Patients were divided into two groups, MTX/FA and MTX/F. Both groups received 50 mg/day MTX on days 1, 3, 5, 7 of treatment. On days 2, 4, 6, 8 of treatment 0.1 mg/kg folinic acid was administered to the MTX/FA group, and 20 mg/day folic acid was administered to the MTX/F group to reduce MTX toxicity. The groups were compared according to treatment success, resistance, and the development of toxicity.</p><p><strong>Results: </strong>Among 102 low-risk GTN patients included in the study, 64.7% (n = 66) were in the MTX/FA, while the remaining 35.3% (n = 36) were in the MTX/F group. There was no significant difference between MTX/FA and MTX/F groups regarding treatment success, resistance, and toxicity rates (84.8%, 13.6%, 1.6% vs 83.3%, 13.8%, 2.7%), respectively.</p><p><strong>Conclusions: </strong>Replacement of folinic acid with folic acid in low-risk GTN patients to reduce the toxicity that may develop associated with MTX treatment resulted in similar treatment success rates. Still, no significant change was observed regarding toxicity and resistance. {"title":"Strategies to reduce cesarean deliveries: surveying Polish obstetricians on external cephalic version practices.","authors":"Maisa Manasar-Dyrbus, Agnieszka Drosdzol-Cop, Szymon Stojko, Rafal Stojko, Jakub Staniczek","doi":"10.5603/gpl.102550","DOIUrl":"10.5603/gpl.102550","url":null,"abstract":"<p><strong>Objectives: </strong>The cross-sectional survey was conducted aiming to evaluate the knowledge and experiences of the Polish obstetricians and gynecologists regarding the External Cephalic Version (ECV) and investigate their practices concerning this procedure.</p><p><strong>Material and methods: </strong>An online survey constituting author-created questionnaire with 22 questions, was distributed among gynecologists and obstetricians. The questionnaire evaluated participants' knowledge about ECV, work experiences, and workplace practices.</p><p><strong>Results: </strong>Out of 461 respondents, 56.20% were specialists in gynecology and obstetrics. Elective cesarean section (CS) was preferred by 78.70% for primiparas and 73.50% for multiparas with non-cephalic presentation, while ECV would be chosen by 21.3% and 23.6%, respectively. While 73.80% knew centers performing ECV, only 16.70% had actively participated in the procedure. Major differences in the experiences and knowledge regarding ECV were observed based on work experience, and workplace reference level. Experienced physicians showed higher concerns about ECV complications and emergency CS risks. The most common concerns regarding the procedure referred to periprocedural pain, perceived low efficacy, and complications, and were more prevalent among respondents with longer experience and from lower-reference centers.</p><p><strong>Conclusions: </strong>The study demonstrated that among Polish obstetricians for term pregnancies with non-cephalic presentation, elective cesarean section is preferred over ECV, especially among experienced practitioners. Knowledge about ECV was relatively low, indicating a need for improved educational efforts. {"title":"Modified laparoscopic sacrocolpopexy for advanced posterior vaginal wall prolapse: a 3-year prospective study.","authors":"Yifan Yin, Yufang Xia, Shujun Ji, Enhui Guo, Chen Chen, Yanhui Lou","doi":"10.5603/gpl.100017","DOIUrl":"10.5603/gpl.100017","url":null,"abstract":"<p><strong>Objectives: </strong>To evaluate and validate the safety and efficacy of modified laparoscopic sacrocolpopexy for advanced posterior vaginal wall prolapse at up to 3 years of follow-up.</p><p><strong>Material and methods: </strong>As a prospective observational study, we collected 56 cases with advanced posterior vaginal wall prolapse and performed modified laparoscopic sacrocolpopexy (MLSC) with self-cut mesh. The main improvement is the cutting and fixing of the mesh. Patients were followed up at 6, 12, 24 and 36 months. The main indicators of follow-up were postoperative anatomic success rate and Pelvic organ prolapse quantitation (POP-Q) score, and secondary indicators were related to quality-of-life scales and postoperative complication rates.