Furthermore, the use of clozapine in treatment-resistant schizophrenia has been thought to induce or aggravate these disorders. To date, there is a paucity of research regarding the prevalence and associated factors. Hence, this study aims to report the prevalence of OCS and OCD, and examine potential risk factors, in clozapine-treated schizophrenia.</div></div><div><h3>Methods</h3><div>This is a cross-sectional study conducted in the only tertiary hospital for psychiatric patients in Singapore. In total, 162 patients on a stable dose of clozapine were recruited for this study; 159 patients with a diagnosis of schizophrenia or schizoaffective disorder were included in the current analysis. Sociodemographic, clinical and treatment factors were analysed to identify factors associated with OCS and OCD.</div></div><div><h3>Results</h3><div>The prevalence of OCS and OCD is 21.4% and 12.6% respectively. Factors associated with OCS include younger age (OR:0.96, <em>p</em> <!-->=<!--> <!-->0.033) and younger age of onset of psychosis (OR:0.92, <em>p</em> <!-->=<!--> <!-->0.017). There were no significant factors associated with OCD. However, in an analysis of both OCS and/or OCD, factors associated include younger age (OR:0.96, <em>p</em> <!-->=<!--> <!-->0.027) and younger age of onset of psychosis (OR:0.91, <em>p</em> <!-->=<!--> <!-->0.016). Severity of psychotic illness and Clozapine dose were not associated with OCS or OCD in clozapine-treated schizophrenia.</div></div><div><h3>Discussion & conclusions</h3><div>Our results suggest a high prevalence of OCS and OCD in clozapine-treated schizophrenia which clinicians should routinely screen for. As it was reported in previous studies, patients with preserved executive functions respond better to pharmacological treatment, while others need to keep trying different pharmacological strategies.</div></div><div><h3>Material and methods</h3><div>In this work we used machine learning techniques to predict pharmacological response (OCD patients’ symptomatology reduction) based on executive functioning and clinical variables. Among those variables we used anxiety, depression and obsessive-compulsive symptoms scores by applying <em>State-Trait Anxiety Inventory</em>, <em>Hamilton Depression Rating Scale</em> and <em>Yale-Brown Obsessive Compulsive Scale</em> respectively, while <em>Rey-Osterrieth Complex Figure Test</em> was used to assess organisation skills and non-verbal memory; Digits’ subtests from <em>Wechsler Adult Intelligence Scale-IV</em> were used to assess short-term memory and working memory; and <em>Raven's Progressive Matrices</em> were applied to assess problem solving and abstract reasoning.</div></div><div><h3>Results</h3><div>As a result of our analyses, we created a reliable algorithm that predicts Y-BOCS score after 12 weeks based on patients’ clinical characteristics (sex at birth, age, pharmacological strategy, depressive and obsessive-compulsive symptoms, years passed since diagnostic and <em>Raven's Progressive Matrices</em> score) and Digits’ scores. A high correlation (0.846) was achieved in predicted and true values.</div></div><div><h3>Conclusions</h3><div>The present study proves the viability to predict if a patient would respond or not to a certain pharmacological strategy with high reliability based on sociodemographics, clinical variables and cognitive functions as short-term memory and working memory. However, these recommendations are mainly based on cross-sectional evidence. The preregistered Repeated Assessment of Behaviors and Symptoms in the Population (RABSYPO) study sought to establish prospective longitudinal evidence from a cohort with a demographic distribution similar to that of the Spanish population to provide evidence for developing solid universal recommendations to reduce anxiety and depressive symptoms during times of uncertainty.</div></div><div><h3>Material and methods</h3><div>We first recruited via social networks a pool of Spanish individuals willing to participate and then randomly selected some within each stratum of age<!--> <!-->×<!--> <!-->gender<!--> <!-->×<!--> <!-->region<!--> <!-->×<!--> <!-->urbanicity to conduct a one-year-long bi-weekly online follow-up about the frequency of ten simple potential coping behaviors as well as anxiety (GAD-7) and depressive symptoms (PHQ-9). Mixed-effects autoregressive moving average models were used to analyze the relationship between past behaviors’ frequency and subsequent symptom changes across the twenty-seven time points.</div></div><div><h3>Results</h3><div>Among the 1049 who started the follow-up, 942 completed it and were included in the analyses. Avoiding excessive exposure to distressing news and maintaining a healthy/balanced diet, followed by spending time outdoors and physical exercise, were the coping behaviors most strongly associated with short and long-term reductions of anxiety and depressive symptoms. Engaging in relaxing activities and drinking water to hydrate were only associated with short-term symptom reductions. Socializing was associated with symptom reductions in the long term.</div></div><div><h3>Conclusions</h3><div>This study provides compelling prospective evidence that adopting a set of simple coping behaviors is associated with small but significant reductions in anxiety and depressive symptoms during times of uncertainty. Patients were digitally monitored for objective assessment of behavioral changes.</div></div><div><h3>Results</h3><div>After one year of its initiation, DBS of the nucleus accumbens (in subjects N2, N3, and N4) and subgenual anterior cingulate cortex (in N1) produced a significant clinical improvement, associated with decreases in the Clinical Global Impression (from 5.25<!--> <!-->±<!--> <!-->0.5 to 3.5<!--> <!-->±<!--> <!-->1, <em>p</em> <!-->=<!--> <!-->0.035) and in the Hamilton Depression Rating Scale (HADRS scores, from 14.5<!--> <!-->±<!--> <!-->6.56 to 1.5<!--> <!-->±<!--> <!-->1.29, <em>p</em> <!-->=<!--> <!-->0.020). We observed a notable, durable therapeutic response in two patients from this cohort (N1 and N3), a clinically relevant relief in a third (N2), and a lack of a significant response in the last one (N4). Maintenance electroconvulsive therapy sessions could be discontinued in the three patients that responded to DBS (N1–3). There were no side effects or relevant changes in cognitive functioning. There were relevant differences between physical activity and sleep time among the four participants.</div></div><div><h3>Conclusions</h3><div>These results suggest initial evidence that DBS may be an effective and safe alternative for treating complex and resistant forms of schizophrenia-related disorders and BD. There is a current need to improve the detection and management of the individuals in which depression and pain coexist. Hence, the aim of this document is to provide recommendations in the diagnosis and management of patients with major depressive disorder (MDD) who have pain as a physical symptom (PPS), in order to reduce the variability of clinical practice.</div></div><div><h3>Material and methods</h3><div>The methodology used is based on the internationally recognized RAND/UCLA consensus method. The scientific committee, consisted of a group of eight multidisciplinary experts, defined 12 clinically relevant questions. After the systematic review of the literature, the scientific committee assessed the evidence and developed recommendations. The panel group with 15 participants validated these recommendations using a single Delphi round. To conclude, there was a final consensus meeting held to redefine with minor modifications the final recommendations.</div></div><div><h3>Results</h3><div>The scientific committee developed a total of 19 recommendations on the diagnosis and detection, impact of PPS in MDD, treatment of MDD with associated PPS, use of healthcare resources, additional recommendations, and care coordination of these patients. Globally, a substantial level of agreement (≥80%) was reached on all items during the Delphi round. All the 19 achieved consensus, seven of them (37%) were agreed with unanimity during the Delphi round.
