Un des enjeux est d’identifier les prises en charge les plus adaptées en fonction du stade de la maladie et de préciser la place des différents acteurs. Cette revue systématique de la littérature propose d’établir un panorama des méthodes de prise en charge en soins primaires ayant montré leur efficacité dans les cas de symptomatologie dépressive (syndromique et sub-syndromique). Les bases de données PubMed, Cochrane, PsycInfo/PsycArticles et Cairn, ont été interrogées entre 2006 et 2016. Sur la base de 591 articles correspondant à l’algorithme de recherche, 27 articles dont 23 essais cliniques et 4 revues de la littérature ont été intégrés à l’analyse. Nos résultats suggèrent qu’il existe trois types de dispositifs efficaces : les thérapies guidées, non-guidées et le suivi par entretien téléphonique. La majorité d’entre eux reposent sur les principes des thérapies cognitivocomportementales. Au regard des enjeux cliniques et économiques de la dépression, cet article cible des recommandations concernant la prise en charge de première intention de la symptomatologie dépressive chez l’adulte.</p></div><div><p>Currently, depression is a real public health problem associated with very high healthcare costs and a significant impact on the patient's daily life and their entourage. One of the challenges is to identify psychological support devices according to the stage of the disorder and to also specify the role of different actors. This systematic literature review aims to establish an overview of the effective methods of intervention in primary care in cases of depressive symptoms (subsyndromal and syndromal depression). PubMed, Cochrane, PsycInfo/PsycArticles and Cairn were searched for articles published between 2006 and 2016 containing the following words in the title according to the algorithm: [Health promotion (title)] OR [prevention (title)] OR [primary care (title)] AND [depression (title)]. Based on 591 items matching the search algorithm, 27 items including 23 clinical trials and 4 literature reviews were included in this analysis. The results suggest that there are three types of effective prevention devices: guided therapies, self-help therapies, and monitoring by telephone interview. The majority of them are based on the principles of cognitive behavioral therapy. Guided therapies take place in direct interaction with a therapist who is physically present by videoconference or phone. Effective guided therapies are cognitive behavioral therapy (CBT), problem-solving therapy (PST), and psychoeducation. Self-help therapies are standardized psychological interventions based on the total or partial autonomy of the patient. Cent trente-cinq participants ont complété trois questionnaires évaluant : (1) les quinze schémas précoces inadaptés de Young (assujettissement, exigences élevées, manque d’autocontrôle, sacrifice de soi, etc.) ; (2) la triade workaholique de Spence et Robbins composée de l’implication au travail, la tendance compulsive à travailler et la satisfaction au travail et (3) les deux composantes de l’échelle Dutch Work Addiction Scale (travailler excessivement, travailler compulsivement). Nous avons comparé l’activation des schémas précoces inadaptés dans trois groupes (non-activisme, activisme enthousiaste et activisme non enthousiaste). Deux schémas précoces inadaptés (abus–méfiance, exigences élevées) sont significativement plus activés dans l’activisme que dans le non-activisme et le schéma manque d’autocontrôle est significativement plus activé dans l’activisme non enthousiaste que dans l’activisme enthousiaste. L’activation différentielle de schémas précoces inadaptés dans les deux formes d’activisme professionnel a des implications thérapeutiques cognitives qui sont discutées.</p></div><div><h3>Introduction</h3><p>This study explores the relationship between early maladaptive schemas identified by Young (2005) and workaholism according to Spence and Robbins (1992). These authors differentiate four non-workaholic employee profiles (satisfied, unengaged, disenchanted and relaxed professionals) and two workaholic profiles (enthusiasts and non enthusiasts). According to Spence and Robbins, the only true workaholics are non-enthusiastic workaholics whose satisfaction scores are low, contrary to enthusiastic workaholics who obtain high work enjoyment scores. However, these two workaholic profiles share high work involvement and a high drive to work. This research aims to compare the activation of schemas in different professional profiles (non-workaholics, enthusiastic workaholics, non enthusiastic workaholics) for the purpose of completing current data, which agrees on the implication of excellence and performance beliefs in workaholics without distinguishing the enthusiast profile from the non-enthusiast. Even if the last years have been characterized by a growing interest for the use of CBIs in correctional settings, a lack of training in and of knowledge of CBIs, and how to use them, represents a major issue. Specifically, while correctional professionals are rarely trained in CBIs, healthcare professionals working in correctional settings are not properly trained in issues pertaining to the prevention of recidivism. Since this situation creates the risk of