</p><p><strong>Results: </strong>All patients completed the operation through minimally invasive surgery, and there were no vital organs and blood vessel damage during the operation. The mean age was (58.32 ± 7.63) years. There was no recurrence of stage I or lower during the follow-up maximum of 36 months (median 24 months), and the anatomic success rate was 100%. The quality-of-life scores improved significantly (p < 0.001) and the quality of sexual life was not affected (p = 0.5). There was 1 case of continuous vaginal mesh exposure at 12 months (2.86%) and 1 case of severe infection with poor healing of vaginal stump within 6 months (1.79%). {"title":"Impact of body mass index and gestational weight gain on cesarean delivery rates: a comparative study of dinoprostone-induced vs spontaneous labor.","authors":"Andrzej Jaworowski, Agnieszka Micek, Magdalena Kolak, Katarzyna Skibinska-Frankowska, Julia Jurga, Kuba Ptaszkiewicz, Hubert Huras","doi":"10.5603/gpl.100230","DOIUrl":"10.5603/gpl.100230","url":null,"abstract":"<p><strong>Objectives: </strong>This study investigates the relationship between pre-pregnancy body mass index (BMI), BMI before labor, and weight gain during pregnancy with the incidence of cesarean delivery (CD) in dinoprostone-induced labor versus spontaneous labor.</p><p><strong>Material and methods: </strong>This retrospective analysis was carried out at the Jagiellonian University Hospital's Obstetrics and Perinatology Department, encompassing term singleton pregnancies from May 2019 to February 2021. BMI was categorized following WHO guidelines. Gestational weight gain was assessed against the Institute of Medicine's 2009 recommendations.</p><p><strong>Results: </strong>Of the 366 cases reviewed, 183 were in the dinoprostone-induced labor group, and 183 were in the spontaneous labor group. The study identified a significant association between higher pre-pregnancy BMI and increased weight gain during pregnancy with elevated CD rates, especially in dinoprostone-induced labor compared to spontaneous labor. Specifically, the dinoprostone-induced labor group showed a 33.9% CD rate compared to 16.9% in the spontaneous labor group. Logistic regression analysis further established that for each 1 kg/m² increase in pre-pregnancy BMI, the odds of undergoing a CD increased by 10%.</p><p><strong>Conclusions: </strong>Elevated pre-pregnancy BMI and excessive gestational weight gain significantly heighten the risk of cesarean delivery, particularly in induced labor. To describe a novel technique (Dogan technique).</p><p><strong>Material and methods: </strong>This is a retrospective cohort study. Between January 2021 and January 2023, patients who had central cystocele and rectocele higher than stage1 were included in the study. The Pelvic Organ Prolapse Quantification (POP-Q) score was used to determine the degree of the prolapsus. All cystocele and rectocele repair surgeries were performed by the same physician. The patients' voiding habits were assessed using ICIQ-SF and OAB-V8. Sexual function results were assessed with FSFI questionnaire before and after the operation. Transperineal ultrasonography was performed to examine mobility of the anterior and posterior compartments.</p><p><strong>Results: </strong>Total of 36 patients were diagnosed with grade 2 and above central cystocele (19, 52%) and rectocele (n = 17, 48%). After the operation the anatomical cure of anterior and posterior compartments was achieved for all patients in the two-years follow-up. According to voiding habits before the surgery, there were symptoms of stress urinary incontinence (SUI),urge urinary incontinence (URGE), both SUI and URGE, and no incontinence at the patients; 7 (36.8%), 14 (73.7%), 5 (26.3%), 3 (15.7%) respectively. Of those URGE patients (n = 5/14, 35.7%) incontinence symptoms were mixed-type. After the cystocele operation, significant improvement was seen in their voiding problems according to the ICIQ-SF and OAB-V8 questionnaires (p < 0.05). Innovative approaches, like sampling for HPV testing and cytology outside medical settings, have been proposed. This study assesses the feasibility and agreement of results between traditional and novel sampling methods.</p><p><strong>Material and methods: </strong>A cohort of 50 women aged 25-74 underwent HPV DNA and liquid-based cytology sampling both in-office using standard method and outside the medical setting by trained personnel. Samples were analyzed for HPV DNA using Real-Time PCR and cytology according to the Bethesda System.