Methods Totally 80 stroke patients were assigned to four groups according to random digit table, i.e., the routine rehabilitation group, the SEA group, the CIMT group, and the comprehensive intervention group. Patients in the routine rehabilitation group strengthened the training of upper limbs on the affected side by Bobath dominated technology and Brunnstrom assisted technology. Patients in the SEA group received Jiao's SEA combined EA therapy. Those in the CIMT group restricted the upper limbs of the healthy side and strengthened training of the affected side. Those in the comprehensive intervention group used SEA combined CIMT treatment. Fugl-Meyer assessment scale (FMA) , grading of hand function and range of wrist movement were observed before intervention, at week 4 and 12 after intervention, respectively. Results Compared with before treatment in the same group, FMA scores of upper limbs significantly increased, grading of hand function, and range of wrist movement were obviously improved in the 4 groups after 4-week treatment (P <0. 05, P <0. 01). There was no statistical difference in FMA scores of upper limbs or grading of hand function among the four groups. But dorsal expansion of wrist and radial deviation were more obviously improved in the comprehensive intervention group than in the routine rehabilitation group (P <0. 05). Compared with the routine rehabilitation group, FMA scores of up- per limbs increased, grading of hand function and range of wrist movement were obviously improved in the comprehensive intervention group (P <0. 05). Conclusions Routine rehabilitation, SEA, and CIMT showed better rehabilitation effect on movement function of ischemic stroke patients' upper limbs. {"title":"[Regulatory Effect of Zishen Yutai Pill on the Biological Behavior of Oocytes in Immune Infertility Model Rats].","authors":"Xiao-Li Zhang, Xiao-Yong Zhao","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Objective To observe the intervention effect of Zishen Yutai Pill (ZYP) on the oo- cytes of infertile rats with positive anti-sperm antibody (AsAb). Methods AsAb positive immune infertil- ity model was established using active immunity. Totally 100 female SD rats were randomly divided into the blank control group, the model group, low, middle, and high dose ZYP groups, 20 in each group. Dif- ferent medications were administered to rats in corresponding groups by gastrogavage from the 22nd day of modeling to the 36th day, once per day. Normal saline (5 mL/kg) was administered to rats in the model group by gastrogavage. ZYP at 0. 075, 0.100, 0.150 g/mL was respectively administered to rats in low, middle, and high dose ZYP groups. AsAb and transforming growth factor β (TGF-β) levels were meas- ured by ELISA. Oocytes were collected after rats were sacrificed. mRNA and protein expression levels were detected using Real-time quantitative polymerase chain reaction ( RT-PCR) and Western blot. Oo- cyte apoptosis rate was determined by flow cytometry (FCM) and transferase-mediated deoxyuridine triphosphate-biotin nick end labeling (TUNEL). The oocyte proliferation level was detected by cell count- ing kit 8 (CCK-8) and 5-ethynyl-2'-deoxyuridine (EdU). Oodyte migration and invasion was determined u- sing invasion chamber (Transwell) and scratch experiment. Results Compared with the blank control group, the apoptosis of oocytes increased, the proliferation of oocytes was reduced, the capabilities of migration and invasion were enhanced, mRNA and protein expression levels of TGF-β were significantly down-regulated (P <0. 05). Compared with the model group, the apoptosis of oocytes was reduced, the proliferation of oocytes was increased, the capabilities of migration and invasion were enhanced, mRNA and protein expression levels of TGF-β were significantly up-regulated in the high dose ZYP group (P < 0. 05). Besides, their effects were obviously superior in the high dose ZYP group than in low and middle dose ZYP groups (P <0. 