{"title":"Comparison of blunt and sharp dissection techniques during cesarean hysterectomy in placenta percreta.","authors":"Seyhun Sucu, Ibrahim Taskum, Furkan Cetin, Muhammed Hanifi Bademkiran, Ozge Komurcu Karuserci, Huseyin Caglayan Ozcan","doi":"10.5603/gpl.101542","DOIUrl":"10.5603/gpl.101542","url":null,"abstract":"<p><strong>Objectives: </strong>This study aimed to compare the effects of the blunt dissection technique (BDT) with finger and the sharp dissection technique (SDT) with scissors during cesarean hysterectomy (CH) in patients with placenta percreta.</p><p><strong>Material and methods: </strong>We included 70 patients with placenta percreta who underwent CH in a territory hospital between 2020 and 2023. The patients were divided into two groups: Group 1 included 34 patients who underwent blunt bladder dissection, and Group 2 included 36 patients who underwent sharp bladder dissection. Demographic data, operative complications, surgical parameters, and transfusion rates were compared between the two groups.</p><p><strong>Results: </strong>BDT was associated with a statistically significant reduction in operation time compared to SDT (95.62 ± 20.76 min vs 107.08 ± 26.04 min, p = 0.046). Moreover, the rate of bladder injury was significantly lower in the BDT group compared to the SDT group (11.8% vs 33.3%, p = 0.032). At preintervention and postintervention assessments, the glycometabolism, muscle sympathetic nervous system activity (MSNA) and cardiovascular parameters were measured and analyzed, containing fasting blood glucose (FBG), fasting blood insulin (FBI), 2 h glucose, glycosylated hemoglobin (HbA1c), homeostatic model assessment of insulin resistance (HOMA-IR), MSNA bursts/min, MSNA bursts/100 heartbeats, resting heart rate (HR); diastolic blood pressure (DBP), mean arterial pressure (MAP), systolic blood pressure (SBP), cardiac index (CI), cardiac output (CO), stroke volume (SV), total peripheral resistance (TPR) and total peripheral resistance index (TPRI).</p><p><strong>Results: </strong>Compared to aerobic group, the overweight or obese pregnant women in aerobic + resistance group showed lower FBG, FBI, 2 h glucose, HbA1c and HOMA-IR (p < 0.05). Moreover, both MSNA bursts/min and MSNA burst/100 heartbeats level were reduced in aerobic + resistance group compared to aerobic group (p < 0.0001). We aimed to investigate the effects of isolated posterior compartment prolapse on LUTS.</p><p><strong>Material and methods: </strong>The study was conducted as a prospective cross-sectional study with female patients with isolated posterior prolapse who presented with any LUTS. All patients were evaluated according to the POP-Q system. A total of 41 patients with stage 2-3 isolated pelvic organ prolapse were included in the study group. The control group consisted of a total of 41 patients without significant pelvic organ prolapse. Study and control groups were compared in terms of demographic data and UDI-6, IIQ-7, ICIQ-FLUTS, LUTS QoL, FLUT Sex scales.</p><p><strong>Results: </strong>The incidence of SUI, UI, frequency, nocturia, abnormal evacuation, difficulty in passage and vaginal farting in the study group was found to be statistically significantly higher than the control group. In the study group, the total scores on the UDI-6, IIQ-7, and LUTS QoL measures were significantly higher. While the total scores of the ICIQ-FLUTS Scale \"Filling\" and \"Incontinence\" subgroups were significantly higher in the study group, no significant difference was found in the \"voiding\" subgroup. However, there are many myths about breastfeeding. The aim of our study was to assess the knowledge of Polish women about breastfeeding.</p><p><strong>Material and methods: </strong>A cross-sectional survey study was conducted among 1536 Polish women. A self-administered questionnaire was created using Google Forms survey management software and distributed online. The participants were asked about the most common myths in society regarding breastfeeding.</p><p><strong>Results: </strong>The highest number of correct answers in all age groups were given to questions related to the nutritional value of breast milk (80.9-94.9%). The fewest correct answers in each age group were recorded to questions concerning the possibility of getting pregnant during the first 6 months of breastfeeding (16.1-35.3%), the safety of drinking non-alcoholic beer during lactation (24.4-37.1%), the benefits of brushing the breast while breastfeeding (16.0-37.1%), and the effectiveness of compresses made of cabbage leaves or sage infusions in relieving ailments during milk rush (6.8-12.4%). Higher education and being a mother were associated with a higher number of correct answers. Age below 25 years was associated with lower number of correct answers.</p><p><strong>Conclusions: </strong>The results of the survey regarding breastfeeding suggest the existence of various beliefs in the population of Polish women which are not evidence based. Our study aimed to identify and validate a novel gene signature for diagnosing endometriosis based on histone-related genes (HRGs), and to investigate their biological functions in endometriosis.