</p><p><strong>Results: </strong>Cytology and HPV DNA positivity rates showed substantial agreement between methods, with almost perfect agreement for high-risk HPV types. Visual assessment of the cervix was successfully conducted in all cases. Preliminary results suggest remote sampling for HPV DNA and cytology is a viable alternative to traditional methods, with the effectiveness in detecting HPV and cytological abnormalities comparable to this reported in literature, offering potential benefits for individuals with mobility limitations or logistical barriers to attending medical appointments.</p><p><strong>Conclusions: </strong>The study highlights the potential role of remote sampling for HPV DNA and cytology in enhancing cervical cancer screening accessibility and adherence. Implementation of such methods could improve coverage, particularly among underserved populations. Patients were randomized to either the Vcare UM or SecuFix UM group. Both groups were compared for operative time, colpotomy time, intrauterine manipulator insertion time, pneumoperitoneum maintenance, blood loss, complications, and length of hospital stay.</p><p><strong>Results: </strong>A total of 100 patients were enrolled, 50 with VcareUM and 50 with SecuFix UM. SecuFix UM demonstrated shorter colpotomy time (p < 0.001) and better pneumoperitoneum maintenance (p = 0.008) compared to VCare. VCare had shorter insertion time (p = 0.001) but a higher uterine rupture rate (p = 0.001). No significant differences were found in operative time, blood loss, complications, or length of hospital stay (p > 0.05).</p><p><strong>Conclusions: </strong>SecuFix may offer advantages in colpotomy and pneumoperitoneum maintenance during TLH compared to VCare. {"title":"mTOR inhibitor in the treatment of TFE-positive advanced maligmnant PEComa of the uterus: a case report and literature review.","authors":"Yaoxiang Zhong, Haikun Yang","doi":"10.5603/gpl.99247","DOIUrl":"10.5603/gpl.99247","url":null,"abstract":"<p><strong>Background: </strong>The pre- and intra-operative diagnoses of malignant uterine vascular perivascular epithelioid cell tumors (PEComas) can be challenging, for which the literature is limited. Some cases have been shown to have TSC gene mutations or rearrangements of the MiT factor family, resulting in variable responses to mTOR inhibitors. We report a case of a TFE-positive malignant PEComa of the uterus with pulmonary metastases that responded favorably to the mTOR inhibitor, everolimus.</p><p><strong>Case presentation: </strong>A 52-year-old female underwent a total hysterectomy 5 years ago for suspected sub-serosal or broad ligament fibroids. The intraoperative pathologic diagnosis was leiomyosarcoma of the uterus and the postoperative diagnosis was malignant PEComa of the uterus. The patient declined genetic testing and further treatment. In December 2020 the patient presented with a pelvic mass and underwent open abdominal mass resection and pelvic adhesiolysis. The pathologic findings confirmed recurrent malignant PEComa of the uterus. The pulmonary lesions gradually progressed during the follow-up period, so treatment with everolimus was initiated. Close follow-up evaluation for nearly 3 years showed disease remission without recurrence or progression.</p><p><strong>Conclusion: </strong>The patient described herein had a TFE-positive uterine malignant PEComa with lung metastasis and responded well to the mTOR inhibitor, everolimus. Long non-coding RNA (lncRNA) has been found to be of great value in the study of its pathogenesis and treatment. This study aimed to explore the expression and diagnostic value of lncRNA HCG18 in GDM.</p><p><strong>Material and methods: </strong>The expression levels of HCG18 in serum of participating GDM patients and healthy controls were detected by polymerase chain reaction (RT-qPCR). The correlation between the expression of HCG18 and the blood glucose level was clarified by the detection of blood glucose levels in GDM patients. The receiver operating characteristic curve (ROC) was used to evaluate the clinical diagnostic value of HCG18 for GDM. Furthermore, multivariate logistic analysis was used to verify the diagnostic value of HCG18 in GDM.</p><p><strong>Results: </strong>This study concluded that the expression level of HCG18 was upregulated in the serum of GDM patients compared with the control group. ROC curve showed that the AUC was 0.916, the sensitivity was 80.5%, the specificity was 90.2%, and multivariate logistic regression analysis verified that HCG18 (OR = 6.984, 95% CI = 3.751-13.005, p < 0.001) was significantly associated with GDM, which suggesting that HCG18 has diagnostic significance for GDM.
