{"title":"Efficacy comparison between gastrectomy combined with chemotherapy and chemotherapy alone for treatment of advanced gastric cancer with abdominal para-aortic lymph node metastasis","authors":"Xue Wang, Jiqing Wang, Song Liu","doi":"10.3760/CMA.J.ISSN.1006-9801.2019.11.009","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1006-9801.2019.11.009","url":null,"abstract":"Objective \u0000To investigate the therapeutic effect and prognosis of gastrectomy combined with chemotherapy and chemotherapy alone for treatment of advanced gastric cancer with abdominal para-aortic (the 16th) lymph node metastasis. \u0000 \u0000 \u0000Methods \u0000The clinical data of 236 patients with advanced gastric cancer with abdominal para-aortic (the 16th) lymph node metastasis in Jincheng People's Hospital of Shanxi Province from January 2003 to December 2012 were retrospectively analyzed. According to the treatment method, the patients were divided into chemotherapy alone group (121 cases) and gastrectomy + chemotherapy group (115 cases), and mFOLFOX6 regimen was used for chemotherapy. The efficacy and adverse reactions of the two groups were compared. \u0000 \u0000 \u0000Results \u0000The incidence of upper gastrointestinal adverse events in the gastrectomy+chemotherapy group was higher than that in the chemotherapy alone group [8.7% (10/115) vs. 1.7% (2/121)], and the difference between the two groups was statistically significant (χ 2 = 3.881, P = 0.049). The median overall survival was 14.8 months (10.9-17.8 months) in the chemotherapy alone group and 11.7 months (9.8-15.6 months) in the gastrectomy+chemotherapy group, and there was no significant difference between the two groups (P > 0.05). The 2-year overall survival rate was 32.3% in the chemotherapy alone group and 24.3% in the gastrectomy+chemotherapy group, and there was no significant difference between the two groups (χ 2 = 3.105, P = 0.078). All patients underwent multi-disciplinary treatment(MDT)-directed conversion therapy, and D2 radical gastrectomy was performed after the disappearance of non-healing factors. The remission of patients after conversion therapy and the related intraoperative and postoperative indicators were analyzed. \u0000 \u0000 \u0000Results \u0000After the conversion therapy, there were 2 cases (8.7%) of complete remission, 13 cases (56.5%) of partial remission and 8 cases (34.8%) of stable disease. The remission rate of the conversion therapy was 65.2% (15/23). In all 23 cases, the surgical time was (135.5±32.1) min, the intraoperative blood loss was (356.7±101.3) ml, the number of lymph nodes resected was 35.5±10.3. The time of gastric tube indwelling was (70.5±29.6) h, the time to first liquid intake was (85.0±21.5) h, the time of drainage tube placement was (196.2±36.2) h and the postoperative hospital stay was (10.7±3.2) d. After surgery, two cases had grade ⅢA post-operative complications and 2 had gradeⅡ. At present, stress granules have been proved to be related to the occurrence and development of a variety of diseases, including tumors. The production of stress granules is promoted by microenvironment such as hypoxia and hyperactive oxygen in tumor cells, while stress granules-related proteins such as G3BP1, RACK1, YB-1 and mammalian target of rapamycin (mTOR) can promote the occurrence and metastasis of tumors, but the mechanism is not yet clear. In addition, studies have linked the formation of stress granules to the survival of tumor cells during chemotherapy, and believe that stress granules play a role in the treatment of tumors by different anti-tumor drugs. Among HPV-positive examiners, the infection rate of single genotype was higher than that of multiple genotypes infection [75.90% (400/527) vs. 24.10% (127/527), P 0.05]. Among 50 examiners with positive results of HPV and TCT tests, the top four genotypes were type 58, 52, 53 and 16, and they were in consistence with the overall HPV infection genotyping. \u0000 \u0000 \u0000Conclusions \u0000The HPV infection in the female physical examiners in Xi'an is dominated by the single genotype infection, and the main genotypes of HPV infection are type 52, 58, 53 and 16. Zhang, Zhen-jun Peng","doi":"10.3760/CMA.J.ISSN.1006-9801.2019.11.002","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1006-9801.2019.11.002","url":null,"abstract":"Objective \u0000To improve synchrony tracking components of CyberKnife (tracking vest and tracking markers) and to analyze the clinical application value of