the second clinical case a rectangular upper left central incisor with a delacerated apex and a reversed direction of a crown-root was found, with the crown near the nasal cavity. The most accurate morphology of the affected teeth may be performed by a CBCT study.\u0000The treatment approach in both patients is extraction of the tooth with root dilaceration. In the first clinical case, the mechanical force that compresses and changes the direction of tooth formation is the cystic collection. In the second clinical case, the real cause of the delaceration and inverted direction of the germ of an upper left central incisor is not clear.\u0000A trauma of temporary teeth is often overlooked by parents. An early and timely intervention can save patients from the consequences of this trauma. Clinicians, who treat such as cases should use pre-diagnostic means such as CBCT to plan the treatment. Parents and dentists should devote particular attention to the deviated temporary teeth which shift is delayed. It is more common in male than female patient, with a wide age range. Regarding varied and often unspecific symptomatology it poses a great problem for diagnostic and overall treatment with a very high mortality rate in untreated patients. Recent diagnosis and treatment are based on modern imaging techniques that enable precise and rapid diagnostics. The following is a presentation of a case that refers to a young male patient with splenic abscess.\u0000CASE REPORT. A 37-year-old patient with a history of hypertensive crisis and multiple previous admittances to the emergency room regarding acute attacks of pancreatitis and persistent reduction in body weight was admitted to the emergency room with nonspecific symptoms of abdominal pain, more prominent in the upper left quadrant. Laboratory findings revealed leukocytosis and elevated levels of CRP. Infective panel for hepatitis and HIV was negative. Additional ultrasound investigations were made which revealed hypoechogenic lesion in the spleen, clearly demarcated from the neighboring parenchyma and partially encapsulated, with near proximity to the tail of the pancreas. Hospitalization with additional MRI and surgical treatment was advised, but the patient refused and contrary to the advice of the doctor decided to leave the hospital. After 10 days he was again admitted to the emergency room with persistent leukocytosis and elevated CRP. Because of the worsening condition, he accepted the previously proposed treatment and was hospitalized. The conducted MRI investigation revealed three cystic lesions with dense content in the spleen, measuring from 5x3sm to 2sm in diameter with propagation towards the tail of the pancreas. Surgical splenectomy with partial pancreatectomy was performed, and the surgical specimen was forwarded for histopathological examination. The microscopic examination on the selected specimens revealed severe stasis in the splenic parenchyma with subcapsular abscess formation, and also confirmed the chronic pancreatitis condition. The postoperative course went well without any complications, as well as on the following regular checkups.\u0000DISCUSSION. Splenic abscess is very uncommon entity associated with versatile etiologies which covers primary immunocompromised patients, trauma or patients with infective endocarditis. As a potentially life-threatening condition it is essential to emphasize the need for fast detection and splenectomy as a choice of treatment. The goal of this study is to investigate the changes in the facial appearances in treated patients withFace mask orthopedic treatment and untreated Class III patients. The sample consisted 49 patients (boys and girls),with average age of 9 years, who had a Class III Malocclusion with an anterior crossbite and a component of maxillary deficiency. 28 of them were treated with protraction Face mask- Delair mask (petit tipe), and the other 21 were presenting the control group consisted of untreated Class III Patients.In treated group pretreatment and posttreatment cephalometric radiographs from 28 patients(15 males and 13 females) were analyzed and compared with the results of cephalometric analyzes in untreated group(observation period of 1 year). Zdravkovska, Irena Eftimovska Rogac, Nadica Bozhinovska","doi":"10.35120/kij3104925b","DOIUrl":"https://doi.org/10.35120/kij3104925b","url":null,"abstract":"Tumors of the small intestine are extremely rare, and clinical signs and symptoms are nonspecific, resulting in prolongation of the diagnosis process and subsequently worsening the outcome of the treatment. In addition to non-specific symptomatology, additional difficulty in diagnosing is the very inaccessibility of the jejunum through endoscopic techniques. The following is a review of the case of a female patient with jejunum carcinoma. CASE REPORT. A 64-year-old patient with nonspecific symptoms of fatigue, weight loss and sore pain in the stomach initially hospitalized due to anemic syndrome. An endoscopic evaluation was