Four patients received 12 Branemark Novum implants and were followed from 7 to 18 months after implant placement. The jaw was prepared to accomodate three special 4.5 mm wide implants. Precise implant positioning was accomplished with special drilling templates. After the mucosa had been sutured back into position, a prefabricated titanium lower bar was connected with titanium screws to the transmucosal implant. Another bar was then attached, and a bite registration was performed. The bridge was attached to the upper bar. The permanent reconstruction was provided to the patient the same day, about 10 h after the beginning. However, the oral cavity contains a very significant anaerobic flora: our assumption is thus that the contamination of the sinus by the anaerobic germs is unavoidable during the bone graft surgery and that this phenomenon harms largely the quality of its maturation. To validate this theory, a scan control is carried out in the first post-operative days after sinus lift: we then note the presence of gas bubbles in the graft, testifying to anaerobic germs activity. The solution suggested would be to use metronidazole (0.5%) during bone graft procedures. Our study is related to 120 patients. With the use of metronidazole, no infectious complication occurs. The tomodensitometric aspect of all the clinical cases studied in this series is absolutely identical: it is completely homogeneous. In this first article, we were interested in the key molecule of all platelet concentrates, fibrin, in order to emphasize its nature, angiogenic and cicatricial functions and its synergies with cytokines. These data enable us to evaluate the biochemical range of various technologies using fibrin as surgical additive: fibrin adhesives, cPRP (concentrated platelet rich plasma) and PRF (platelet rich fibrin). The tridimensional architecture of fibrin is indeed deeply disturbed by most clinical processes of artificial polymerization, particularly by the massive bovine thrombin addition. The set sat back maxillo-facial implants take into account these orthopaedic data to get an optimal leaning osseous cicatrisation. This article exposes the theoretical bases of the complete maxillo-mandibular rehabilitations using set sat back implants. Moreover, this study introduces the principles of an extreme implantology on an atrophic bone and it describes the management of the main associated anatomical obstacles. The platelet concentrates, as biological adhesives enriched in cytokines, were used in a multitude of clinical situations in order to evaluate their supposed effects. In ophthalmology, they were added to the surgical treatment of macular holes. In plastic surgery, they are used as fibrin adhesives in order to prevent the formation of keloid scars, which aesthetic and mechanical qualities are very prejudicial. In periodontal, peri-implant and maxillo-facial surgeries, they play the biological role of link between various elements during bone grafts or gingival surgeries and the role of protective seal in the operational site, as well as the fibrin adhesives used before. However, published works suffer from many skews and the really scientific absence of methodology to evaluate the range of it.
