These currents result from electrical gradients between ischemic and healthy zones, driven by differences in depolarization and repolarization. The diastolic current, directed toward healthy tissue, and the systolic current, oriented toward the injured region, explain phenomena such as TQ depression (diastolic current of injury) and QRS complex modifications accompanied by ST-segment deviations (systolic current of injury). Historical and contemporary studies have elucidated the underlying pathophysiology of these currents, emphasizing changes in radial myocardial activation, alterations in electrical vectors, and the potential presence of intramural conduction blocks. Furthermore, precise correlations between these electrocardiographic findings and acute ischemic events have been demonstrated in both experimental and clinical models. Understanding these currents not only facilitates the diagnosis of myocardial ischemia but also enhances the capacity for prompt intervention. Our aim was to assess changes in attitudes, knowledge, confidence, and care pathways of HF patients among physicians in Argentina after a structured educational intervention.</p><p><strong>Methods: </strong>Cross-sectional survey in 22 public and private health centers of physicians who participated in a comprehensive multi-module educational program (patient identification, classification/therapeutic options in advanced HF, structure/organization of the HF Day Hospital, education of patients in different stages/scenarios, and education of nursing staff in the management of patients at different stages/scenarios). Simple evidence-based roadmaps were created to address knowledge gaps.</p><p><strong>Results: </strong>The intervention improved physicians' confidence in the diagnosis of HF with preserved ejection fraction (p < 0.001), and prioritization of quadruple therapy for outpatients (p = 0.01) and patients with acute HF (p < 0.001). The proportion of \"confident\"/\"very confident\" physicians in identifying HF patients with reduced ejection fraction who could benefit from an implantable cardioverter-defibrillator increased significantly (p = 0.01), as did the relevance of hypertonic saline administration and intravenous iron infusions. A total of 34 questions will be given to consecutive patients aged 18 years or older through an online survey.</p><p><strong>Results: </strong>The data will be analyzed by country and region according to seven clusters: demographics, socioeconomic and educational level, cardiometabolic profile, cardiovascular interventions, medical follow-up and treatments, and COVID-19 vaccination status. The study will be conducted by the Prevention Council of the Inter-American Society of Cardiology.</p><p><strong>Conclusions: </strong>This study will provide insight into the impact of influenza and pneumococcus vaccinations in Latin American populations and the barriers preventing the immunization targets from being actualized. On the Internet it is possible to access free of charge databases created and maintained, for the most part, by research centers, universities and public organizations. Most Latin American journals are considered in regional bibliographic databases, favoring the regional exposure of research products. However, the great challenge is indexing in Medline, mainly due to its global impact. The quality, scientific relevance and innovation of the articles published in a journal marks its impact factor. {"title":"Difficulties in monitoring cardiometabolic risk in the chronic use of atypical antipsychotics for non-traditional indications, in a specialty hospital in Argentina: a qualitative study.","authors":"María N Vivacqua, María F Grande Ratti, José L Faccioli","doi":"10.24875/ACM.24000120","DOIUrl":"10.24875/ACM.24000120","url":null,"abstract":"<p><strong>Objective: </strong>To explore the barriers that hinder the proper monitoring of adverse effects in patients prescribed antipsychotics in a medical specialty hospital in Argentina.</p><p><strong>Method: </strong>A qualitative interpretive study of narrative design with ten semi-structured individual interviews with doctors from various specialties, conducted in outpatient clinics between May and November 2022, with an ad hoc guide based on a bibliography.</p><p><strong>Results: </strong>Barriers in interpersonal relationships emerged, highlighting the insufficient duration and infrequent consultations, communication problems between professionals and between professionals and their patients, and difficulties in teamwork. The obstacles were related to 1) limited direct communication; 2) communication via electronic medical records (without prior agreement or confirmation of receipt of messages); 3) use of patients as intermediaries for message exchange; 4) disregarding another professional's indications without communicating with them; 5) expressing differing criteria to the patient, delegating responsibility for treatment decisions to the patient; 6) assuming that antipsychotic monitoring is carried out by another professional; 7) hierarchies that hinder communication, and 8) experienced medical referents. These topics relate to inherent healthcare system issues, mainly the lack of available time and communication between colleagues as an unpaid task.</p><p><strong>Conclusions: </strong>Communication barriers imposed by the healthcare system can result in misunderstandings that lead to suboptimal health outcomes.