05). Methods Peripheral blood was collected from 229 ACS patients from June 2014 to March 2015. DNAs were extracted, amplified, and sequenced. Correlations between CYP2C19 *2/CYP2Cl9 *3 gene polymorphisms and clopidogrel resistance/distribution of CM syndrome were analyzed. Gene frequency and allele frequency were tested using gene counting and one-sample K-S test. Correlation between gene types and distribution of CM syndrome was tested by Pearson corre- lation test. Results (1) The CYP2C19 *2 polymorphism distribution: CYP2C19 *2(A/A) (mutant homozygous) 12 cases (5. 2%) ; CYP2C19 * 2 ( G/A ) ( mutant heterozygote ) 93 cases (40. 6%), and CYP2C19 *2 (G/G) (normal homozygous) 124 cases (54. 2%). The mutant allele frequency was 0. 255. (2) The CYP2C19 *3 polymorphism distribution: CYP2C19 *3 (A/A) 0 case (0) ; CYP2C19 *3 (G/A) 26 cases (11. 4%), and CYP2C19 *3 (G/G) 203 cases (88. 6%). The mutant allele frequency was 0. 056. (3) Correlation between CYP2C19 gene polymorphism and clopidogrel resistance: Clopidogrel resistance was more liable to occur in mutant homozygous than in mutant heterozygote and normal homozygous (R =0. 30, P <0. 01). Clopidogrel resistance was more liable to occur in mutant heterozygote than in normal homozygous (R =0. 34, P <0. 01). (4) Among the 229 patients, the CM syndrome distribution were distributed as follows. Blockage of Xin vessels syndrome (BXVS, 33 cases, 14. 41%) ; qi deficiency blood stasis syndrome (QDBSS, 51 cases, 22. 27%) ; qi stagnation blood stasis syndrome (QSBSS, 92 cases, 40.18%) ; phlegm obstructing Xin vessel syndrome (POXVS, 17 cases, 7. 42%) ; yin-cold coag- ulation syndrome (YCCS, 8 cases, 3. 49%) ; qi-yin deficiency syndrome (QYDS, 13 cases, 5.68%) ; Xin-Shen yin deficiency syndrome (XSYDS, 5 cases, 2.18%), yang and qi deficiency syndrome (YQDS, 10 cases, 4. 37%). (5) CYP2C19 *2 gene type was significantly correlated with syndrome typing of CM (R =0. 26, P <0. 01). Mutant homozygous and most mutant heterozygote patients were syndrome typed as QDBSS. Conclusions The polymorphism of CYP2C19 was closely correlated with clopidogrel resist- ance in 229 ACS patients. Its occurrence rate was correlated with CYP2C19 *2/CYP2C19 *3 gene muta- tion frequency. Blood stasis syndrome ( QSBSS, QDBSS, BXVS) were main syndromes of ACS. Be- sides, QSBSS was obviously higher than the rest syndrome types. The polymorphism of CYP2C19 * 2 was correlated with syndrome typing of CM. Methods A total of 215 infertility patients with VC were assigned to the treatment group (109 cases, treated by needle pricking therapy) and the control group (106 cases, treated by drug therapy). Patients in the treatment group received CHEN's needle pricking therapy, once per week for a total of 10 times. Those in the control group took Guizhi Fuling Cap- sule (GFC), 0. 31g/pill, 3 pills each time, three times per day for 70 successive days. The diameter of VC, whole blood apparent viscosity, and plasma viscosity value were detected in the two groups after 75- day treatment. Sperm count, sperm motility rate, sperm motility, teratopermia rate, and other parame- ters were determined to evaluate the efficacy. Routines of blood, urine, and stool were detected. Renal and liver functions were examined to evaluate the safety. Results Compared with before treatment in the same group, the diameter of VC was narrowed in grade I and II patients of the two groups after treatment (P <0.05). The diameter of VC was obviously narrower in grade II patients of the treatment group after treatment (P <0. 01). Compared with before treatment in the same group, semen quality was improved in the two groups after treatment (P <0. 05,P <0.01). Besides, sperm density was reduced, sperm motility rate was elevated, spermatozoa with normal morphology was increased with longer fluidized time in the treatment group after treatment, which were all better than those of the control group (P <0.05). Compared with before treatment, whole blood apparent viscosity and plasma viscosity value both obviously decreased in the treatment group after treatment (P <0. 05). The total effective rate was better in the treatment group than in the control group (P <0. 