Vingt-trois sujets sont évalués par questionnaires mesurant la PCD, l’anxiété générale et situationnelle, les symptômes dépressifs, les émotions, et le plaisir perçu. Une analyse de cluster classifie les individus selon deux profils : un groupe appelé « Mindful » (<em>n</em> <!-->=<!--> <!-->16), et un groupe appelé « Unmindful » (<em>n</em> <!-->=<!--> <!-->7). De fortes disparités sont présentes au niveau des scores d’anxiété et de dépression. Le groupe « Unmindful » montre des scores d’anxiété deux fois plus élevés, ainsi qu’un score de dépression quatre fois plus élevé. Les scores d’émotions positives, négatives et de plaisir perçu de « Mindful » sont supérieurs à « Unmindful ». Étonnamment, le groupe « Mindful » rapporte un score plus élevé d’émotions négatives. Une explication serait que la boxe pourrait activer des émotions telles que la peur ou la colère, d’ordinaire perçues comme négatives, mais qui seraient fondamentales dans la pratique. Une autre possibilité serait que les adolescents « Mindful » acceptent davantage leurs émotions, alors que ceux présentant un bas niveau de PCD seraient enclins à les réprimer. Ainsi, cette étude identifie un lien entre PCD et bien-être émotionnel chez des jeunes boxeurs compétiteurs.</p></div><div><p>This study aimed to assess the links between dispositional mindfulness and the indicators of emotional well-being in non-clinical teenagers practicing competitive boxing. Twenty-three individuals completed questionnaires assessing their dispositional mindfulness, general and situational anxiety, depressive symptoms as well as emotions and perceived pleasure during boxing. A cluster analysis was then performed, based on the scores of dispositional mindfulness, anxiety and depression symptoms. This allowed to classify individuals into two groups: a first cluster called “Mindful”, representing 70% of the participants, and a second called “Unmindful”, representing 30%. These groups can be distinguished from each other by an important difference of the dispositional mindfulness score, the “Mindful” group displaying a mindfulness level higher than the “Unmindful” by more than twice the value of the standard deviation. In addition, these 2 clusters showed important differences in terms of levels of anxiety and depressive symptoms. “Unmindful” showed an anxiety score twice higher and a level of depressive symptoms 4 times more elevated than “Mindful”. In addition, “Mindful” displayed situational anxiety scores (during training or competition) much lower than “Unmindful”. Furthermore, the levels of positive or negative emotions and of perceived pleasure of “Mindful” were higher than in “Unmindful”. Il est intéressant de constater que ces BAS peuvent être modifiés à l’aide de procédures de modifications de biais cognitifs (CBM-A). Malgré son intérêt, la documentation relative à l’évaluation et à la modification des BAS dans le cadre de l’API reste, à ce jour, sporadique et controversée. Les objectifs principaux du présent article sont d’examiner de manière systématique la récente littérature traitant des BAS et de leur modification dans le cadre de l’API. Cette revue nous permettra de dresser les implications cliniques pour des futures campagnes de prévention de l’API basées sur cette technologie. Une nouvelle plateforme informatique (MoBiCo) destinée à la modification des BAS pour des images associées à l’alcool sera présentée. MoBiCo permet d’effectuer un ensemble prédéfini de sessions d’entraînement à la CBM-A délivré sur internet, de manière totalement automatisée et indépendante. L’offre proposée par MoBiCo est bien acceptée par les utilisateurs, facilement diffusable et s’adapte à de nombreux stimuli ou besoins préventifs. Progressivement, MoBiCo devrait compléter l’éventail des ressources disponibles pour une campagne de prévention théorique et empirique chez les jeunes adultes.</p></div><div><p>In Switzerland, more than 30% of young adults between 15 and 19 years of age practice binge drinking (BD) at least once a month. BD refers to a high level of alcohol consumption over a short period of time with a “binge and crash” pattern of consumption that alternate phases of acute poisoning with phases of abstinence. BD is often associated with risky behaviors, such as reckless driving, physical or verbal abuse, but also with mild cognitive impairment. The great majority of the BD prevention campaigns developed to date has mainly been built without any theoretical background explaining attitudes and behavioral modifications. Moreover, these prevention campaigns have barely been validated empirically. The most recent cognitive models of BD suggest that, like in alcohol dependence, BD may be associated with attentional bias (BAS) for appetitive information related to alcohol. Interestingly, it has been shown that BAS can successfully be modified with well-defined computerized tasks such as the one based on the attentional bias modification procedure (CBM-A). Despite its interest, the literature concerning BAS assessment and modification in BD remain sporadic and controversial. Quatre étapes sont importantes dans cette démarche TCC : l’évaluation, l’alliance thérapeutique, l’analyse fonctionnelle par la méthode des cercles vicieux ou l’utilisation d’une grille SECCA, dont vont découler les méthodes comportementales et/ou cognitives. Cinq techniques sont par ailleurs possibles pour augmenter la motivation, dont notamment la balance décisionnelle. Nous présentons dans ce cas clinique différentes séances d’une patiente qui a souhaité stopper son tabagisme par une approche en TCC. Nous présentons l’histoire de sa consommation de tabac, une analyse fonctionnelle de type SECCA, les évaluations réalisées au démarrage de la thérapie et leur évolution au cours de la thérapie, quelques séances spécifiques de thérapie avec le recours en particulier à la balance décisionnelle, une approche cognitive sur les « je dois » et « il faut », les colonnes de Beck avec distorsion cognitive, la reprise de croyance sur le tabac, les cercles vicieux et les cercles vertueux. L’approche a permis d’obtenir un sevrage en douceur. Nous discutons dans un dernier temps les intérêts des TCC et des produits pharmacologiques associées aux TCC dans le sevrage tabagique.</p></div><div><p>Behavioural and cognitive therapies (CBT) are validated in smoking cessation but are not sufficiently used in current clinical practice. Four steps are important in this CBT process: (1) psychometric assessment (before, during and after therapy); (2) therapeutic alliance (combining empathy, authenticity, warmth and professionalism); (3) 4R interview technique: reformulate (repeat what the patient has just said, as much as possible in his own words, sometimes by formulating an hypothesis), recapitulate (at the beginning of the session, at the end of the session and for the functional analysis), recontextualize (refocus the patient on the situation, on what he feels; the questions take a turn for the worst, which also facilitates the synchronic functional analysis, starting from a specific situation), reinforce (the patient's resources, re-emphazing the progress); (4) functional analysis using the Cungi vicious circle method (i.e. situation, emotions, automatic thoughts, behaviour, concrete and relational consequences) or the use of a SECCA grid. The behavioral methods used are relaxation, stimulus control, assertiveness and problem-solving techniques. The cognitive methods include four stages: highlighting automatic thoughts through the vicious circle method or through Beck's columns, finding alternative thoughts, highlighting and modifying cognitive distortions, and working on beliefs.
