Hematochezia was present in 100% of cases, in which 97.5% of the bleeding was painless. The most frequent was red or dark red hematochezia in 71.6% of cases, followed by red stool with blood dripping in 9 (11.1%) cases. The time interval between the first episode of hematochezia and the colonoscopy ranged from 1 to 47 months. Only 24.7% of polyps were diagnosed by digital rectal examination. Most of the Juvenile polyps were solitary (82.8%) and located in the rectosigmoid part of the colon. The size of the polyps varied from 0.3 to 4 cm. Early post-polypectomy bleeding occurred in two cases, while late post-polypectomy bleeding occurred in only one case.</p></div><div><h3>Conclusions</h3><p>Although solitary juvenile polyps in the rectosigmoid colon are more common, some patients had multiple and some had proximal polyps. NAs as the first line drugs effectively inhibit viral replication, whereas the suppressive effects are much weaker on HBV antigen expression. Chinese patent medicine Liuweiwuling tablet (LWWL) has been used for chronic hepatitis B (CHB) patients for more than 10 years as an adjuvant treatment. <em>Objective.</em> This study aimed to assess the efficacy and safety of LWWL combined with NAs in treating CHB by meta-analysis. <em>Methods.</em> Systematic searches for randomized controlled trials (RCT) were conducted in four databases (up to December 2020) without language restrictions. Data were analyzed using Cochrane RevMan software version 5.3.0 and Stata software version 13.1, and reported as weight mean difference (WMD) or odds ratio (OR) with 95% confidence intervals (CIs). The primary outcomes were negative conversion rate of serum HBV DNA and HBeAg. The secondary outcomes were improvement of liver function including ALT and AST, and adverse drug reactions. The quality of evidence was evaluated according to criteria from the Cochrane risk of bias. <em>Results.</em> A total of 201 potentially relevant publications were identified, and 20 RCTs were eligible for inclusion. For the negative rate of HBV DNA (OR = 2, 95%CI:1.64–2.45, <em>P</em> <0.05) and HBeAg (OR = 1.83, 95%CI:1.46–2.30, <em>P</em><0.05), there were both statically significant benefit in favor of LWWL combined with NAs. In addition, Meta-analysis also showed that LWWL combined with NAs significantly reduced ALT (WMD = −17.73, 95%CI: 21.37∼-14.09, P < 0.01), AST (MD = -23.49, 95%CI: −30.52∼-16.45, <em>P</em> < 0.05), and increased the normalization rate of ALT (OR = 3.89, 95%CI:2.82–5.36, <em>P</em> < 0.01). <em>Conclusion.</em> The negative conversion rates of HBV DNA, HBeAg, and improvement of liver function in patients treated with LWWL combined with NAs were significantly higher than those treated with NAs alone, and no serious ADRs were identified. Remarkably, he developed acute pancreatitis on the third day of his hospital stay, which was marked by a significant rise in serum lipase levels within two days following the enteritis onset, accompanied by a sustained increase in serum pancreatic enzyme levels. While the patient showed no symptoms by the fourth day, his elevated serum lipase levels persisted. Nevertheless, the common mechanism of its occurrence is still obscure, the purpose of this study is to discover the shared pathogenesis.</p></div><div><h3>Methods</h3><p>The Gene Expression Omnibus (GEO) database was used to get the gene expression data sets for CHB (GSE83148) and NAFLD (GSE89632). Following the identification of the common differentially expressed genes (DEGs), functional annotation was carried out. Moreover, protein-protein interaction (PPI) network and key sub-network were constructed. Finally, the hub gene was discovered and their transcription factors (TFs) analysis were performed.</p></div><div><h3>Results</h3><p>In the subsequent analyses, 54 common DEGs were selected. Functional analysis highlights the crucial role of immunity in these two diseases. In comparison to their younger counterparts with IBD, genetic factors play a diminished role in the development of elderly IBD, whereas immune system dysregulation and disruptions in gut microbiota assume greater significance. Among the elderly, various other prevalent conditions frequently exhibit symptoms akin to IBD, thereby rendering diagnosis notably challenging. Given that both IBD and aging process independently constitute risk factors for malnutrition, the probability of concurrent malnutrition substantially escalates within this subgroup. However, the realm of research regarding malnutrition in elderly IBD patients remains constrained. Rey","doi":"10.1016/j.gande.2023.07.006","DOIUrl":"https://doi.org/10.1016/j.gande.2023.07.006","url":null,"abstract":"<div><h3>Objectives</h3><p>The aim of this paper relies on our clinical experience in a single center with an evaluation of feasibility, clinical benefits, and safety in daily practice based on diagnosis and therapeutic yield using a newly designed Motorized Spiral Enteroscope (MSE) marked by Olympus.