inappropriate professional practices, the involvement of well-trained professionals is crucial. L’objectif de cet article est de montrer la place des thérapies comportementales et cognitives dans le traitement des troubles schizophréniques. Deux patients souffrant de schizophrénie du centre national hospitalier et universitaire de psychiatrie (CNHUP) de Cotonou ont bénéficié de la technique d’entraînement aux habilités sociales dans le but d’acquérir des compétences afin de mieux gérer au quotidien leur traitement, leur équilibre familial et leurs relations interpersonnelles. Les modules d’entraînement proposés concernaient l’hygiène corporelle, les habiletés de conversation et de loisir, et l’éducation au traitement neuroleptique. Ce type d’intervention a permis d’accroître les compétences cognitives et sociales de ces patients. Les durées d’hospitalisation étaient réduites, la fréquence des hospitalisations diminuée et les capacités d’adaptation socio-familiale améliorées. L’efficacité des programmes de réhabilitation des états psychotiques chroniques est signalée par plusieurs auteurs soit en monothérapie, soit en association avec la chimiothérapie. La variabilité des résultats obtenus découle de divers facteurs. Les thérapies comportementales et cognitives permettent d’élargir les possibilités thérapeutiques proposées aux patients souffrant de schizophrénie. Leur mise en œuvre présente un intérêt particulier dans la réhabilitation psychosociale des patients souffrant de psychose chronique.</p></div><div><h3>Introduction</h3><p>Chemotherapy alone is rarely sufficient in the treatment of chronic psychoses and recently there has been growing interest in the rehabilitation of the patient and their family. However, no previous research on the psychosocial rehabilitation of patients suffering from schizophrenia has been carried out in Benin.</p></div><div><h3>Aim</h3><p>The aim of this study was to highlight the importance of psychosocial rehabilitation cognitive-behavioral techniques in the treatment of patients suffering from schizophrenia.</p></div><div><h3>Method</h3><p>Two patients from the Centre national hospitalier et universitaire de psychiatrie (National University Hospital Centre for Psychiatry) in Cotonou suffering from schizophrenia were offered psychosocial skills training with the objective of acquiring rehabilitation skills. The program was preceded by motivational interviews based essentially on open questions, reflective listening and summaries. Discussions focused on the advantages of such a program and the secondary benefits of taking responsibility for their own care. The importance of making changes as well as the ability to do so were also considered. A therapeutic contract allowed patients and therapists to commit to and respect each stage of the programme. Un cas clinique est présenté. Des évaluations quantitatives à intervalles espacés ont été réalisées avant la première séance, après la dernière séance et en suivi à trois puis six mois, avec l’échelle d’ESPT (PCLS), l’échelle modifiée des symptômes traumatiques, le questionnaire d’expériences dissociatives, mais également à l’aide de l’échelle HADS pour l’évaluation de l’anxiété et de la dépression et du questionnaire de qualité de vie FACT-B. Des retranscriptions des séances ainsi que des mesures à intervalles rapprochés (nombre d’évitement, nombre de reviviscences et intensité de l’anxiété par semaine) complètent ces évaluations. À travers la diminution de l’ensemble des mesures quantitatives effectuées, mais également par les changements cliniques perçus dans les propos de la patiente, la thérapie EMDR prétend à une efficacité dans le traitement d’un ESPT après cancer du sein.</p></div><div><p>The aim of this paper is to assess benefits of eye movement desensibilization and reprocessing (EMDR) therapy within treatment of post-traumatic stress disorder (PTSD) that may occur as a consequence of breast cancer. Indeed, scientific community attests to the powerful traumatic effect of cancer on human beings (APA, 2000; Brennsthul et al., 2015). Cancer gives rise to feelings of endangerment of one's life, of one life's quality and also that of psychological and physical integrity of the concerned, ill person. Cancer induces vulnerability, loss of control and feelings of helplessness. Moreover, the disease or the chirurgical intervention-related context may reactivate previously suffered psychological traumas and such trauma-related memories then may play a role in the perseverance of PTSD symptoms within the population of patients suffering from breast cancer. Several publications have pointed to the efficacy of EMDR within the domain of PTSD psychotherapy (Tarquinio, 2007), namely in the treatment of several psychopathologies (Shapiro, 1995; 2001; 2002; De Jongh et al., 1999). For instance, EMDR helped patients suffering from cancer to reduce their PTSD symptoms and more specifically the intrusive symptoms (Capezzani et al., 2013). Simply put, pains and other corporal perceptions may – due to their associations – recall the hard times related to the disease or its treatments. It is in this vein that EMDR is supposed to be efficient in PTSD treatment when applied to the phase that immediately follows after breast cancer.
