In order to facilitate this surgical procedure, the nasal aspiration is an easy, effective and without risk technique. On a patient under general anaesthesia, it creates an intranasal and intrasinusal depression using a cannula connected to a surgical aspiration. This depression facilitates the separation of the sinus membrane and maintains it in high position, far from the dangers of the operative field. It highlights the need for rationalized, reproducible and reliable protocols of analysis of the results obtained by addition of these preparations. Because of the total absence of work on the PRF (Platelet-Rich Fibrin), whose action on the osseous remodelling is in theory more significant than that a simple biological adhesive containing fibrin (cPRP: concentrated Platelet-Rich Plasma), we undertook a histological study of bone biopsies obtained from a series of 9 sinus lifts carried out with freeze-dried bone allograft Phœnix from TBF<sup>®</sup>, with or without PRF. After four months of maturation, the bone tissue grafted with PRF has acquired an almost superposable architecture with that obtained after 8 months without PRF. Moreover, the quantities of newly formed bone are equivalent between the two protocols. In this third article, we were interested in the immunological features of this biomaterial. Indeed, during the production of the PRF by centrifugation, the platelets are not the only cellular elements of blood to be activated: the leucocytes will also secrete cytokines in reaction to the haemostatic and inflammatory phenomena artificially induced in the centrifuged tube. We thus undertook to measure out the concentrations of five molecules within the different parts of a PRF: three pro-inflammatory cytokines (IL-1 β, IL-6 and TNf-α), an anti-inflammatory cytokine (IL-4) and a key growth promoter of the angiogenesis (VEGF). Our data are correlated with those obtained in plasma (inactivated blood) and in sera (activated blood). Our results revealed that the PRF would have the ability to regulate the inflammatory phenomena and to stimulate immune processes by chemotactism. Its architecture remains one of most complex in higher mammalian organisms. And the anatomical lesions of the low and middle face, which are traumatic, tumoral, infectious, congenital or genetic, are extremely difficult to rehabilitate. As in the two preceding articles, the strategies of maxillo-facial reconstruction with set sat back implants imply the orthopaedic concept of leaning osseous cicatrization with external fixers. However, in such losses of substance, the postural and dynamic data become crucial. Therefore, this article presents all the parameters to control for the treatments of these extreme situations, and particularly develops the concepts of balancing pyramids. In this second article, we were interested in the platelet associated features of this biomaterial. Indeed, during the production of the PRF by centrifugation, the blood platelets are activated, which induces their massive degranulation and the release of the platelet cytokines. The function of these molecules is crucial during first stages of any phenomenon of hemostasis and wound healing. The concentrations in cytokines obtained during the production of cPRP are already well known, whatever the protocol used. We thus undertook to measure out the concentrations of these molecules within the different parts of a PRF and the comparison between our data and those from some cPRP protocols. Our results revealed that the slow fibrin polymerization during the production of PRF would allow the intrinsic incorporation of the platelet cytokines in the fibrin meshing, especially in the network of glycanic chains trapped within the fibrin fibrillae.
