In an effort to respond to members’ needs, a second journal will be created and entirely devoted to publishing case studies. Thus, the two publications of the AFTCC will better meet the needs of both the scientific and therapeutic communities represented by its members. The current editorial retraces the recent editorial evolution of the JTCC; a French only journal that increased in popularity and quality over its first two decades of existence, then evolved into a bilingual format in year 2013. This new step towards an English-only journal represents a coherent passage in this evolution, as it will no doubt result in furthering the steadily growing quality of the publication seen over its lifespan. D’après le modèle métacognitif de Wells (1994), certaines croyances métacognitives dysfonctionnelles contribueraient au développement et au maintien des troubles mentaux. Nous disposons en effet aujourd’hui de nombreuses données attestant d’un lien significatif entre ces croyances métacognitives et la majorité des psychopathologies. Parmi ces études, certaines ont étudié spécifiquement les liens entre métacognitions et conduites addictives. Il semble que des croyances spécifiques à propos des pensées et du comportement addictif jouent un rôle dans le développement et le maintien d’une conduite problématique. Ces études ont abouti récemment à la proposition d’un modèle métacognitif des addictions. Nous proposons à travers cet article une synthèse des études sur le sujet, une présentation dudit modèle et de ses implications cliniques.</p></div><div><h3>Introduction</h3><p>Metacognitive beliefs refer to beliefs about thinking, as well as a process that controls, monitors and appraises thinking. Wells’ metacognitive model suggests that maladaptive metacognitive beliefs lead to psychological distress (Wells, Matthews, 1994). These beliefs are assessed using the Metacognition questionnaire, considered to be the gold standard for metacognition assessment. Numerous studies have shown that maladaptive metacognition is present across a large range of Axis I disorders (DSM-IV) (Sun et al., 2017). Among these studies, several were specifically interested in the links between metacognitive beliefs and addictive behaviors (Spada et al., 2014). The following article reviews empirical research, which has examined the different relationships between metacognitive beliefs and addictive behaviors, and presents the metacognitive model of addictive behaviors and its clinical implications.</p></div><div><h3>Methods</h3><p>An electronic database search of MEDLINE, PsycINFO and PsycARTICLES between 1994 and September 2017 was conducted. Only studies examining metacognition based on Wells's metacognitive model in the field of addictive behavior assessing quantitatively dysfunctional metacognition using MCQ-65 and its short form (MCQ-30), or using tools that assessed specific metacognition in addictive behavior, were included. Studies that assessed qualitatively metacognition were also included.</p></div><div><h3>Results</h3><p>The results showed that general metacognitive beliefs are elevated across addictive behaviors and that beliefs about the need to control thoughts appear to positively predict addictive behaviors (Spada et al., 2014). In addition, a series of interviews identified specific metacognitive beliefs about engaging in addictive behaviors. There are two kinds of specific beliefs. This theory was based on two hypotheses: the sequential appearance of specific affective states; shock and denial, anger, bargaining, depression, and acceptation; theses states will consistently appear in an invariable order. Data from many studies have not provided consistent support for this model; and this for at least two major reasons. Firstly, emotional states linked to grief are variable and non-specific. No emotional state is essential to define grief except strong yearning. Secondly, if they do appear, these affective states often overlap, and may subside and return spontaneously in a different order, thus refuting the notion of a stage theory. Cette adolescente a bénéficié d’une thérapie comportementale proposée par notre équipe. Ce traitement s’inspire de la thérapie comportementale dialectique (TCD) utilisée à l’origine dans le trouble de personnalité borderline (TPB). Cette thérapie postule que les patients souffrant d’un TPB ont des déficits importants en termes de compétences interpersonnelles, de régulation émotionnelle et de tolérance à la détresse. Le protocole a été adapté à la singularité de la patiente. Les stratégies thérapeutiques utilisées y sont décrites : gestion des émotions, des comportements-problèmes et leurs conséquences, analyse comportementale en chaîne, choix et mise en place des solutions. Une analyse clinique de l’évolution de la patiente est proposée, de même que des pistes de réflexion.</p></div><div><p>This article describes a clinical case presenting the treatment of a 13-year-old adolescent with disruptive mood dysregulation disorder (DMDD) associated with parasuicidal behavior. This new diagnosis, its epidemiology and the future of these patients are discussed in the first part. Disruptive mood dysregulation disorder (DMDD) is characterized by the presence of frequent temper outbursts, associated with a persistent irritable mood between outbursts. The diagnosis was created to differentiate between patients who present chronic irritability and who, up until now, received a diagnosis of bipolar disorder. Its prevalence is estimated at between 1.8 and 3.3% of the population of 9–19 year olds. There is a long-term risk of depression. For the moment there are no recommendations for the treatment of these patients. The lack of treatment for this population led to extrapolating a treatment which was already validated for parasuicidal behavior in borderline personality disorder: dialectical behavioral therapy (DBT). In fact, these disorders have the dimension of emotional dysregulation in common. This dimension results from the interaction between emotional vulnerability and a disabling environment. The effect of this treatment is proven and it is currently the most comprehensive, validated treatment for emotional regulation and the decrease of suicidal and parasuicidal behavior. It is in this context that dialectical behavior therapy (DBT) was used with an adolescent in the treatment of this behavior. A clinical case was chosen principally because the patient had severe, chronic emotional difficulties leading to parasuicidal behavior involving scarification. The DBT program appeared to be suitable to adapt to this patient. Given the size of the organization, individual interviews were used, contrary to Linehan's program which envisaged group sessions.
