{"title":"Evaluation Recovery of Ulcerative Colitis with a <i>Lactobacillus</i> Cocktail Derived from Traditional Dairy Products: <i>In vivo</i> Study.","authors":"Shaghayegh Rezai, Elnaz Ghorbani, Majid Khazaei, Seyedeh Elnaz Nazari, Farzad Rahmani, Hamideh Naimi, Asma Afshari, Amir Avan, Mikhail Ryzhikov, Saman Soleimanpour, Seyed Mahdi Hasanian Mehr","doi":"10.4103/abr.abr_157_24","DOIUrl":"https://doi.org/10.4103/abr.abr_157_24","url":null,"abstract":"<p><strong>Background: </strong>This investigation investigates the anti-inflammatory and fibrinolytic effects of a cocktail of probiotics derived from traditional dairy products in a murine model of ulcerative colitis (UC).</p><p><strong>Materials and methods: </strong>A mix of newly isolated probiotics containing <i>L. plantarum, L. brevis, L. delbrueckii</i>, and <i>L. helveticus</i> was characterized and orally administered to inbred eight-week-old C57BL/6 male mice (n = 6). Clinical symptoms, pathohistological changes, and inflammatory and fibrosis markers were analyzed in the existence and absence of probiotics in colitis mice.</p><p><strong>Results: </strong>Dairy <i>lactobacillus</i> probiotics potently attenuated colitis symptoms by decreasing dextran sulfate sodium (DSS)-induced body weight loss, colon shortening, rectal bleeding, and rectal prolapse. Consistently, a cocktail of probiotics could significantly improve histopathological grading by suppressing crypt loss, mucosal damage, and inflammation scores in colitis tissues. Moreover, the mix of probiotics suppressed pro-inflammatory genes including interleukin (IL)-1β, IL-6, tumor necrosis factor-alpha (TNF-α), and interferon-gamma (IFN-γ), and increased anti-oxidant markers and activity such as superoxide dismutase and catalase in colon tissue. Furthermore, compared to the no-treated group, the administration of probiotics reduced fibrosis by decreasing collagen deposition in tissue sections and down-regulating levels of pro-fibrotic genes including alpha-actin-2 (Acta2), collagen (Col) 1a1, and Col 1a2 in colitis tissue homogenates.</p><p><strong>Conclusions: </strong>The results show the newly isolated cocktail of probiotics elicits a potent protective effect on UC symptoms in mice model. Therefore, this study aimed to determine how adult obesity and meal-based DAQ scores correlate.</p><p><strong>Materials and methods: </strong>A cross-sectional study including 850 men and women in Tehran was conducted. Three 24-hour dietary recalls were used to evaluate dietary consumption. Based on the design of DAQs, the meal-based DAQ score was evaluated. Higher scores indicate better DAQ. The overall DAQ score varied from 0 to 5. To investigate the relationship between DAQs and obesity in each meal, logistic regression analysis was utilized.</p><p><strong>Results: </strong>There was no correlation between a higher breakfast DAQ score and being overweight or obese (for overweight: OR = 0.69; 95% CI: 0.38-1.24/for obesity: OR = 0.65; CI 95%: 0.31-1.37). In the basic model, a higher lunch DAQ score was inversely related to probabilities of being overweight (OR: 0.62; 95% CI: 0.39-0.99) Such a connection remained significant after relevant confounders were considered (OR: 0.48; 95% CI: 0.27-0.84). In both the raw and fully adjusted models, there was no discernible correlation between the meal DAQ score and overweight or obesity.</p><p><strong>Conclusion: </strong>We discovered that a higher DAQ score for lunch was substantially associated with a lower risk of being overweight but not obese. It has been suggested to add calcium channel blocking (CCB) drugs or magnesium sulfate (MgSO<sub>4</sub>) to normal care to decrease the release of acetylcholine (ACh) at the cholinergic synapse. Moreover, the diagnosis of OP poisoning is chiefly based on clinical evidence. Oximes and atropine are the recognized antidotes of OP. However, low-priced medications such as MgSO<sub>4</sub> and sodium bicarbonate (NaHCO<sub>3</sub>), as well as novel adjunct therapies, have been introduced recently. Furthermore, antioxidants are recommended for managing OP poisoning. In addition, hemoperfusion, fresh frozen plasma (FFP), and K-oximes are a number of innovative management modalities that deserve further evaluation. However, prevention seems to be the most effective management modality in this respect. Therefore, this study aimed to briefly discuss the controversies in OP poisoning management and present recent advances in its management and prognosis. This study aims to investigate the role of regulatory T-cells (Tregs), specifically the CD4+CD25+/high CD127-/low subset, in PsA pathogenesis, and their potential as biomarkers and therapeutic targets.