In the Ministry of Health in Bulgaria have been reported and confirmed 223 cases of measles by 03.04.2019.\u0000Measles is an acute infectious disease characterized by common intoxication, inflammation of the upper respiratory tract mucosa, specific maculopapular rash on the skin and respiratory complications. The causative agent of measles belongs to Paramyxoviridae family, Paramyxovirus genera.\u0000Live vaccines are most widely used for active prophylaxis, derived from attenuated measles virus. In Bulgaria, it is given to children aged 13 months and reimmunization at 12 years of age. The vaccine is also used in combination with a mumps and rubella vaccine. Vaccinated individuals acquire lifetime immunity. Mass administration of the vaccine leads to a reduction in measles morbidity to single cases.\u0000Despite the increase in measles prevalence and morbidity, young people are unaware of this disease and the possible complications after illness. This depends on the training and professional skills of managers and nurses. The skills of managers in planning, organizing, targeting and controlling financial, human and material resources determine the healthcare quality and effectiveness, which is an important condition for the sustainable development of the healthcare establishments in terms of competition and market economy. This underlines the importance of training in educational establishments that create managers, health professionals and provide a basis for development and improvement of their qualification. The quality and effectiveness of education is connected to student satisfaction with the organization and quality of educational process, because this depends to a great extent on their motivation for participation, self-preparation and realization. All this is reflected in the modern educational system. The nurses are trained in universities by highly qualified teachers and have the opportunity to increase both their Bachelor's and Master's degrees, and also have opportunities for specialization in different fields. The aim of the study is to investigate student satisfaction by quality of training in subject \"Healthcare Management\". The study is based on the use of a direct individual survey and covers 200 students. The respondents are between 19 and 55 years old and most of them have professional experience to five years. The analysis of the data from the survey is aimed at the student satisfaction with the learning process and the possibilities for professional realization. A significant part of the students considers the theoretical and practical training as current and applicable for their realization in the healthcare system. This is a sign of the quality of work in the university and a prerequisite for creating managers with the necessary knowledge and skills to manage quality health care. Continuous increase in quality requirements for health services requires a continuous development of learning in conformity with European perceptions. Measures to improve the quality of training should be aimed at enhancing student satisfaction because this determines their motivation to participate in the training and realization in the healthcare system. The motivation of students in the learning process is largely related to their confidence in the applicability of the knowledge obtained after graduation. An important condition for the professional realization of the students is the working environment and the organization of the work in the healthcare establishments. Atrial fibrillation (AF) is the most prevalent arrhythmia in clinical practice. There is increasing evidence for the role of inflammation in the development and maintenance of AF and gout is associated with inflammation and oxidative stress. In the last years several studies were published to assess the role of gout as a risk factor for occurrence of atrial fibrillation (AF). There are not enough data on the importance of gout in patients, who already have AF. The aim of the study is to assess the impact of gout on the clinical course of AF. Overall 101 patients – 51 females and 50 males at mean age 68,02 ± 7,001, with AF after sinus rhythm restoration were included in a clinical trial of one-year placebo-controlled treatment with spironolactone. Gout was reported in 6,8% of them. They were analyzed for AF recurrence, hospitalization for AF, all-cause admissions, composite endpoint (recurrence episodes of AF, all-cause hospitalization and death) and value of biomarker Galectin-3 (Gal-3). Results: Patients with gout had double risk of recurrence of AF, even though not significant, HR=1.97, 95%CI=0,78-4.98, p=0,15. In our study presence of gout was significant predictor for hospitalization for AF in unifactor analysis (HR 4,46, 95% CI=1.51 – 13.19, p=0,007) and the only significant in multifactor analysis – model, including gender, age categories, hypertension, diabetes and use of spironolactone (HR=4,23, 95%CI=1,28–14,1, p=0,018). Gout influenced significant also the all-cause hospitalizations, HR =3.17, 95%CI 1.10-9.14, p=0.033. There was a significant difference between the value of Gal-3 in patients with gout as opposed to patients without (28,52±15 vs 16,02±5,49, р=0,002). Conclusion: We found that gout significantly influences the course of AF. Presence of gout in patients with atrial fibrillation is a risk factor for recurrence and hospitalization – cause-specific for AF and all-cause. The list of symptoms is varied and very impressive, but the real problems posed for the study require a specialized approach to diagnosing the main causes of its occurrence and determines the presence of a new type of disease which described by the WHO in 2002 as \"Electromagnetic Hypersensitivity Syndrome - EHS\". Our attention as researchers focuses on the capabilities of geopathology (geobiology) to identify the causes of geopathic stress and to describe solutions for its neutralization. The aim of presented study was to create specialized tourism destination in pure nature for wellbeing and prevention of geopathic stress and electrosmog impact. The goal is to test the geomedical tourism as a special approach and problem solving. Objects of research: 16 famous Wellness and SPA destinations from 4 countries: Bansko, Sunny Beach, Pomorie (Bulgaria), Sulzberg- Bregenz (Austria), Ochrid (N Macedonia), Bad Griesbach (Germany) which were examined in 2010-2017. Methods: Test of differentiated self-assessment of the functional state (SAN), Applied Kinesiology Test, Geopathic stress Sequences (GSAS) and geomedical assessment of environment. Results: the core of Geomedical tourism destination management is a