</p><p><strong>Material and methods: </strong>RNA sequence data were downloaded from the Gene Expression Omnibus database, and HRGs were retrieved from the GeneCards database. We identified differentially expressed genes using the limma package, and constructed a diagnostic model using the rms package. Kyoto Encyclopedia of Genes and Genomes (KEGG) and Gene Ontology (GO) enrichment analyses were performed for visualization, annotation, and integrated discovery. Subsequently, we validated the model using the recall and decision curve analysis (DCA). Additionally, we analyzed the immune microenvironment features using CIBERSORT.</p><p><strong>Results: </strong>A total of 18 differentially expressed HRGs were identified in patients with endometriosis compared with controls. GO and KEGG enrichment was mainly in spindle organization, positive regulation of the cell cycle process, progesterone-mediated oocyte maturation, and cellular senescence and cell cycle. We obtained a signature of four HRGs (JUNB, FRY, LMNB1, and SPAG1). DCA revealed that the diagnostic model benefits patients with endometriosis, regardless of the incidence. This study investigated the predictive capacity of the systemic immune inflammatory index (SII) to assess the risk of PPH. The SII, which is predicated on cell types of integral to inflammatory processes, correlates with high values of increased inflammatory activity and potentially adverse prognoses.</p><p><strong>Material and methods: </strong>This retrospective cohort study included female patients aged 15-49 diagnosed with PPH who continued to bleed post-treatment, received transfusions of four or more units of erythrocyte suspension, or underwent laparotomy/re-laparotomy.</p><p><strong>Results: </strong>The study found that Hemoglobin, hematocrit, platelet, and SII values were significantly lower, and lymphocyte counts were elevated in the PPH group. Both univariate and multivariate analyses identified hemoglobin and SII levels as significant determinants of PPH. An SII cutoff value of 915 proved to be an effective predictor of PPH. In subjects with an SII ≥ 915, leukocyte, platelet, and neutrophil values were higher, and lymphocyte counts were lower. Furthermore, the rates of PPH and Intensive Care Unit admissions increased in this cohort.</p><p><strong>Conclusions: </strong>The findings suggest that the SII is a potentially significant marker for PPH risk prediction. The study evaluated the impact of anemia, a hemoglobin level lower than 12 g/dL, on the likelihood of cesarean section, labor duration, delivery time, and neonatal outcomes.</p><p><strong>Results: </strong>With 183 participants, findings indicate no significant difference in perinatal outcomes between anemic and non-anemic groups, suggesting that mild anemia may not significantly affect dinoprostone insert-induced labor outcomes.</p><p><strong>Conclusions: </strong>The study highlights the importance of a nuanced approach to managing anemia in pregnancy, emphasizing the necessity for individualized treatment strategies. The former seems safer. However, this superiority may change with the application of a new labor curve. Therefore, we aimed to compare the efficacy and safety of dinoprostone and double-balloon catheters (DBC) in promoting CR in induction of labor (IOL).</p><p><strong>Material and methods: </strong>A total of 877 primipara women with Bishop score ≤ 6 were divided into the dinoprostone group (n = 502) and DBC group (n = 375) according to the IOL way. The women in the dinoprostone group received dinoprostone to perform IOL, while those in the DBC group received DBC to perform IOL. The natural birth rate, time to labor onset and birth, and maternal and neonatal complications were compared between the two groups. A propensity score match (PSM) was used to eliminate the selection bias.</p><p><strong>Results: </strong>A total of 516 cases were left after PSM (1:1) to Bishop score. The dinoprostone was associated with an improved Bishop score. However, there were no significant differences in the vaginal delivery rate, the stage of labor, and the time from ripening to labor onset and delivery between the two groups (p > 0.05). The incidence rates of puerperal infection and blood loss were notably higher in the DBC group than in the dinoprostone group (p < 0.05). The procedure is crucial in preventing cervical cancer by enabling the early detection and treatment of abnormal cells. In the context of pregnancy, cytology remains an essential component of prenatal care. Numerous studies confirmed that cytology was safe for both the mother and the fetus when performed correctly. The interpretation of cytological results during pregnancy requires careful consideration due to physiological changes in the cervix, which may mimic or obscure pathological findings.