</p><p><strong>Materials and methods: </strong>In a case-control study involving 40 PsA patients and 25 healthy individuals, CD4+ CD25+/high CD127-/low Tregs were analyzed in peripheral blood mononuclear cells (PBMCs) using flow cytometry. Disease activity was assessed using the Disease Activity in Psoriatic Arthritis (DAPSA) score.</p><p><strong>Results: </strong>We observed a significant positive correlation between Treg levels and the DAPSA score (<i>P</i> = 0.02) in non-treated PsA patients. Additionally, patient age showed a significant positive correlation with erythrocyte sedimentation rate in the same group (<i>P</i> = 0.04), emphasizing the potential influence of Tregs on disease activity and age-related effects on inflammatory markers in PsA.</p><p><strong>Conclusion: </strong>While not revealing significant differences in Treg populations, our research underscores the importance of considering specific Treg subsets in PsA. These subsets may respond differently to disease micro-environments and treatments, affecting disease progression. This study contributes to the broader comprehension of immune dysregulation in auto-immune diseases and suggests that further investigation into Treg subsets' function and count is warranted. The first group received 1 µg/kg dexmedetomidine over 10 minutes, followed by an infusion of dexmedetomidine at a rate of 0.5 µg/kg/h. The second group received 0.05 mg/kg midazolam, and the third group received 0.2 mg/kg slow IV etomidate. Hemodynamic parameters, sedation level, and adverse effects were recorded before anesthesia, during surgery, and during recovery.</p><p><strong>Results: </strong>The results of this study showed that in the 10<sup>th</sup> minute of surgery, the systolic blood pressure (SBP) in the etomidate group was significantly higher than the other groups <i>P</i> value = 0.029). The pulse rate (PR) in the etomidate group at the 15<sup>th</sup> minute during surgery, 10<sup>th</sup>, 20<sup>th</sup>, and 30<sup>th</sup> minute in the recovery period (mean 70.33 ± 10.34 bpm, 72.10 ± 10.18 bpm, 73.70 ± 10.18 bpm, and 75.03 ± 6.73 bpm, respectively) was significantly higher than the other two groups (<i>P</i> value < 0.05). No adverse effects such as dizziness, restlessness, vomiting, or nausea were observed in the midazolam group. However, decreased heart rate was significantly higher in the dexmedetomidine group (26.7%) compared to the etomidate (3.3%) and midazolam (6.7%) groups (<i>P</i> value = 0.021).</p><p><strong>Conclusion: </strong>According to the results of this study, the sedation level achieved by dexmedetomidine, midazolam, and etomidate was similar. Despite our limited understanding of the mechanisms driving this progression, identifying the key genes involved could significantly improve early detection and treatment.</p><p><strong>Materials and methods: </strong>Two gene expression profiles of GSE9750 and GSE6791, which included cervical cancer HPV-positive and -negative samples, were evaluated using the R limma package with established cut-off criteria of <i>P</i> value < 0.05 and | fold change| ≥ 1. KEGG pathway enrichment was performed to identify potential pathways. Weighted gene co-expression network analysis (WGCNA) was used to discover co-expressed gene modules and trait-module connections.</p><p><strong>Results: </strong>Considering the defined criteria, 115 differentially expressed genes (DEGs) were identified. The DEG's KEGG pathway enrichment analysis revealed enrichment in highly relevant pathways to the HPV infection, including cell cycle, viral carcinogenesis, autophagy-animal, Epstein-Barr virus infection, human T-cell leukemia virus 1 infection, and microRNAs in cancer. WGCNA results in 13 co-expression modules, and the magenta module is identified with significant relations to HPV, cervical cancer stage, and metastasis traits. The survival analysis identified <i>BEX1</i> and <i>CDC45</i> as potential prognostic factors in HPV-associated cervical cancer.</p><p><strong>Conclusion: </strong>The innovation of our work lies in identifying essential genes associated with the multi-step process of cervical carcinogenesis. In fact, the current study has the potential to give a distinct viewpoint on the molecular pathways linked to cervical cancer. Considering the potential side effects of chemical medications and the need for long-term drug administration, complementary treatments such as acupuncture, auriculotherapy, and herbal medicine have been considered. Hence, this study was designed to compare the effects of auriculotherapy, a herbal component (Aslagh capsule), and metformin on the clinical symptoms of PCOS.