</p></div><div><h3>Method</h3><p>All examinations were conducted in the usual endoscopy suite with patients in the left lateral position with deep propofol sedation monitored by an anesthesiologist (bolus 2 mg/kg then 0.5 mg/kg on demand with mean total dose 300 mg (225.6–386 mg). For the anal route, MSE was performed on the X-ray table in order to monitor colonic passage and ileocecal intubation. Inflation was carried out with CO2 used.</p></div><div><h3>Results</h3><p>From June 2019 to July 2022, 204 procedures have been carried out. 188 patients were involved as repeated MSE was required due to multiple bleeding (male 111, 57.1%, female 87, 46.2%). In all cases, we considered a successful MSE when the enteroscope was deeply inserted in the ileum and achieved diagnosis or therapeutic indications as an endpoint (195 patients, 95.5%). Diagnostic yield was achieved in 134 procedures (65.7%).</p><p>Time to deep insertion was 11:36 ± 05.05 min, and the total procedure time was 22:07 ± 07:25 min. Therefore, we aimed to perform a meta-analysis to investigate and clarify the exact association between enteric dysmotility and GnRH antibodies.</p></div><div><h3>Methods</h3><p>PubMed, Web of Science, and Scopus databases were searched. The inclusion criteria were any randomized control trials or observational studies that reported the level or prevalence of GnRH IgM or GnRH IgG antibodies in patients with enteric dysmotility matched with healthy controls. Exclusion criteria: Case reports, case series, reviews, editorials, and animal studies. RevMan software 5.4 was used to perform the meta-analysis.</p></div><div><h3>Results</h3><p>After search of the literature, 4 studies were included in the meta-analysis with a total number of 469 individuals included in the study. The pooled analysis showed a statistically significant association between enteric dysmotility and increased prevalence of GnRH IgM antibodies compared with controls (RR = 4.47, 95% CI = 1.17 to 17.06, p-value = 0.03). Contrarily, no statistically significant association between enteric dysmotility and the levels of IgM antibodies as well as the prevalence of IgG antibodies was found compared with controls, (MD = 0.92, 95% CI = -1.01 to 2.86, p-value = 0.35) and (RR = 1.84, 95% CI = 0.09 to 35.86, p-value = 0.69), respectively.</p></div><div><h3>Conclusion</h3><p>Our study detected a significant association between intestinal dysmotility and high prevalence of GnRH IgM antibodies when matched with controls. Conversely, we found no statistically significant association between enteric dysmotility and IgG antibodies. LM-NBI can detect lesions overlooked by WLI. However, the effectiveness of annual examinations remains unexplored. I conducted this case-control study at a single institution. In LM-NBI group, patients with chronic gastritis underwent a LM-NBI scan, defined as the minimal magnification offering maximal visual field coverage to unveil to the microsurface pattern of gastric mucosa, of the whole stomach following WLI. Historical control was used as the conventional magnifying endoscopy (CE) group. In both groups, index endoscopy, which equalizes the status, was performed first, and subsequently annually endoscopies up to five times were conducted. The first annual examination was performed from April 2019 to March 2020 in LM-NBI group and from April 2015 to March 2016 in CE group. The detection of gastric neoplasia was analyzed according to medical records. Among 388 patients in the LM-NBI group, and 381 in the CE group, 15 and 5 patients with gastric neoplasia were respectively identified. Except for one mucosa-associated lymphoid tissue lymphoma, all were epithelial neoplasias. All endoscopic examinations were performed safely without complications which needed additional medical interventions. Through a Cox proportional hazards model, the hazard ratio of 2.78 (95% CE, 1.01–7.64) was determined. Kaplan–Meier analysis (p = 0.039, log-rank test) revealed superior efficacy of annual LM-NBI over CE in detecting gastric neoplasia.
