{"title":"Microbiological profile of bacterial and fungal corneal ulcers at a national institute of ophthalmology in Perú.","authors":"R Navarro, E Ballesteros-Zamalloa, R Hualpa-Vicente, P Minami, M A Rodriguez-Cuba","doi":"10.1016/j.oftale.2025.03.017","DOIUrl":"10.1016/j.oftale.2025.03.017","url":null,"abstract":"<p><strong>Objective: </strong>To describe the microbiological profile of bacterial and fungal corneal ulcers, as well as the antibiotic susceptibility profile of the most frequent pathogenic species.</p><p><strong>Methods: </strong>A retrospective descriptive study was conducted using the recorded data of corneal ulcer scrapings from the ocular microbiology laboratory of the National Institute of Ophthalmology. Laboratory data from 2015 to 2022 were collected and included for all corneal ulcer scrapings.</p><p><strong>Results: </strong>After excluding repeated, non-cultured, and duplicate results, a total of 3,470 corneal samples were analyzed. Positive samples accounted for 88.1% of all scraps. Of these, 2,816 (81.2%), 904 (26.2%), and 662 (19.1%) showed bacterial, fungal, and mixed growth, respectively. Coagulase-negative Staphylococci (2,257; 69.8%) were the most frequent bacteria, with Staphylococcus epidermidis (1,677; 51.9%) being the most isolated organism, followed by Pseudomonas aeruginosa (275; 8.5%) and Staphylococcus aureus (274; 8.5%). Fusarium spp. was the most common fungus. Less than 60% of S. epidermidis isolates showed susceptibility to Oxacillin, Cefazolin, Moxifloxacin, and Gentamicin, while more than 90% of isolates were susceptible to Gatifloxacin. P. aeruginosa showed less than 80% susceptibility only to Imipenem.</p><p><strong>Conclusion: </strong>Gram-positive bacteria were the most isolated, with S. epidermidis being the most frequent pathogen, followed by P. aeruginosa. Classically, it produces choroidal folds, vascular tortuosity, and optic disc edema. However, in optical coherence tomography (OCT), it may also present as intraretinal cysts (IRC). We present the case of a patient who, 10 years after undergoing glaucoma surgery, developed ocular hypotony, IRC, and decreased visual acuity (VA). Given the patient's history of diabetes mellitus, it was initially diagnosed as diabetic macular edema (DME), which delayed appropriate treatment. We present 3 clinical cases of CSHRPE located in the macular region observed casually and diagnosed thanks to autofluorescence and especially optical coherence tomography, non-invasive tests. Galindo-Ferreiro","doi":"10.1016/j.oftale.2025.01.006","DOIUrl":"10.1016/j.oftale.2025.01.006","url":null,"abstract":"<div><h3>Objective</h3><div>To describe the demographic and clinical characteristics of thyroid orbitopathy (TO) in a population in Spain.</div></div><div><h3>Method</h3><div>It is a retrospective observational study of patients with TO treated at a tertiary hospital center. We gathered demographic data, ophthalmological symptoms, laterality, symmetry, recurrences, activity according to the Clinical Activity Score scale, severity according to the European Group On Graves’ Orbitopathy (EUGOGO), thyroid function and antibody levels. Statistical significance was defined for P values <0.05.</div></div><div><h3>Results</h3><div>156 patients with TO were included. 128 (82.1%) women, the female/male sex ratio was 4.57:1. The cumulative incidence was 7.47 cases per 100,000 inhabitants/year. The mean age was 50.7 ± 13.2 years. 70 (44.9%) patients were smokers. 62 (39.7%) patients had a family history of thyroid dysfunction. Hyperthyroidism occurred in 90 (57.7%) patients, hypothyroidism in 34 (21.8%) patients, and euthyroidism in 32 (20.5%) patients. Thyroid-stimulating immunoglobulin was elevated in 107 (68.5%) patients. The TO was bilateral in 149 (95.51%) and symmetrical in 99 (66%). Active TO occurred in 28 (17.9%) patients. 110 (70.5%) TO were mild, 41 (26.3%) moderate-severe and 5 (3.2%) very severe. Moderate-severe TO was higher in smokers compared to non-smokers (32.9% vs. 20.9%, <em>p</em> = 0.524).</div></div><div><h3>Conclusions</h3><div>TO in our population follows similar patterns to other countries: female sex, mild, bilateral and symmetrical. Druchkiv","doi":"10.1016/j.oftale.2024.07.012","DOIUrl":"10.1016/j.oftale.2024.07.012","url":null,"abstract":"<div><h3>Purpose</h3><div>To analyze the incidence, causes, risk factors and treatment of acute postoperative endophthalmitis (POE) after phacoemulsification in a private ophthalmological group.</div></div><div><h3>Design</h3><div>Uncontrolled retrospective observational study of all cases of POE over 22 years.</div></div><div><h3>Material and methods</h3><div>369,476 eyes were included after phacoemulsification in 41 surgical centers of the Clínica Baviera-AIER EYE Group from 2002 to 2023. POE cases were reviewed.</div></div><div><h3>Results</h3><div>The general group was divided into two: group A (2002–2007), with vancomycin in the intracameral irrigation flow (27,705 eyes); Group B (2008–2023), with intracameral cefuroxime (341,771 eyes). 31 cases of POE were found (incidence, 0.0084%); 5 in group A (0.018%) and 26 in group B (0.0076%) (<em>P</em> = .314), with a mean age of 67.3 years (14 men and 17 women). Cultures were positive in 14 cases in Groups A and B. Treatment was based on intravitreal antibiotics and vitrectomy. The mean time to symptom onset was 6.76 days and the mean time to resolution was 120.7 days. 12 eyes achieved corrected distance visual acuity ≥ 20/40.</div></div><div><h3>Conclusions</h3><div>The incidence of POEwas 0.0084% (0.0181% in group A and 0.0076% in group B). The probability of having POE in group A was 2.37 times greater than in group B (OR: 2.37; 95% CI: 0.71–6.2; <em>P</em> = .079). Lupión Durán","doi":"10.1016/j.oftale.2024.12.004","DOIUrl":"10.1016/j.oftale.2024.12.004","url":null,"abstract":"<div><h3>Antecedents and Objectives</h3><div>The purpose of our study was to showcase that MMS can be used efficiently even if it is not present (James Cook Hospital) in the management of periocular tumors through MMS via referral (Royal Victoria Infirmary Hospital) and to describe the clinical and histological features of our patients.</div></div><div><h3>Materials and methods</h3><div>We conducted a retrospective, longitudinal and descriptive study and 58 patients were included. We divided our patients in three groups; pre-COVID-19: patients from January 2019 to January 28th 2020, COVID-19: patients from January 29<sup>th</sup> 2020 to July 2021 and late COVID-19: patients from August 2021 to 24th of July 2023.</div></div><div><h3>Results</h3><div>There were no differences among each group between; the clinic date and the MMS procedure (p = 0,06), the referral date and the MMS procedure (p = 0,054), the MMS date to reconstruction (p = 0.19), the time of the lesion (p = 0,054). The type of reconstruction was not influenced by the number of days; between the clinic date and reconstruction (p = 0,60), the clinic date and the MMS procedure (p = 0,60), and the referral date and the MMS procedure (p = 0,27).