05). {"title":"[Chaotic Features on Excess/Deficiency Syndrome of Chronic Stable Angina Pectoris Based on Analyzing Approximate Entropy of Heart Rate Variability].","authors":"Xiao-Yu Guo, Fang-Jie Li","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Objective To analyze the chaotic degree of excess/deficiency syndrome (ES/DS) in chronic stable angina pectoris (CSAP) patients by observing 24-h dynamic changes of approximate en- tropy index of heart rate variability per hour, and to observe dynamic changing features. Methods From November 2009 to June 2011, a total of 187 CSAP patients were assigned to ES (36 cases) , DS (42 ca- ses) , intermingled syndrome of deficiency and excess (109 cases, abbreviated as intermingled ES/DS) according to TCM syndrome differentiation standards.24 h dynamic electrocardiograms were collected u- sing USA DMS dynamic ECG, which was then divided into 24 continuous period by hour. The approximate entropy algorithm (by hour) was respectively calculated. The approximate entropy diurnal variation trend was analyzed in patients with different TCM syndromes. These results were controlled with 30 healthy subjects who had normal physical examinations at the same hospital. Results The approximate entropy mean value of each hour throughout the day was lower, as compared with the healthy group (F=7. 847, P <0. 01). Although no statistical difference existed among ES, DS, intermingled ES/DS (F =1. 585, P = 0. 208), the approximate entropy mean value showed the tendency of ES >intermingled ES/DS > ES. From 8:00 a.m. to 7:00 a.m. on the next day, the variation tendency of four-group curves all showed that the approximate entropy level changed as time went by (F =2. 655, P <0. 01). Besides, the approximate en- tropy diurnal variation of ES, DS, intermingled ES/DS showed a similar trend (F =1. 011 ,P =0. 457) , but different from the healthy group (F = 1. 583, P = 0. 003). The curve in the healthy group showed \"two peaks and one valley\" type [one peak from 10:00 to 14:00, and the other peak from 22:00 to 02:00]. The curve for ES, DS, intermingled ES/DS showed an inverted dipper type [only a peak at night, and a relatively stable curve from 8:00 to 19:00]. Compared with the normal group, the day peak disappeared in CSAP patients, and the daytime approximate entropy was significantly reduced (F = 10. 315, P <0. 01). Conclusions Compared with the healthy group, the approximate entropy of CSAP patients decreased, which means the chaotic degree reduced. The adaptability of the cardiovascular system to external envi- ronment changes was weakened, which was more obviously seen at daytime than at night. Methods Totally 56 C57BL/6J male mice (18 -22 g) were recruited in this experiment. Ten were randomly selected as a sham-operation group. The rest 46 mice were used for preparing CRF model by 5/6 nephrectomization. To- tally 33 successfully modeled mice were divided into the model group, the rapamycin (RAP) group, and the KXLR group according to serum creatinine (SCr) level, 11 in each group. Mice in the RAP group were administered with rapamycin (0.13 mg/100 g per day, 0. 5 mL each time) by gastrogavage. Mice in the KXLR group were administered with KXLR (2 g/100 g per day, 0. 5 mL each time) by gastrogavage. Equal volume of distilled water was administered to mice in the model group and the sham-operation group. Mice were sacrificed after 8 weeks of consecutive medication. The expression of neutrophils was ob- served using immunohistochemical assay. Expression levels of p38MAPK/NF-κB p65 protein and TNF-α/ IL-6 mRNA were detected by Western blot and Real-time PCR. Results Compared with the sham-opera- tion group, the number of positive neutrophils increased, expression levels of p38MAPK/NF-κB p65 protein and TNF-α/IL-6 mRNA were enhanced significantly in the model group (P <0. 05, P <0. 01). Com- pared with the model group, the number of neutrophils was reduced, expression levels of p38MAPK/NF- κB p65 protein and TNF-α/IL-6 mRNA were decreased significantly in the KXLR group and the RAP group (P <0. 