</p><p><strong>Materials and methods: </strong>This is a randomized controlled clinical trial study, which allocated 24 women, aged 18-43 years old, with oligomenorrhea in PCOs, in a 1:1:1 ratio, to three groups.</p><p><strong>Result: </strong>A decreasing pattern in the duration of the menstrual cycle was observed in all three groups, but this was significant only in the auriculotherapy group. There were no significant differences between the three groups in acne and hirsutism scores. However, there was a significant reduction in excess hair growth and density of hair in the auriculotherapy group compared to the other two groups (<i>P</i> < 0.05). Besides, in the auriculotherapy group, the reduction in thickness of excess hair at the end of the second and third months of treatment were marginally significant compared to the two other groups.</p><p><strong>Conclusions: </strong>Auriculotherapy and Aslagh capsule can be effective in reducing PCOS symptoms, similar to metformin. This study aimed to evaluate the biochemical, inflammatory and coagulation parameters in pregnant women with severe disease conditions (as one of the high-risk groups) as well as prognosis and outcome.</p><p><strong>Materials and methods: </strong>This cross-sectional study was performed on 135 pregnant women with COVID-19 admitted to ICU. Demographic and clinical information and laboratory parameters of the patients were evaluated and recorded at the time of admission and in the next follow-up until discharge or death in addition to the outcome and also the pregnancy outcome.</p><p><strong>Results: </strong>The mortality rate of pregnant women with COVID-19 was 9.6%. The mortality rate decreases with increasing Hb (OR (95% CI): 0.68 (0.47-0.99); <i>P</i> value = 0.043) and lymphocytes (OR (95% CI): 0.92 (0.85-0.96); <i>P</i> value = 0.028) and will increase significantly with increasing PT (OR (95% CI): 1.24 (1.01-1.51); <i>P</i> value = 0.037), INR (OR (95% CI): 1.89 (1.26-2.25); <i>P</i> value = 0.004), D-dimer (OR (95% CI): 1.68 (1.10-2.08); <i>P</i> value = 0.027), and LDH (OR (95% CI): 1.20 (1.01-1.61); <i>P</i> value = 0.010).</p><p><strong>Conclusion: </strong>According to the results of the present study, inflammatory factors such as leukocytes, neutrophils, NLR, CRP have an increasing and lymphocytes have a decreasing trend, so that lymphocytopenia is more common in non-survivors. In addition, increase of PT, INR, D-dimer and LDH and decrease of Hb were significantly associated with increased chance of mortality. The current study objective was to evaluate the efficacy and safety of jujube oxymel (JO) for treating CSU.</p><p><strong>Materials and methods: </strong>In this randomized double-blind controlled trial, 92 patients (aged 12-65) with CSU were randomly allocated to JO or placebo groups. They received 30cc of each syrup three times daily with 10 mg cetirizine for 28 days, subsequently taking 10 mg cetirizine alone for the next 4 weeks. Outcomes were evaluated using the weekly urticaria activity score (UAS-7) and Chronic Urticaria Quality of Life Questionnaire (CU-Q2oL).</p><p><strong>Results: </strong>After four weeks, the UAS-7 score mean and standard deviation in the JO group significantly decreased to 10.89 ± 4.87 compared to the placebo group at 15.06 ± 7.55 (<i>P</i>: 0.002). In the follow-up period, JO group participants achieved a score of 10.28 (4.67), while a significant increase occurred in the control group (18.33 ± 6.29) (<i>P</i> = 0.001). On day 28, there was a notable improvement in the quality of life within the JO group (<i>P</i> < 0.05). It influences the physiologic processes, such as the activation of the immune system and the antioxidant function. Melatonin has been reported in the samples of patients with periodontitis. Therefore, the role of melatonin in periodontal diseases must be appraised. Using the strategy of electronic search of various databases, we included studies, published until December 2021, measuring the expression of melatonin in patient samples and evaluating the effect of periodontal therapy on melatonin expression. This review also included studies evaluating the effect of melatonin supplementation on periodontal parameters. In total, 15 articles fulfilled the study inclusion criteria. The results revealed that melatonin is negatively correlated with the severity of periodontal diseases, and melatonin supplementation reduces the levels of periodontal inflammatory parameters.