performed, and the fining was chronic gastritis. The patient was prescribed with iron supplementation therapy and was sent home. Within a month, the fore mentioned symptoms started to intensify, and the patient was hospitalized at the General Hospital in Skopje, where extensive investigations were conducted. Upper digestive endoscopy was made, and the finding again only showed chronic gastritis. Due to lack of findings, the doctors performed abdominal ultrasonography that indicated only a bolded intestinal segment with a thick wall of 9mm in the projection of the left flexure to the descending colon. The need for additional investigations grew and computed tomography with contrast of the abdomen and small pelvis was performed. The finding of the computed tomography was in favor of an irregular intestinal segment in the projections of the jejunal convolutions, with a thickened heterogenic wall and pathological post-contrast coloring. Due to the need for correlation with other trials, MRI was performed, and its finding was highly suspected of a tumor change in the jejunum, but an inflammatory disease was not excluded. Because of this finding, the patient was sent to the Department of Digestive Surgery at the same facility. From a surgical point of view, the patient was treated with an upper medial laparotomy, which made it possible for the tumor to be released from its surroundings. The surgery and after surgery course and the patient’s condition were all as expected. The patient was in a stable general condition after the surgery, and she was sent home. The pathophysiological finding resulted in jejunal adenocarcinoma (Latin adenocarcinoma intestinijejuni). After the pathophysiological finding was obtained, the patient was referred to an oncologist for eventual adjuvant therapy. DISCUSSION. Small intestine carcinoma is a specific clinical and surgical entity, which is often diagnosed in an already overdue phase. This is due in part to the non-cohesive symptomatology, but largely due to the still insufficiently sophisticated detection methods. The cervical region of the spine is subjected to significant dynamic and static loads, which often leads to pain and limited mobility. Improper posture, continuous static tension, muscular hypertonia and functional blockages in the cervical region provoke psychosommatic discomfort, stiffness, headaches and reduced capacity to work. An essential place in the treatment of osteoarthrosis occupy physical and rehabilitative means. Very good results are obtained from the recreated physical methods, kinesitherapy, extension, manual therapy, massage and natural factors combined with training of the patient. The purpose of this publication is to present an algorithm of a complex program for physical treatment and rehabilitation of cervicoarthrosis, as well as preventive measures and ergonomic control in a sitting working posture. Today, their uncontrolled and inappropriate usage is considered as a huge public health problem, because of the occurrence of antibiotics resistance resistance of microorganisms to antimicrobial agents. Around 50% of the antibiotics prescribed during the hospitalization period of the patients are unnecessary or inappropriate. Endemic forms of occurrence and maintenance of resistant and multiresistant bacteria in hospitals, especially in intensive care units, increase morbidity, treatment costs and mortality of hospitalized patients. Aim: To confirm the information and knowledge of the antibiotics usage and antibiotics resistance among the population of the eastern region of Republic of North Macedonia. Material and Methods: The paper is a transphase study conducted in the period from 01.10.2018 30.11.2018 on the territory of the municipalities of Stip, Probistip, Kocani, Radovis and Strumica. Fur fulfilling the set goals, a specially designed questionnaire was made, and a total of 526 respondents fulfilled it voluntarily and anonymously. The descriptive and the analytical epidemiological method of work were used. Results: From the total number of 526 respondents, 230 (43,7%) were men, and 296 (56,3%) were women. The respondents were on the age from 18 to 70 years. 385 (73,2%) of them live in a city. The female population take antibiotics with a doctor's recommendation more in comparison with the male population. More than 50% of the respondents confirmed that the antibiotics affect both bacterias and viruses. 98 (42,6%) of the men, and 264 (56,1%) of the women responded that they are aware what antibiotics resistance is. From the total number of 264 respondents who said that know about antibiotics resistance, only 127 (48,1%) indicate that the doctors and tge pharmacists as the source of information. Conclusion: The population of the eastern region of our country is not informed enough for antibiotics usage and the antibiotics resistance. The information that only 127 (24,1%) respondents of a total number of 526 respondents has gained their information from doctors and pharmacists is worrying.
