This technique was first used for foreign body removal from the bladder.1 We previously described a practical application of this method for bladder diverticulectomy.2 A potentially significant role of T-LESS has been noticed by other investigators.3 We present a video of the use of this approach for simultaneous bladder diverticulectomy and cystolithotripsy.4 Materials and Methods: A total of 10 patients underwent the T-LESS diverticulectomies. They performed sequential dilatation up to 16F and used for pediatric population in 1997.1 In 1998, Jackman and colleagues performed 13F miniPNL first in adult patients with smaller sheaths and instruments.2 Today, the miniPNL is generally defined for the PNL procedure performed through the access tract smaller than 18F.3 Ultra-minimally invasive PNL (UMP) was first described by Desai and colleagues using a novel 6F mini nephroscope through an 11F to 13F metal sheath to perform holmium:YAG laser lithotripsy.4 Foreign bodies such as forgotten ureteral J stents especially upper parts of the stents can be removed through percutaneous access. In most cases, the conservative management is the first option. Surgical treatment is undertaken when the conservative management fails. The standard treatment is open surgery with ureter reimplantation. The objective of this video is to show the endoscopic dilatation of the POM as an alternative to the standard surgical treatment. Materials and Methods: We use a 10.5F Storz compact cystoscope with a 5F work channel under general anesthesia. A flexible guide is introduced in the ureter, and it is progressed up to the renal pelvis. A 3F–5F balloon catheter is passed over the guide and left positioned above and beneath the area to be dilated (the balloon's position is fluoroscopically controlled). The stenotic segment is dilated by inflating the balloon with a contrast medium up to a pressure of 12–14 atm, and the rupture of the problem area is verified by means of X-rays. As an alternative to traditional laparoscopy, robotics provides optical magnification and instrument articulation, which can be used in intricate dissection of the vessels and obtaining maximum length for the graft. We provide a reproducible 12-step instruction on our standard left RALDN and results of our initial experience. Methods: The patient is placed in the right lateral decubitus position and the robotic ports are placed. Numerous studies have since confirmed that the technique is efficacious with durable outcomes, and many investigators now consider HoLEP the new gold standard. The surgeon searches for the capsular plane by making two incisions at the 5- and 7-o'clock positions beginning at the bladder neck and ending at the verumontanum. It is important at this stage to proceed one layer at a time while opening the bladder neck until the circular fibers of the neck are exposed. The surgeon should then proceed from the neck to the verum, deepening and widening the incision by separating the lateral lobe from the median portion. Management can be challenging due to deep location in the pelvis and recent surgery. We present a case of a patient whose prostatectomy was complicated by unrecognized rectal injury and pelvic abscess. After diverting colostomy, he was referred for management of the resulting rectovesical fistula. The patient failed primary closure with a sliding rectal advancement flap. Methods: In this video, we demonstrate robotic repair of a rectovesical fistula. The fistula tract was first dissected posterior to the bladder. Due to the location of the fistula, a transvesical approach was also required to identify the distal end of the fistula. Once the fistula was completely separated from the bladder, it was primarily closed. An omental flap was then mobilized and secured over the fistula site.