05, P <0. 01). RAP showed better effect in decreasing p38MAPK protein expression than KXLR (P <0. 05). There was no statistical difference in the rest indices between the KXLR group and the RAP group (P >0. 05). Conclusions KXLR participated the regulation of p38MAPK/NF-κB p65 mediated in- flammation factors. Methods FLP containing serum was prepared. A co-culture system of A549 cells and macrophages was established. A549 cells were divided into 3 groups, i.e., the blank serum group (A549 +NRS) , the co-culture cells + blank serum group (co-culture + NRS) , the co-culture cells with FLP containing serum group (co-culture + FLP). The effects of FLP on A549 cell EMT related gene (SNAIL1, SNAIL2, ZEB1, CDH1, CDH2, VIM, TJP1, CLDN1, CTNNB1, FLRT1) and proteins (E-cadherin, N-cadher- in, ZO-1, Vimentin) expressions were observed under co-culture conditions by fluorescent quantitative PCR. Results A549 cells developed into mesenchymal-like cells in co-culture conditions, which could been blocked by FLP containing serum in part. Compared with the A549 +NRS group, mRNA expressions of SNAIL1, ZEB1, CTNNB1, FLRT1, CDH2, and VIM were up-regulated (P <0. 05), but the expression of TJP1 was down-regulated in the co-culture + NRS group (all P <0. 05). Compared with the co-culture + NRS group, mRNA expressions of SNAIL2, TJP1, CLDN1, CDH1, and VIM were up-regulated, but mRNA ex- pressions of CTNNB1, FLRT1, and CDH2 were down-regulated in the co-culture + FLP group (all P <0. 05). Immunofluorescent results showed that E-cadherin expressed on cell membrane and inside cytoplasm, and most expressed on cell membrane. N-cadherin expressed on cell membrane and inside cytoplasm, and most expressed inside cytoplasm. Vimentin expressed within the cytoplasm. ZO-1 expressed mainly in cell junction. Parts of the cell membrane were positively stained. Compared with the A549 +NRS group, mRNA expression of E-cadherin was down-regulated in A549 cells, and mRNA expressions of N-cadherin and Vi- mentin were up-regulated in the co-culture +NRP group. However, E-cadherin was up-regulated and protein expressions of N-cadherin and Vimentin were down-regulated after intervention of FLP containing serum. (all P <0. 05). Methods KK-A(y) transgenic mice were fed with high fat forage to induce hyper- glycemic obese DM model. The C,7ice at same age were used as a normal control group (fed with e- qual volume of sterile water, n =11). Successful modeled 55 mice with DM obesity were divided into 5 groups by random digit table (11 in each group) , including the model group (fed with equal volume of ster- ile water, with no treatment) , the Pioglitazone Hydrochloride Tablet treatment group (8 mg/kg; as a posi- tive control group) , and JTXZT groups [high (10. 0 g crude drugs/kg) , middle (5. 0 g crude drugs/kg) and low dose (2. 5 g crude drugs/kg) ]. All medications were fed by gastrogavage, once per day for 8 succes- sive weeks. All mice were weighed and levels of random blood glucose (RBG) determined after 8 weeks of treatment. Blood was collected from ophthalmic vein. Levels of insulin (INS) , serum total cholesterol (TC) and triglyceride (TG) were detected. HOMA-IR and ISI were calculated. The morphological changes of pancreas tissues were extracted for performed pathological examinations. The expressions of INS and insulin receptor (InsR ) were measured by immunohistochemistry ( IHC ). Expressions of insulin receptorp βInsRP) and insulin receptor substrate-1 (IRS-1) in pancreas tissues were detected using Western blot. Results Compared with the normal control group, obesity, obviously increased blood glu- cose and blood lipids occurred in each group after modeling (P <0. 01). After 8 weeks of medication mice in the model group had put up body weight (P <0. 01) , blood glucose and blood lipids were kept on quite higher levels. Compared with the model group, body weight, serum levels of TG, INS, and HOMA-IR obvi- ously decreased in each JTXZT group (P <0. 05, P <0. 01). Besides, RBG decreased obviously lower in the high dose JTXZT group (P <0. 