the improved tracking components in CyberKnife treatment of thoracic and abdominal tumors. \u0000 \u0000 \u0000Methods \u0000The tracking apron was made of knitted four-side elastic spandex cloth and suture design of Velcro, which was used to stick the tracking markers on the chest and abdomen of patients. The tracking markers added a 2 cm thick light foam block to the bottom of the original markers, and then the hook face of the Velcro was fixed to the bottom of the light foam. The improved trace component (the improved component) and the original component (the vendor component) were applied to the lung tracking treatment model, and the manufacturer components were included in the reference group. Adoption of improved components into the observation group; 20 different types of respiratory waveforms were simulated and applied to the same mold plan. After treatment, the coverage rate, mean standard deviation, maximum standard deviation and the slope of XYZ-axis vs. R correlation graph were recorded. The relevant parameters of Synchrony model and wearable time of two components were compared, and the application significances of the improved tracking component in the breathing tracking process of the CyberKnife were evaluated. \u0000 \u0000 \u0000Results \u0000The maximum slope [median(interquartile range)] of XYZ-axis vs. R related graph in the reference group was 0.73 (3.89), 0.27 (0.49) and 0.34 (1.02), respectively. The maximum slope of XYZ-axis vs. R related graph in the observation group was 0.70 (2.78), 0.31 (0.30) and 0.36 (0.75), respectively. There was no statistically significant difference in the slope of XYZ-axis vs. R between the reference group and the observation group (all P > 0.05). There was no significant difference in the average standard error and maximum standard error between the reference group and the observation group [(1.7±0.4) mm vs. (1.7±0.5) mm, t=-0.382, P= 0.710; (2.0±0.6) mm vs. (1.7±0.5) mm, t=-0.877, P= 0.401], and the difference of the model coverage rate between the two groups was statistically significant [(48±18)% vs. (60±22)%, t= 2.762, P= 0.042]. The setup time of tracking components in the observation group was less than that in the reference group, and the difference was statistically significant [(44±24) s vs. (81±15) s, t=-4.310, P= 0.001]. \u0000 \u0000 \u0000Conclusions \u0000The improved tracking components are comparable to the manufacturer tracking components in the standard error of the Synchrony model. As a highly efficient ferroptosis inducer, Erastin mediates ferroptosis through multiple molecules, such as cystine-glutamate transport receptor, voltage-dependent anion channel and p53. More importantly, Erastin can enhance the sensitivity of cancer cells to chemotherapy and radiotherapy, so it can be used as a new type of anticancer drug. All patients had no history of systemic application of hormone therapy, no history of diabetes, no history of radiation therapy, no history of metastasis of the jaw, and no history of infection. The average usage time of BP was 28 months. MRONJ occurred in 2 cases on maxilla and 4 cases on mandible. There were 2 patients with tooth extractions history in BP treatment. Clinical symptoms included maxillofacial pain, loosened teeth, fistula suppuration, and exposed sequestrum. Radiographic findings included osteolysis and bone sclerosis or the mixed manifestation of both, with or without periosteal reaction. In addition, nonhealing tooth sockets and sequestrum separation imaging were also included. \u0000 \u0000 \u0000Conclusions \u0000Tooth extraction is considered as an increased risk for MRONJ in patients with malignant bone metastases after BP therapy. MRONJ is more likely to appear in the mandible, but it can also appear in the maxilla. The study included training subset (6 162 images), verification subset (555 images), and test subset (617 images), which were performed in the artificial intelligence-assisted diagnosis model. The outcome results of test subset in diagnosis model were compared with the pathological results. The sensitivity, specificity and accuracy of the artificial intelligence-assisted diagnosis model were calculated. \u0000 \u0000 \u0000Results \u0000After the test of 617 images, the model diagnostic results could be automatically output with a rectangular frame indicating the nodule position, benign and malignant diagnosis, benign and malignant probability values. The diagnosis time was approximately 4 seconds for each nodule.