geomedical environment assessment of Climate recreation potential, Forest recreation potential- herbs, woods, lichens, Landscape recreation assessment, Environment assessment, Geopathic stress evaluation, Geological places assessment, Biomedical assessment of natural resources and Wellbeing effect. Some outlined benefits of outdoor recreation including post treatment geopathic stress analysis and Applied Kinesiology test-AK. Their positive results we can apply as an additional key for Geomedical tourism destination’s modelling. Our results from SAN test before and after treatment are very motivated to go over it. Conslusion. Geomedical tourism as a live style coaching including outdoor physical exercises, anti-stress massages, special nutrition menu, breathing treatment, forest meditation and geotherapy. Obviously, all forms of proactive and reactive healthcare including geomedical practices are part of health prevention tourism. Treatment with extraction of molars is more complicated because the space that needs to be closed is larger than the premolar spaces and the time of treatment will be longer. However, in cases where we have a lack of space, teeth protrusion or dental asymmetry, and the extraction of permanent teeth is indicated, the damaged molars should be the first option for extraction when premolars are in better condition. The third molars in different ways are affected by orthodontic therapy. One of them is orthodontic treatment by applying the extraction of the third molars, this situation being widely explored. Extraction of third molar for orthodontic reasons is justified when it prevents the eruption of the second molar or otherwise affects the other teeth. The indications of extraction and selection of teeth to be removed depend on the specific aspects of malocclusion. Premolars are teeth that are usually extracted from orthodontic reasons, but this is not the rule and depends on individual treatment needs. The decision to correct the malocclusion with teeth extraction should be based on the ratio of the size of the dental arch with the size of the teeth, facial appearance and other factors arising from the diagnosis itself. Indications for extraction of the third molars during orthodontic treatment are: mandibular incisor crowding, if they in some way constitute a obstruction of orthodontic treatment as well as preparation for orthognathic surgery. The success of an orthodontic treatment involving the extraction of molars is directly related to the patient complaints and psychological profile, the absence of diseases and skeletal discrepancies, the professional experience as well as the fact whether the patient underwent previous orthodontic treatment with extraction. Patients with partially or completely non-erupted teeth are significantly more likely to have certain consequences: before, during or after the orthodontic treatment. It is essential that the period from the onset of the trauma to the beginning and method of treatment. Depending on timely and adequate medical care for victims with burns, possible complications can be significantly reduced and prevented. According to the normative requirements for acquiring professional competence, health care professionals must possess special training and education in the scope of the specialty \"Plastic and Reconstructive Surgery\" (PVEH). The purpose of this study is to study awareness of the burns and the need to train healthcare professionals from outpatient care. The study was conducted in the period March-April 2019 year among 134 health care professionals working in outpatient clinics for specialized medical care (individual or group practice), medical centers and diagnostic advisory centers on the territory of Sofia City. The study used the dokumen¬tal and sociological method through its own survey. Results. The highest relative share is the health care professionals with work experience from 21 to 30 years (38.06%) from 11 to 20 years (34.33%). The analysis of the results showed that over half (55.97%) Professionals can partly cope with a burning incident. Every fifth of respondents (20.15%) Professional preparation for medical care and 23.88% of them respond negatively. A significant share of respondents (94.77%) Want to participate in training on the topic of burns 67.16% of them giving a categorical answer \"yes, completely” , 27.61% say \"yes, partly\". 98.05% of professionals consider it necessary to provide information to parents about the possible risk of incineration in children, with 91.79% expected to contribute to reducing incidents in their rearing. Conclusion. In recent years, health care reforms have been oriented towards improving the quality of medical care, as the main task of medical institutions to provide affordable health care. The overlapping of pharmacokinetics and/or the pharmacodynamics of medicines causes the occurrence of overlapping clinical syndromes and diagnostic issues, potentiated in overdoses. We report a case of severe venlafaxine poisoning where the clinical presentation and the results of rapid immunoassay test overlapped with tramadol intoxication.\u0000Case presentation. An unconscious women with recurrent seizers, hypertension and supposed acute medication poisoning in suicidal attempt was transported to our clinic. Previously, she had been lavaged, rehydrated and treated with 20 mg diazepam iv, 40 mg furosemide at the local general hospital. Her regular tablet therapy consisted of losartan, levothyroxine, venlafaxine, occasionally tramadol.\u0000At admission she was comatose, with isochoric normal pupils, BP 130/80 mm Hg, SaO2 86%, and recurrent episodes of seizures treated with 10mg diazepam iv, ocular clonus, hypertonus, temperature 38.9C, diaphoresis, facial hyperaemia, dark coloured urine, hyponatremia and rhabdomyolisis. The lateral flow immunoassay (AbuGnostR) was positive for tramadol, but the homogeneous enzyme immunoassay did not confirm it. After 36 hours of intensive treatment she became somnolent and reported ingestion of 2250 mg tbl Venlafaxine. The AbuGnost R test detects tramadol at cut off urine values 200ng/ml, but present cross reactivity with O-desmethyl-venlafaxine at cut off values up to 25000ng/ml. The following days she complained of muscular weakness, headaches and cognitive impairment, which lasted for more then one month after release from hospital.\u0000Conclusion. High concentrations of venlafaxine metabolites induce false positive tramadol immunoassay (AbuGnostR) test. Overlapping clinical presentations and metabolic pathways of venlafaxine and tramadol should alert physicians when interpret rapid immunoassay test.