01). ISI obviously increased in low and high dose JTXZT groups (P < 0. 05, P <0. 01). Pathological results of HE staining in pancreas showed that atrophied islets with obvious- ly reduced numbers in the model group. They were sparsely distributed with reduced islet density.-Islet cells were compensatively hypertrophy, with degenerated vacuoles. Apoptosis of islet cells could also be seen in the model group, manifested as swollen cytoplasm and paryopyknosis. Islet number was obvious- ly increased in high and middle dose JTXZT groups, with reduced apoptosis and degenerated cells. Re- sults of IHC assay showed, as compared with the normal control group, the grey values of INS and InsR were significantly decreased in the model group (P <0. 01). Methods Disease location and disease nature were syndrome identified in PCOS outpatients confirmed at Department of Gynecology, Ningbo Municipal Hospital of TCM. Totally 110 patients in line with GDQSS were graded by GDQSS score from low to high. GDQSS score ranging 70 -100 belonged to grade I (mild) ; 101 -150 to grade II (moderate) ; >150 to grade III (severe). The correlation between the severity of GDQSS and TSH level was an- alyzed using Pearson and Partial correlation analyses. Results Levels of TSH, testosterone (T) , pro- lactin (PRL) were the lowest in grade I GDQSS patients, and they were the highest in grade lIl GDQSS patients (P <0. 01). Pearson correlation analysis showed that GDQSS score was positively correlated with TSH level (r =0. 676, P <0. 01). They were still positively correlated by using Partial correlation after controlling body mass index (BMI) , waist hip ratio (WHR) , homeostasis model of assessment-insulin resistance (HOMR-IR) factors (r =0. 663, P <0. 01). {"title":"[Treatment of Chronic Heart Failure with Qi-Yang Deficiency, Blockage of Static Blood Syndrome by Xinmailong Injection: an Open Multi-center Clinical Study].","authors":"Cheng-Zeng Yao, Xiao-Long Wang","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Objective To assess the safety and effectiveness of Xinmailong Injection (XI) for treating chronic heart failure (CHF) patients with qi-yang deficiency, blockage of static blood syndrome (QYDBSBS). Methods Totally 2 104 patients with QYDBSBS at 25 centers were recruited. XI at 5 mg/kg was intravenously injected to all patients after skin test, twice per day for 5 successive days. The safety and effectiveness were observed within 8 days after injection (5 days of treatment and 3 days of follow- ups). Results were analyzed after full analysis set (FAS) and safety set (SS). Efficacy of heart function (HF) and efficacy of Chinese medicine (CM) syndrome were subgroup analyzed by age ( ≤65 years old or >65 years old) or types of HF (whole HF, right HF, or left HF). Results FAS analysis showed after treatment markedly effective of HF in 550 cases, effective in 873 cases, ineffective in 673 cases, deteriorated in 8 cases. The markedly effective rate was 26.14% and the total effective rate was 67.63%. After treatment markedly effective of CM syndrome was shown in 795 cases, effective in 1 009 cases, ineffective in 288 cases, deteriorated in 12 cases. The markedly effective rate was 37.79% and the total effec- tive rate was 85.74%. Results of CM symptom score: CM symptom score was (11.381 ±4.574) before treatment and (4.987 ±3.984) after treatment, with statistical difference (P <0.01). The decrease rate was 56.8% ±27.0%. Results of subgroup analysis showed that markedly effective rates of HF efficacy and CM syndrome efficacy were superior in patients ≤65 years old than in patients >65 years old (P<0.01 Efficacy of HF was superior in patients with left HF than in those with whole HF or with right HF (P<0.01). In aspect of efficacy of CM, the markedly effective rate and the total effective rate were superior in patients with left HF or with whole HF than in those with right HF (P<0.01 ). No death occurred during the tri- al. No severe adverse event occurred either. Conclusions XI could improve clinical symptoms of CHF patients with QYDBSBS, reduce CM symptom score.
